UNLABELLED: The effect on ventilatory function of fixation of a flail segment in internal (FIP) and external (FEP) position and oxygen administration was studied in an experimental flail chest with pleural indemnity. Variations of tidal volume (TV), respiratory rate (RR), minute volume (MV), and arterial blood gases are reported. These parameters were measured in nine dogs in control and flail conditions (FC). The effect of FIP, FEP, and oxygen administration were studied.
RESULTS: Significant differences were found: TV decreased from control values to FC and from FC to FIP, but increased fro

Although severe intra-articular fractures of the distal radius are often treated with external fixation/distraction, little attention has been devoted to the amount and duration of fixator distraction required to maximize outcome. To determine these effects, we conducted a retrospective study of 26 patients whose primary treatment was external fixation/distraction. Patients were evaluated by chart review, questionnaire, x-ray films, and physical examination an average of 104 weeks after injury. The carpal height index was used to quantify distraction. Scores for pain, function, wrist motion, a

The dynamic ASIF-BM external fixator incorporates telescopic tubes for axial dynamization and angled bayonet screws to keep the tubes parallel to the bone axis, that is essential for gliding, and adjustment of rotation. The results after treatment of 70 type II and III open tibial fractures with static ASIF and dynamic ASIF-BM frames were analysed. Of the fractures, 32 were treated with the delta ASIF static frame, the other 38 with delta and uniplanar dynamic ASIF-BM frames. The groups were found to be comparable. The results of the study confirm that tubular external fixation is a safe metho

A prospective study of 59 patients with Grade II or III open tibial shaft fractures compared internal and external fixation. Bony stabilization was with plating by AO principles or with external fixation with the one-half pin technique, prospectively randomized. In 12 cases, minimal internal fixation of the tibia and external fixation were combined. Definitive wound closure was delayed in all cases. Three free-flap transfers and two gastrocnemius myoplasties were done; vascular injury necessitated three early limb amputations. Fifty-six patients were followed for at least one year. Five plate

Femoral fractures in children are mostly managed conservatively. The recommended methods are overhead traction, 90/90 degree traction on a special table (Weber) and skeletal traction in a Thomas splint or on a Brown's frame. This treatment proves cumbersome in most multiply injured children, especially in those with head injury. Due to uncooperative behaviour the fracture is difficult to hold in an acceptable position. Therefore, operative treatment has been favoured by several authors in the past. Over a period of 8 years, 167 adolescent multiple trauma victims were treated in our institution

The effect of external skeletal fixation on blood supply and healing of the tibia was investigated in the New Zealand white rabbit. Bilateral osteotomies were made in ten rabbits. One osteotomy was stabilized with a small unilateral external fixator and the other with a conventional long-leg cast. The animals were killed at various intervals from one to ten weeks. Microsphere infusions indicated a greater blood supply in the cast-treated osteotomies after five weeks. Histologic examination showed that external fixation alters the proportions of periosteal and medullary ossification in the heal

The general question of the influence of fracture stability on bone healing remains unanswered and has important bearing on fracture stabilization by external fixation. The stiffness of an external fixator is dependent on pin placement and frame orientation. These parameters are under the surgeon's control, within limits set by soft tissue injury and fixator design. Fixator configuration parameters include common fragment pin separation, common fragment pin angle, common fragment pin number, effective pin length, use of transfixing pins versus half-pins, and use of two versus three connecting

INTRODUCTION: The definitive management of fractures is not completely unhazardous, with fracture-related infection (FRI) being one of the complications. The main objective of this study was to determine the prevalence of FRI in patients with closed femur fractures treated with initial external fixation followed by definitive fixation at a reference hospital.
MATERIALS AND METHODS: A cross-sectional observational study was conducted over a seven-year period. Patients aged 18 to 65 years with closed diaphyseal femoral fractures were included. FRI was defined according to the international cons

OBJECTIVES: Pilon fractures are among the difficult injuries to treat in orthopedic surgery. We aim to evaluate the feasibility, advantages, and disadvantages of temporary external fixation for pilon fractures and compare its outcomes with cases managed with internal fixation and primary open reduction.
METHODS: In a prospective trial, 30 patients were divided into two cohorts: a two-stage cohort with external fixation and secondary ORIF (15 patients) and a one-stage primary ORIF cohort (15 patients). We compared the two cohorts' rates of infection (deep or superficial infection), non-union,

BACKGROUND: Modern external ring fixation has been hypothesized to reduce complications requiring hospital readmission compared with internal fixation when treating patients with high-energy open tibial shaft fractures. In this study, the 1-year probability of a major limb complication was compared between external and internal fixation of severe open tibial fractures.
METHODS: This multicenter randomized clinical trial included patients 18 to 64 years of age with severe open tibial shaft fractures randomly assigned to either modern external ring fixation (n = 127) or internal fixation (n = 1

BACKGROUND: Mid-shaft clavicle fractures are common and may require surgery. The objective of this study in adults with high-risk mid-shaft clavicle fractures was to evaluate the clinical and radiological outcomes of a minimally invasive surgical technique involving a minimally invasive approach, fracture reduction, temporary intra-operative external fixation, and locking plate internal fixation.
HYPOTHESIS: This minimally invasive surgical technique for mid-shaft clavicle fractures ensures satisfactory radiographic fracture healing and medium-term functional outcomes, with a short immobilisa

INTRODUCTION: In the staged management of tibial pilon fractures, overlap between definitive internal fixation and external fixation pin sites has been investigated as a risk factor for infection with equivocal conclusions. Our aim was to determine if overlap or proximity of definitive internal fixation to external fixation pin sites influences the risk of deep infection.
PATIENTS AND METHODS: We reviewed 280 AO/OTA 43B or 43C type distal tibia fractures in 277 patients at two level-one trauma centers. Patients underwent staged management using early temporizing external fixation followed by

Background The proximal tibia with the meta-diaphysis junction is a critical weight-bearing area. An injury around this region may be restricted to the tibia or associated with a significant soft-tissue injury. The objective of the present study is to assess the results of closed reduction and Ilizarov external fixation in the management of complex tibial plateau fractures. Patients and methods The study included 26 patients with high-energy tibial plateau fractures (Schatzker types V and VI). The ages ranged from 23 to 60 years, with an average of 35 years. The trauma was a road traffic accid

The use of locking plates with intramedullary rodding in children with osteogenesis imperfecta reduces rotational and longitudinal instability. However, adjunctive locking plates do not allow early weight-bearing. We explored the outcomes of intramedullary rodding combined with external fixation or titanium threaded wires (ThW). The study included osteogenesis imperfecta children who underwent surgery with intramedullary telescopic rods and adjunctive fixation with external fixation or ThW. The timing of and reason for implant failure were primary outcome variables. Our cohort consisted of 62

BACKGROUND: External fixation remains an accepted option for certain unstable distal radius fractures, particularly when ligamentotaxis and … mplication of external fixator removal from small-diameter long bones. When removing wrist-spanning external fixators, particularly in older patients or those with decreased bone quality, clinicians should consider disassembling the construct and removing metacarpal pins intact rather than cutting pins under constraint. When pin cutting is unavoidable, support of t …

Background/Objectives: Tibial plateau fractures (TPFs) are commonly associated with complex patterns requiring advanced surgical strategies. High-energy trauma often results in severe soft tissue damage, complicating surgical outcomes. Despite advancements in soft tissue management, postoperative complications such as surgical site infections (SSIs) remain prevalent, with rates ranging from 9.9% to 30%. This study aims to analyze risk factors and surgical approaches influencing acute SSIs following TPF fixation. Methods: A retrospective analysis was conducted on 365 patients treated for TPFs w
