Safety and efficacy of resistance exercise in prostate cancer patients with bone metastases.

BACKGROUND: Due to concerns of fragility fracture, exercise is a perceived contraindication for prostate cancer patients with bone metastases. These patients experience significant functional impairment and muscle atrophy, which may lead to an increased likelihood of skeletal complicaTIOns (i.e., pathological fracture, bone pain) and/or falls. Safe resistance exercise prescription may counteract this effect. The aim of this feasibility trial was to determine the safety and efficacy of resistance exercise by prostate cancer survivors with bone metastatic disease. METHODS: Twenty men with estab

Randomised controlled trial of partitioned aerobic exercise training using one-leg cycling in patients with idiopathic pulmonary fibrosis.

BACKGROUND: Although many individuals with idiopathic pulmonary fibrosis (IPF) are referred for pulmonary rehabilitation (PR), their breathlessness can be so severe as to prevent their tolerating the most effective aerobic training intensity for adequate time. Ventilation is less with one-leg cycling and a meaningful exposure to intensity can be achieved. This study aimed to determine if one-leg training improved exercise tolerance more than two-leg training in individuals with stable IPF. METHODS: This was a prospective randomised controlled trial comparing one-leg versus two-leg cycle exerc…

Effectiveness of exercise training in people with non-cystic fibrosis bronchiectasis with and without COPD.

BACKGROUND AND AIM: Non-Cystic Fibrosis (CF) bronchiectasis is associated with reduced exercise tolerance and symptoms such as dyspnea and fatigue, impairing functional capacity and limiting physical activity. In addition to airway clearance techniques, pulmonary rehabilitation (PR), including aerobic and resistance training, is recommended in these individuals. This retrospective, multicentric study covering a ten-year period compared in non-CF bronchiectasis people with and without COPD the effectiveness of PR, including exercise training, on exercise tolerance (primary objective) and other …

Exercise Intolerance Is Associated with Cardiovascular Dysfunction in Long COVID-19 Syndrome.

Background/Objectives: Cardiorespiratory complications are commonly reported among patients with long COVID-19 syndrome. However, their effects on exercise capacity remain inconclusive. We investigated the impact of long COVID-19 on exercise tolerance combining cardiopulmonary exercise testing (CPET) with resting echocardiographic data. Methods: Forty-two patients (55 ± 13 years), 149 ± 92 days post-hospital discharge, and ten healthy age-matched participants underwent resting echocardiography and an incremental CPET to the limit of tolerance. Left ventricular global longitudinal strain (LV-GL…

Diaphragmatic fatigue and high-intensity exercise in patients with chronic obstructive pulmonary disease.

Patients with chronic obstructive pulmonary disease (COPD) are at a mechanical disadvantage and should be predisposed to the development of diaphragmatic fatigue when the ventilatory system is stressed by exercise. The purpose of this study was to determine whether patients with moderately severe COPD develop contractile fatigue of the diaphragm after cycle exercise to the limits of tolerance. Twelve male patients with COPD, age 61.4 +/- 3.0 yr, participated. Their forced expiratory volume in 1 s (FEV(1)) was 1.79 +/- 0.14 L, 49.6 +/- 3.4% of predicted. Patients cycled at 60-70% of their prede

Acute neuromuscular and short-term recovery responses to low-load exercise with individualized blood flow restriction in elite male volleyball players.

Blood flow restriction (BFR) training is widely used to augment the effects of low-load resistance exercise, yet the influence of individualized BFR intensity on acute neuromuscular responses and early recovery remains incompletely understood, particularly in elite athletes. This study examined acute neuromuscular, morphological, and tolerability responses to low-load leg press exercise performed under individualized BFR conditions in elite male volleyball players. Thirty-three athletes completed four repeated exercise sessions at 0%, 40%, 60%, and 80% of arterial occlusion pressure. Vastus la

Restoration of the pulmonary valve reduces right ventricular volume overload after previous repair of tetralogy of Fallot.

BACKGROUND: Severe pulmonary regurgitation (PR) and associated right ventricular (RV) dilatation are late complications of surgical repair of tetralogy of Fallot (TOF). For the past several years, we have restored pulmonary valve competence with the exclusive use of cryopreserved allografts. METHODS AND RESULTS: Sixteen patients with symptoms of diminished exercise tolerance and echocardiographic evidence of progressive PR with severe RV dilatation underwent placement of allografts in the RV outflow tract at a median age of 12 years (10 years after TOF repair). Abnormal exercise tolerance tes…

Oxygen supplementation increases the total work and muscle damage markers but reduces the inflammatory response in COPD patients.

INTRODUCTION: Oxygen supplementation (O2-Suppl) is recommended for pulmonary rehabilitation with higher exercise intensities. However, high-intensity exercise tends toward muscle damage and a greater inflammatory response. We aimed to investigate the effect of O2-Suppl during exercise test (EET) on CRP level and muscle damage (CPK, LDH, lactate) in non-hypoxemic COPD patients. METHODS: Eleven non-depleted patients with COPD (FEV1 65.5 ± 4.3 %) performed two EET (room-air or O2-Suppl-100 %), through a blind, randomized, and placebo-controlled crossover design. CPK, LDH and CRP were measured be

Exertional dyspnoea in obesity.

The purpose of cardiopulmonary exercise testing (CPET) in the obese person, as in any cardiopulmonary exercise test, is to determine the patient's exercise tolerance, and to help identify and/or distinguish between the various physiological factors that could contribute to exercise intolerance. Unexplained dyspnoea on exertion is a common reason for CPET, but it is an extremely complex symptom to explain. Sometimes obesity is the simple answer by elimination of other possibilities. Thus, distinguishing among multiple clinical causes for exertional dyspnoea depends on the ability to eliminate p…

Fontan Patients with a Systemic Left Ventricle Have Greater Exercise Capacity than those with a Systemic Right Ventricle: A Systematic Review and Meta-Analysis.

Cardiopulmonary exercise testing (CPET) is a useful metric to track the functional capacity and prognosis in patients with Fontan circulation. Systemic ventricular morphology may influence CPET interpretation. This systematic review and meta-analysis assessed the impact of systemic ventricular morphology on CPET outcomes. PubMed, Embase, and Cochrane were searched from inception through December 2024. Inclusion criteria were (1) Fontan circulation, (2) CPET via treadmill or cycle ergometer, and (3) outcomes stratified by systemic ventricle. Extracted outcomes included peak VO2, O2-pulse, VE/VC

Cardiopulmonary exercise test to detect cardiac dysfunction from pulmonary vascular disease.

BACKGROUND: Cardiac dysfunction from pulmonary vascular disease causes characteristic findings on cardiopulmonary exercise testing (CPET). We tested the accuracy of CPET for detecting inadequate stroke volume (SV) augmentation during exercise, a pivotal manifestation of cardiac limitation in patients with pulmonary vascular disease. METHODS: We reviewed patients with suspected pulmonary vascular disease in whom CPET and right heart catheterization (RHC) measurements were taken at rest and at anaerobic threshold (AT). We correlated CPET-determined O2·pulseAT/O2·pulserest with RHC-determined SV
