Intestinal fistulization following acute pancreatitis is a complication of abscess formation and may occur after initial surgical drainage. It should be suspected in anyone with protracted pancreatitis in whom an abdominal mass suddenly disappears or in whom gastrointestinal bleeding develops. Although transient improvement may occur, decompression will often be incomplete and will usually be followed by recurrent sepsis or severe life threatening hemorrhage. The tenth reported case of congenital urethral fistula with chordee was repaired by a 1-stage procedure. The ventral intact prepuce was used to form a tubular pedicle flap from which the distal portion of the new urethra was constructed. The proximal neourethra was fashioned from a tubular pedicle flap based at the proximal fistulous orifice. A patient with recurrent urinary vaginal fistula involving bladder, bladder neck, and urethra was treated by transpubic approach. This afforded excellent exposure for careful repair of the urologic defects. Difficulty with ambulation during the first few weeks postoperatively may be related to removal of larger segment of pubic bone than is usually reported.
