The purpose of this study was to estimate the prevalence of erectile dysfunction (ED) in Colombia, Ecuador, and Venezuela. A 49-item questionnaire was completed by 1946 men aged 40 years and older. The age-adjusted combined prevalence of minimal, moderate, and complete ED for all three countries was 53.4%, with 19.8% of all men reporting moderate to complete ED. Fifty consecutive patients with erectile failure who had previously proved refractory to papaverine and phentolamine intracavernosal therapy or were inappropriate candidates for such treatment were treated with intracavernosal prostaglandin E1. Forty patients (80%) achieved an erection sufficient for sexual intercourse and after a mean follow-up period of 5.9 months, 32 patients were continuing to use treatment successfully. A method is described to confirm vascular disorders in sexual impotence by a simple noninvasive screening technique. Doppler ultrasound signals are registered from superficial dorsal penile arteries and arteria radialis. The penile flow index (PFI) is determined by dividing the mean values of both measurements. An insufficient vascular support of the penis is assumed if the amplitude of the dorsal penile arteries is less than one third of that of the a. radialis. Identical Doppler signals are registered in superficial dorsal and deep cavernosal arteries.
