Erythema Ab Igne in a 12-Year-Old Boy Diagnosed via Telemedicine.

In March of 2020, an otherwise healthy 12-year-old boy developed a unilateral patch of reticulated erythema limited to his left lower extremity. The child could not be examined in the clinic due to limited in-person appointments during the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic, so he was examined via a telemedicine visit. The diagnosis of erythema ab igne was made as his mother verified that the child was spending approximately two hours per day playing video games in the cold basement of his house, with a space heater positioned close to his left leg. Our case

Mansonella ozzardi: a neglected New World filarial nematode.

Mansonella ozzardi (Nematoda: Onchocercidae) is an understudied filarial nematode, originally described by Patrick Manson in 1897, that can be transmitted by two families of dipteran vectors, biting midges (most of them members of the genus Culicoides) and black flies (genus Simulium). With a patchy geographic distribution from southern Mexico to northwestern Argentina, human infection with M. ozzardi is highly prevalent in some of the Caribbean islands, along riverine communities in the Amazon Basin, and on both sides of the border between Bolivia and Argentina. There is no clinical entity un

Getting stung by black scorpion Androctonus crassicauda: a case report.

Importance of the correct diagnosis in the correct early management of a scorpion stung patient by using antivenom is not emphasized, particularly when there are little evidences. A 65-year-old female was brought to our emergency department with the chief compliant of being stung by an unknown object 3 h earlier while traveling in an intercity bus. She became agitated and simultaneously experienced tachycardia, very severe generalized sweating, cold and wet extremities, bilateral diffuse crackle in the base of lungs, tachypnea, and lethargy. With the primitive diagnosis of myocardial infarctio

Septo-optic dysplasia: fitting the pieces together.

A 7-year-old boy was admitted for a general tonic-clonic seizure with severe hypoglycaemia (1.39 mmol/l). His medical history was remarkable for a congenital left eye strabismus, unilateral cryptorchidism and three previous episodes of hypoglycaemic seizures with inconclusive metabolical and neurological investigations. Physical examination revealed a hoarse tone voice, dry skin, cold extremities and height in the third percentile (target height between 50th and 85th percentile). Left wrist radiography revealed a bone age of 4.5 years ±6 months Laboratory studies confirmed growth hormone defic

Retroperitoneoscopic drainage of complicated psoas abscesses in patients with tuberculous lumbar spondylitis.

PURPOSE: Nowadays, endoscopic techniques are widely used in surgical procedures. Retroperitoneoscopy has been an extremely valuable tool for a wide variety of urologic disorders, whereas, it has limited use in orthopedic procedures. METHODS: We performed retroperitoneoscopic drainage (in combination with medical treatment) of complicated psoas abscess on 12 patients with tuberculous spondylitis. All the procedures were done under general anesthesia and in the lateral decubitus position. Psoas abscess was evacuated during procedure, and postoperatively, drainage was continued through a large s

Novel application of percutaneous cryotherapy for the treatment of recurrent oral bleeding from a noninvoluting congenital hemangioma involving the right buccal space and maxillary tuberosity.

Cryotherapy is the application of varying extremes of cold temperatures to destroy abnormal tissue. The intent of this article is to describe a novel technique using percutaneous cryotherapy for treating a noninvoluting congenital craniofacial hemangioma (NICH). An 18-year-old woman with type 1 von Willebrand's disease, as well as a qualitative platelet aggregation disorder, presented with multiple recurrent episodes of oral bleeding from a NICH involving the right buccal space and maxillary tuberosity. The patient was initially treated with a combination of endovascular particulate embolizati

Office evaluation and treatment of Raynaud's phenomenon.

BACKGROUND: Raynaud's phenomenon, an episodic vascular disorder induced by cold temperatures or stress and characterized by white, blue, and red discoloration of the fingers and toes, may affect up to 20% of the general population. KEY POINTS: Raynaud's phenomenon may exist independently (primary) or in association with an underlying disease (secondary), most commonly systemic sclerosis. The pathophysiologic features include vasospasm, endothelial cell changes, vessel obstructive features, and hemorrheologic factors. Raynaud's phenomenon is the initial manifestation of disease in 70% of patie

Raynaud's phenomenon of occupational origin.

Vibration delivered to the hand and arm by industrial pneumatic tools is a common cause of vascular and neurovascular problems, including cold-induced vascular spasm (vibration white finger) and peripheral neuropathies with paresthesias, dysesthesias, and sensory abnormalities. A decade ago, the US Public Health Service estimated that 1.2 million American workers were at risk. Differentiation of primary and secondary Raynaud's phenomenon from the thoracic outlet syndrome and from the carpal tunnel syndrome pose potential diagnostic difficulties. Despite growing public recognition of upper extr

Extending Preservation in Pediatric Kidney Transplantation: First Contemporary Experience With Hypothermic Oxygenated Machine Perfusion.

INTRODUCTION: Hypothermic machine perfusion currently represents the gold standard preservation modality for kidneys retrieved from adult deceased donors, with reduced delayed graft function (DGF) rates and improved early transplant survival compared to static cold storage (SCS). To date, clinical experience with hypothermic oxygenated machine perfusion (HOPE) in pediatric kidney transplants (KT) setting remains extremely limited. METHODS: We conducted a single-center retrospective analysis of pediatric KT recipients preserved using HOPE. Between November 2019 and December 2023, 129 pediatric

Cervical Staphylococcus aureus Infection after Receiving the Third Dose of COVID-19 Vaccination: A Case Report.

INTRODUCTION: Vaccination is one of the most effective ways to control the COVID-19 pandemic. However, as the number of people vaccinated against COVID-19 continues to increase, there are more reports on the safety of vaccines. So far, there have been no reported cases of spinal infection associated with COVID-19 vaccination. Recently, we admitted a patient who developed cervical Staphylococcus aureus infection resulting in high paraplegia after receiving the third dose of COVID-19 vaccine when the symptoms of cold did not completely disappear. CASE PRESENTATION: The patient was a 70-year-old

Allodynia after lateral-medullary (Wallenberg) infarct. A PET study.

We used PET to study regional cerebral blood flow (rCBF) changes in nine patients with unilateral central pain after a lateral medullary infarct (Wallenberg's syndrome). All patients presented, on the abnormal side, a combination of hypaesthesia to noxious and thermal stimuli and allodynia to rubbing of the skin with a cold object (i.e. abnormal pain to innocuous stimulation). The rCBF responses during allodynia were compared with those obtained during stimulation of the normal side using (i) a cold non-noxious stimulus identical to that applied to the painful side, and (ii) an electrical high

Autochthonous Plasmodium falciparum malaria in a high-altitude, non-endemic region of Ethiopia: a case report.

BACKGROUND: Malaria transmission in Ethiopia is traditionally limited to altitudes below 2000 m. Debark Town, situated at 2850 m, is a high-altitude highland fringe area characterized by a cold climate and has historically been considered a non-endemic zone. This report describes a rare, locally acquired case of Plasmodium falciparum in this extreme environment. CASE PRESENTATION: A 15-year-old Ethiopian female of Amhara ethnicity resident in Debark Town presented with a 5-day history of high-grade fever (39.5 °C), headache, and vomiting. Significantly, she reported no history of travel to kn

Compensation of the effects of temperature on a motor system in the crab, Cancer borealis.

Understanding how warming affects life-critical physiological processes is increasingly important as ocean temperatures rise at unprecedented rates. We study the stomatogastric system of the crab Cancer borealis. Movement depends on multiple temperature-sensitive processes, from motor pattern generation to neuromuscular transmission and contraction. Previous studies examined these levels separately and showed that warming increased pyloric rhythm frequency until the rhythm crashed at extreme temperatures. Here, we examined how the neuromuscular transform responds to this increase in motor neur

Sustainable bioremediation of undiluted matured compost leachate and enhanced lipid partitioning in Desmodesmus sp.: a zero-dilution approach for circular bioeconomy.

The economic feasibility of microalgal biodiesel remains constrained by the high cost of synthetic nutrients, driving interest in waste-derived cultivation systems. This study investigates the mixotrophic growth of Desmodesmus sp. using 100% undiluted matured compost leachate as a dual-purpose medium for biomass production and wastewater remediation. To decouple microalgal assimilation from bacterial degradation, a strictly sterile abiotic control was employed, enabling accurate attribution of nutrient removal pathways. The system achieved bioremediation efficiencies of 82.6% chemical oxygen d

Impact of Menstrual Cycle and Oral Contraceptives on Haematological and Inflammatory Biomarkers in Highly Trained Female Athletes.

Haematological and inflammatory biomarkers play an important role in athlete performance and health, with some of them used in the fight against doping. However, little is known about how they are modulated by sex hormone fluctuations in highly trained female athletes. We therefore measured the haematological parameters monitored in the athlete biological passport (ABP) as well as erythropoietin, serum markers of iron and inflammatory statuses (iron, ferritin, transferrin, transferrin saturation, albumin, creatinine, total protein, interleukin-6 and TNF-alpha) in 20 highly trained female athle

Right-Predominant Pulmonary Edema After Complete Avalanche Burial: Two Cases and Mechanistic Considerations.

Avalanche burial occasionally precipitates pulmonary edema, yet its pathogenesis remains poorly understood. Two male back-country skiers (aged 39 and 53 years) were completely buried for 30 min and 20 min, respectively, by the same avalanche, and immobilized in a right-lateral posture without safety devices. They arrived 2 h post-extrication, conscious but hypothermic (32.4 °C and 34.2 °C, respectively), tachycardic, tachypneic, and hypoxemic (SpO2 88% and 83%, respectively). Chest radiography and computed tomography demonstrated extensive infiltrates or ground-glass opacities confined predomi

Genomic inbreeding analysis reveals resilience and genetic diversity in Indian yak populations.

The yak (Bos grunniens), renowned for its adaptability to extreme cold and hypoxic conditions, stands as a remarkable domestic animal crucial for sustaining livelihoods in harsh climates. We conducted a comprehensive analysis of the whole genome sequence data from three distinct Indian yak populations: Arunachali yak (n = 10), Himachali yak (n = 10), and Ladakhi yak (n = 10). The genomic data for Indian yaks were meticulously generated by our laboratory and compared with their Chinese counterpart, the Jinchuan yak (n = 8), for a more nuanced understanding. Our investigation revealed a total of…

From pole to pole, life-long research of sleep in extreme environments.

In November 1965, Michel Jouvet accepted me into his laboratory in Lyon as a medical student at a time when sleep research was an adventure. After 4 years of investigations in cats, I obtained my medical doctorate. Being a military physician, I was posted to Antarctica for wintering over and was initiated by Jean Rivolier into the psychology of small isolated human groups. I recorded 180 polysomnographic (PSG) nights in eight of my companions. This was my first contribution to research on human sleep under extreme environments and conditions. I then entered René Hénane's military thermophysiol
