Fixation for distal radius fractures.

Distal radius fractures are common fractures that can cause significant disability. As techniques and implants have improved, better results can be expected from internal and external fixation of complex wrist fractures. With meticulous technique, articular alignment can be secured and small articular fragments can be replaced in anatomic locations through limited open or arthroscopic techniques. Bone graft should be used liberally in comminuted articular fractures. Such procedures are demanding, however. Familiarity with the techniques described in this article will enhance the surgeon's abil

Comparison of different internal fixation implants in the treatment of talar neck fractures: A finite element analysis.

OBJECTIVES: This study aimed to analyze the biomechanics of cannulated screws (CS) with or without a lateral locking plate (LLP) in talar neck fractures through a finite element analysis. PATIENTS AND METHODS: The computed tomography image of the talus from a healthy volunteer (adult male) was used to reconstruct a three-dimensional talar model. The method for fixing talar neck fractures with CS and an LLP was planned using computer-aided design software. Afterward, the three-dimensional models of comminuted talar neck fractures were used to simulate fixation with anteroposterior parallel dua

Mandibular Angle Fractures.

Angle fractures are the most common among the mandibular fractures. History and physical examination are crucial in guiding time course and specifics of management. Computed tomography (CT) has become the gold standard for diagnosis of mandible fractures, offering advantages for both surgical planning and assessing dental involvement. Currently the use of a single monocortical plate with the Champy technique for osteosynthesis is used preferentially for noncomminuted fractures of the mandibular angle. Other load-sharing options for plating include strut plates, malleable plates, and geometric

Treatment of Traumatic Depressed Compound Skull Fractures.

BACKGROUND: A skull fracture widely occurs in patients with traumatic brain injury, leading to intracranial hematoma, brain contusion, and intracranial infection. It also influences the prognosis and death of patients. This study aimed to discuss cases of patients with comminuted skull fractures. METHODS: From October 2015 to December 2018, 38 patients with comminuted skull fractures were admitted to the hospital. All patients underwent three-dimensional reconstruction of computed tomography scan images. Digital subtraction angiography or magnetic resonance venography was performed to find ou

Long-term outcomes of total elbow arthroplasty for distal humeral fracture: results from a prior randomized clinical trial.

BACKGROUND: Total elbow arthroplasty (TEA) is a reliable treatment for elderly patients with comminuted intra-articular distal humeral fractures. However, the longevity and long-term complications associated with this procedure are unknown. The objectives of this study were to examine long-term outcomes and implant survival in elderly patients undergoing TEA for fracture. METHODS: Patients from a previously published randomized controlled trial of 42 patients in which TEA was compared with open reduction-internal fixation (ORIF) were followed up long term. Patients were aged 65 years or older

Quantitative biomechanical analysis of fracture patterns in ipsilateral femoral neck and shaft fractures: an in-silico study.

INTRODUCTION: Ipsilateral femoral neck and shaft fractures (IFN-SFs) caused by high-energy trauma pose a significant risk of complications related to bone healing. Prompt identification of fracture types and maintenance of fracture fixation stability can mitigate this risk. This study employed finite element analysis to evaluate biomechanical parameters for the stability of fixation in IFN-SFs and quantify differences in biomechanical stability among various fracture types. METHODS: Patient-specific femur models were constructed from computed tomography data of 10 individuals. Simulating comb

Strategy for ipsilateral multiple concomitant upper extremity fractures in an elderly polytraumatized patient with severe osteoporosis: A case report.

RATIONALE: Elderly patients with severe osteoporosis present significant challenges in managing multiple concomitant upper extremity fractures, especially when high-energy trauma is involved. The rarity of such cases necessitates a structured approach to achieve optimal outcomes, balancing surgical intervention and early rehabilitation. PATIENT CONCERNS: A 79-year-old woman presented to the emergency department following a severe car accident. She had multiple concomitant fractures, including those of the proximal humerus, proximal ulna, and open fractures of the radius in the ipsilateral upp

Forearm bone mineral density as a predictor of reduction loss in distal radius fractures treated with cast immobilization.

OBJECTIVE: Many potential predictors have been identified and proposed for predicting late reduction loss in distal radius fractures. However, no report exists on whether the bone mineral density (BMD) of the forearm correlates with the loss of reduction in distal radius fractures. This study aimed to investigate whether forearm BMD can be used as a predictor of reduction loss in distal radius fractures treated with cast immobilization. METHODS: Ninety patients with distal radius fractures were divided into two groups according to the maintenance or loss of reduction evaluated from radiograph

External fixation for phalangeal and metacarpal fractures.

From 1987 to 1993 we treated 33 patients with 29 phalangeal and seven metacarpal fractures by external fixation using a mini-Hoffmann device. There were 27 open and 25 comminuted fractures. In 12 patients one or more tendons was involved. The mean follow-up was 4.4 years. Complications occurred in ten fractures; two required repositioning of the fixator. All the fractures healed. The functional results after metacarpal fractures were better than those after phalangeal fractures and fractures of the middle phalanx had better recovery than those of the proximal phalanx. Twenty-eight of the 33 pa

[Current treatment standard for patella fractures in Germany].

BACKGROUND: The treatment of patella fractures is technically demanding. Although the radiological results are mostly satisfactory, this often does not correspond to the subjective assessment of the patients. The classical treatment with tension band wiring with K‑wires has several complications. Fixed-angle plate osteosynthesis seems to be biomechanically advantageous. OBJECTIVE: Who is treating patella fractures in Germany? What is the current standard of treatment? Have modern forms of osteosynthesis become established? What are the most important complications? Comparison of surgical techniques of 111 medial malleolar fractures classified by fracture geometry.

BACKGROUND: Evaluation of operative techniques used for medial malleolar fractures by classifying fracture geometry has not been well documented. METHODS: One hundred eleven patients with medial malleolar fractures (transverse n = 63, oblique n = 29, vertical n = 7, comminuted n = 12) were included in this study. Seventy-two patients had complicating comorbidities. All patients were treated with buttress plate, lag screw, tension band, or K-wire fixation. Treatment outcomes were evaluated on the basis of radiological outcome (union, malunion, delayed union, or nonunion), need for operative re

Locked intramedullary nailing for displaced tibial shaft fractures.

We analysed the results of 93 tibial shaft fractures treated with the Grosse-Kempf locked nail. Twenty-six fractures were comminuted, 19 were open grade I to II, and 54 were located outside the middle third of the tibia. The deep infection rate was 3.2%. There were only two poor results. The use of this method is recommended and discussed.

Fractures of the phalanges of the hand in children.

Fractures of the phalanges of the hand were studied in 203 children. Fingertip fractures caused disability for three weeks but usually had good results. Comminuted fractures often took the form of longitudinal splitting. Epiphyseal fractures were mostly of the Salter type 2 and metaphyseal types and usually had good results: remodelling occurred in both planes at the base of the proximal phalanx but only in the sagittal plane at the base of the middle phalanx. Epiphyseal fractures at the base of the distal phalanx often had unsatisfactory results. Fractures of the neck of phalanx showed no rem

Management of Clavicle Implant Failure With Olecranon Bone Graft: A Case Report.

Bone grafts have been important treatment tools in reconstructive orthopaedic surgery for years for traumatic osseous defects, defects resulting from resection of bone tumours, arthrodesis, and osteotomy procedures. We used an autologous olecranon bone graft to fill the bone gap due to fracture comminution in a patient with right clavicle implant failure. A 21-year-old male initially underwent primary fixation with a clavicle locking plate for a comminuted middle-third right clavicle fracture. At the follow-up visit after four weeks, the patient complained of pain over the right shoulder. Repe
