Diagnostic validity of somatosensory evoked potentials in subgroups of patients with sciatica.

The diagnostic utility of scalp-recorded somatosensory evoked potentials (SEP) in patients with sciatica has generally been regarded as low. The purpose of the present study was to determine the validity of sensory nerve SEP in different subgroups of sciatic patients. A total of 65 consecutive patients with sciatica showing disc pathology and/or facet joint hypertrophy on lumbar computed tomography (CT) and/or myelography were studied. Symptomatic myelographically compressed nerve roots were defined as truly compromised roots. Asymptomatic myelographically normal nerve roots were defined as tr…

Small-fiber dysfunction in trigeminal neuralgia: carbamazepine effect on laser-evoked potentials.

BACKGROUND: In patients with trigeminal neuralgia, results of clinical examination of sensory function are normal. Reflex and evoked potential studies have already provided information on large-afferent (non-nociceptive) function. Using laser-evoked potentials (LEP), the authors sought information on small-afferent (nociceptive) function. METHODS: The brain potentials evoked by CO(2)-laser pulses directed to the perioral and supraorbital regions were studied in 67 patients with idiopathic or symptomatic trigeminal neuralgia and 30 normal subjects. Of the 67 patients, 49 were receiving carbama…

Effect of sodium valproate on somatosensory evoked potentials in juvenile myoclonic epilepsy.

We analysed somatosensory evoked potentials (SEPs) in the patients with juvenile myoclonic epilepsy (JME) in order to find out if sodium valproate (VPA) affects SEP latencies and amplitudes. SEPs were studied in 23 patients with JME receiving VPA monotherapy, eight patients with JME not receiving VPA, and a control group consisting of 20 healthy subjects. The N20, P24, and N34 latencies bilaterally were significantly prolonged in the JME group receiving VPA as compared with the control group. In the untreated patients the P24, and N34 latencies bilaterally and the N20-P24 interpeak latency on …

Middle-latency auditory evoked potentials during induction of thiopentone anaesthesia in pigs.

A method is described for measuring middle-latency auditory evoked potentials (MLAEP) in consciously awake, non-sedated pigs during the induction of thiopentone anaesthesia (0.6 ml/kg, 2.5% thiopentone solution). It was done by using autoregressive modelling with an exogenous input (ARX). The ability to perceive pain during the induction was compared with (1) the changes in latencies and amplitudes of the MLAEP, (2) the change in a depth of anaesthesia index based on the ARX-model and (3) the change in the 95% spectral edge frequency. The pre-induction MLAEP was easily recordable and looked mu…

Longitudinal stability of visual evoked potentials in children and adolescents with hydrocephalus.

There were two purposes for this study: to determine the within-subject changes of visual evoked potentials (VEPs) over time, and to evaluate the usefulness of baseline VEP testing in the early diagnosis of increased intracranial pressure (ICP). Twenty children with hydrocephalus between the ages of 4 and 18 years (18 males, 2 females) were evaluated monthly for up to four months. Flash-evoked VEP tests were done in a standardized manner using a Cadwell Quantum 84 evoked potential stimulation and recording instrument. One participant had abnormal VEPs found during a previous episode of shunt m…

Evoked potentials in acute head injured patients with MRI-detected intracerebral lesions.

BACKGROUND: Magnetic resonance imaging (MRI) allows precise detection of intracranial lesions in head injured patients. We compared intracranial lesions detected in MRI to somatosensory evoked potentials (SEP) and brainstem auditory evoked potentials (BAEP) concerning their prognostic value. METHODS: Thirty patients with traumatic brain injury and prolonged recovery were studied. Size, side and number of 474 intra- and extraparenchymal lesions as well as lesion localisation based on a specific anatomical classification were entered into a database (a total of 7080 data). In addition, we recor…

Utility of brainstem auditory evoked potentials in children with spastic cerebral palsy.

In a retrospective study of 75 children with spastic cerebral palsy (CP), brainstem auditory evoked potentials (BAEP) were recorded and subsequently correlated with birthweight, gestational age, aetiology and type of CP, neuroradiological findings, additional impairments and disabilities (including the inability to walk independently). Seventeen patients (22.7%) had abnormal BAEP recordings. Thirteen of these 17 patients (76.5%) had spastic tetraplegia, 16 patients (94.1%) were full-term infants, 12 patients (70.6%) had myoskeletal problems, 9 (52.9%) had epilepsy, 16 (94.1%) had visual impair…

Reconsiderations about the abnormalities of somatosensory evoked potentials in motor neuron disease.

The frequency of involvement of sensory pathways in motor neuron disease (MND) remains the matter of controversy. For this reason the purpose of the present work was to test how often sensory system involvement might be detected by somatosensory evoked potentials (SEP) studies and then to verify the presence of alteration of the sensory conduction and to detect the frequency of abnormalities of somatosensory peripheral, spinal, subcortical and cortical potentials in MND. SEP were tested after median nerve stimulation at the wrist, recorded from Erb's point, Ce2, Ce7 and scalp. Pearson's correl…

Serial visual evoked potentials in 90 untreated patients with acute optic neuritis.

To establish the value of visual evoked potentials (VEPs) for monitoring disease evolution, we undertook a population-based study of 90 untreated patients 12 to 57 years of age (median, 32 years) at the onset of optic neuritis (ON) and after 2, 4, 12, and 52 weeks. Optic neuritis was monosymptomatic (AMON) in 58 patients and part of the clinically definite multiple sclerosis (CDMS) in 32 patients. The VEP was abnormal in eyes with acute ON in 69 (77%) of 90 patients at onset and in 80 (89%) of 90 patients at one or more of the follow-up sessions. In eyes with acute ON, normalization of an init…

Motor and somatosensory evoked potentials in tuberculous meningitis: a clinico-radiological correlation.

OBJECTIVE: The sensory and motor functions in severe tuberculous meningitis (TBM) may be difficult to assess clinically and may be helped by evoked potential studies. Lack of motor and somatosensory evoked potential studies in TBM prompted the present study. METHODS: All the patients with TBM underwent detailed neurological evaluation and cranial CT scan study. Motor and somatosensory evoked potentials to both upper and lower limbs were carried out bilaterally. The outcome was defined on the basis of 3 month Barthel Index (BI) score into good (BI > 12) and poor (BI < 12). RESULTS: Forty-one …

Somatosensory evoked potentials after posterior tibial nerve stimulation--normative data in children.

We report normative data of somatosensory evoked potentials to posterior tibial nerve stimulation from 47 children 4-15 years of age. We recorded near-field potentials from the peripheral nerve, the cauda equina, the lumbar spinal cord and the somatosensory cortex. Far-field potentials were recorded from the scalp electrodes with a reference at Erb's point and on the earlobe. The near-field potentials N8 (peripheral nerve) and P40 (cortex) were present in all children. N20 (near-field from the cauda equina) was recorded in 38 subjects. N22 (near-field from the lumbar spinal cord), P30 and N37 …

Brain functions of an infant with hydranencephaly revealed by auditory evoked potentials.

Hydranencephaly is a severe condition of brain characterized by the almost complete absence of the cerebral hemispheres with the thalamus, cerebellum and brainstem being preserved. This paper reports hydranencephaly in a 3-month-old boy. Cranial magnetic resonance imaging (MRI) revealed the condition and an auditory evoked potential study which was performed in order to investigate residual brain function. They showed normal auditory brainstem function but no appearance of other auditory evoked potentials originating from the higher brain.

Normal postexercise facilitation and depression of motor evoked potentials in postpolio patients.

We studied the effects of exercise on motor evoked potentials (MEPs) elicited by transcranial magnetic stimulation in healthy subjects and postpolio patients. Subjects performed repeated sets of isometric exercise until the muscle fatigued. In both groups, the mean MEP amplitude immediately after each exercise set was approximately twice that of the baseline amplitude, indicating similar postexercise facilitation, and after fatigue was approximately half that of the baseline amplitude, indicating similar postexercise depression. We conclude that the intracortical component of central fatigue i…

Middle latency auditory evoked potentials in congenitally blind and normal sighted subjects.

Middle latency auditory evoked potentials were recorded in two groups of ten subjects each, viz, congenitally blind (CB) and age-matched subjects with normal vision (NV). The age range for both groups was 13 to 16 years. The CB group subjects had peripheral deficits, with absence of visual evoked responses. The peak latency of the Nb wave (the maximum negativity between 38 and 48 ms) was significantly lower in the CB group compared to NV group (p < .05, one-tailed, two factor ANOVA, Tukey test). In addition to these recordings from the vertex, recordings were also made from occipital areas, to…

Temporal recovery of auditory evoked potentials in individuals at risk for alcoholism.

The present investigation examined auditory evoked potential (AEP) recovery functions in both high-risk (HR, N = 23, mean = 22.3) and low-risk (LR, N = 27, mean = 23.0) males. A series of binaural auditory stimuli, with randomly interposed interstimulus intervals (ISIs) of 0.5, 1.0, and 10.0 s, were used to elicit the N1 and P2 components of the AEP. Scalp potentials were recorded from the 19 electrodes comprising the 10/20 International System. For purposes of statistical analysis, five electrode groups were created: frontal (F), central (C), parietal (P), occipital (O), and temporal (T). The

Somatosensory evoked potentials predict neurologic outcome in full-term neonates with asphyxia.

OBJECTIVE: To investigate the ability of somatosensory evoked potentials (SSEP) to predict neurologic outcome in term neonates with birth asphyxia. METHODOLOGY: Upper limb SSEP were performed on nine infants of 1-7 weeks of age who had perinatal asphyxia and an encephalopathy still present at 7 days of age. Comparison was made between the cranial ultrasound, electroencephalogram (EEG), SSEP and neurologic outcome at 9-36 months. RESULTS: Normal SSEP were found in four infants, all of whom were normal on neurologic follow up at 9-12 months. Neonatal EEG performed on two out of four of these i…

Somatosensory evoked potentials and jerk-locked EEG back-averaging in myoclonic epilepsy.

Median nerve somatosensory evoked potentials (SEPs) and jerk-locked EEG back-averaging (JLA) were performed in 2 patients with left-hand myoclonus. Both SEPs were characterized by a normal parietal N20 and an enhanced N27-P27 fronto-parietal complex, but the giant central N40 described by most authors was only observed in case 1. Similarily, the aspect of JLA clearly differed in both patients. We hypothetize that both patients exhibited signs of an increased drive of the motor cortex by inputs coming from the sensory cortex. This increased drive could be mediated by the cortico-cortical connex…
