Extended applications of the endoscopic modified Lothrop procedure.

OBJECTIVE: The endoscopic modified Lothrop procedure is mainly used for refractory frontal sinusitis. However, we have used it as an access procedure to facilitate treatment for an extended range of additional frontal sinus pathologies. METHODS: A retrospective review of patients who underwent the endoscopic modified Lothrop procedure for 'alternative' frontal sinus pathologies was conducted. Patient data were reviewed. The main outcome parameter measured was signs of recurrence. RESULTS: Twelve patients (6 males, 6 females) from a 7-year study period, with a mean age of 45.2 years (range, 1…

Radiological correlation between pneumatization of frontal sinus and height of fovea ethmoidalis.

BACKGROUND: We need more data about the variations of skull base to minimize the complications of ethmoidectomy. The aim of this study was to analyze the relationship between the pneumatization of the frontal sinus and height of the fovea ethmoidalis. METHODS: Paranasal coronal computed tomography (CT) scans of 487 sides of 300 patients were evaluated. The presence of the frontal cell and pneumatization of the frontal sinus were studied with respect to the height of the fovea ethmoidalis. RESULTS: We found a statistically significant correlation between the frontal cell and frontal sinus hyp

Subdural Empyema Complicating a Giant Fronto-Ethmoidal Osteoma.

Osteomas of the paranasal sinuses rarely lead to intracranial manifestations. We present an unusual case of a giant frontal sinus osteoma leading to subdural empyema formation. Determine the origin and the optimal surgical approach of these unusual lesions by analyzing giant osteomas of the frontal and ethmoidal sinuses in the literature. We report a rare case of giant frontoethmoidal osteoma with intracranial extension in a 34-year-old man, revealed by seizures. Neuroradiological studies revealed frontoparietal subdural empyema associated to a large osteoma in the right frontal sinus. The pat

Pneumosinus Dilatans frontalis: a case of incidental autopsy diagnosis.

Pneumosinus Dilatans Frontalis (PDF) is a rare pathologic condition characterized by expansion and hyper aeration of the frontal sinuses. This abnormality has been described in otolaryngology, ophthalmology, neurology, and radiology journals, while only a few autopsy studies are reported in the English literature. The etiology of this condition remains unclear, although multiple theories have been proposed. Clinically, PDF is often an asymptomatic condition, but it can lead to significant bone deformation with esthetic complaints, and even severe associated neurological deficits. In the follow

Pneumosinus dilatans: a sign of intracranial meningioma.

BACKGROUND: Pneumosinus dilatans affecting the sphenoid and posterior ethmoid sinuses has been reported in association with spheno-orbital meningiomas and is believed by some authors be a sign of an adjacent meningioma. METHODS: We report the case of a 57-year-old man who developed progressive neurologic signs and symptoms consistent with a frontal lobe lesion. Neuroimaging studies revealed a large partially cystic mass at the base of the anterior cranial fossa that appeared to be invading the left frontal lobe and that was associated with pneumosinus dilatans of the adjacent left frontal sin

The safety of frontal sinus fat obliteration when sinus walls are missing.

The dilemma of the management of a severely fractured frontal sinus when portions of the walls are missing presents a number of therapeutic options. The safety of placing a fat graft in such a sinus for obliteration when the graft will be required to obtain its nourishment from the scant vessels contained within the subcutaneous tissue or dura, or worse still, from a dural or bone graft is questioned. Clinical experience has suggested that such a procedure may not be safe. An experiment was performed in nine cats in which all of either the anterior or posterior frontal sinus wall was removed i

Endoscopic endonasal surgery--concepts in treatment of recurring rhinosinusitis. Part I. Anatomic and pathophysiologic considerations.

Many years of endoscopic investigation and observation proved that most infections of the paranasal sinus are rhinogenic, spreading from the nose into the sinuses. The common focus of infection in cases of recurring sinusitis is the stenotic areas of the anterior ethmoid, with infection recurring in the larger sinuses. The anterior ethmoid, especially its infundibulum, is thus a key location for infection or cure, and maxillary as well as frontal sinuses are fully dependent on the pathophysiologic conditions there. Histologic examination demonstrates that massive changes of the nasal glands ar

[Complications of sinusitis].

The complications of sinusitis are essentially secondary to ethmoidal and frontal sinusitis, occurring in patients weakened, particularly in case of immunodeficiency or anatomical defects. The gravity is due to the risk of spreading infection in intracranial tissues and orbital cavity. The diagnosis is always to discuss any symptomatology resistant to treatment and the appearance of orbital or neurological signs. The scanner and MRI contribute greatly to the orientation and diagnostic evaluation of the extent of the lesions. Treatment is based on prolonged antibiotic therapy and surgery and re…

Frontal mucocele with intracranial extension causing frontal lobe syndrome.

PURPOSE: Mucoceles are mucus-containing cysts that form in paranasal sinuses; although mucoceles themselves are benign, this case report highlights the extensive damage they can cause as their expansion may lead to bony erosion and extension of the mucocele into the orbit and cranium; it also presents a rarely reported instance of frontal sinus mucocele leading to frontal lobe syndrome. A thorough discussion and review of mucoceles is included. CASE REPORT: A 68-year-old white man presented with intermittent diplopia and a pressure sensation in the right eye. He had a history of chronic sinus

[Dynamic imaging of the nasal cavity and the paranasal sinuses with polarized 3helium MRI].

PURPOSE: Reduced or blocked ventilation of the paranasal sinuses is probably the most important factor in the development of sinusitis. Recently, the use of optically polarized noble gas isotopes has attracted increasing interest for use in a variety of promising MR applications. The aim of this study was to test the feasibility of imaging and visualization ventilation of the nasal cavity and paranasal sinus in MR by inhalation of hyperpolarized (3)helium. The goal was to evaluate ventilation defects of the paranasal sinuses. VOLUNTEERS AND METHODS: Three volunteers were enrolled in the study

Gas monitoring in human sinuses using tunable diode laser spectroscopy.

We demonstrate a novel nonintrusive technique based on tunable diode laser absorption spectroscopy to investigate human sinuses in vivo. The technique relies on the fact that free gases have spectral imprints that are about 10.000 times sharper than spectral structures of the surrounding tissue. Two gases are detected; molecular oxygen at 760 nm and water vapor at 935 nm. Light is launched fiber optically into the tissue in close proximity to the particular maxillary sinus under study. When investigating the frontal sinuses, the fiber is positioned onto the caudal part of the frontal bone. Mul

Sinus barotrauma leading to extradural muco-pneumocephalus.

An amateur scuba (Self Contained Underwater Breathing Apparatus) diver experienced an acute frontal headache following a low-depth dive. After 12 hours of worsening and unremitting headache, CT and MRI imaging were performed; these revealed gas and mucous in the frontal sinus and extradural space. This appears to be the first case of MRI-documented barotrauma of the frontal and ethmoidal sinuses resulting in extradural pneumo- and muco-cephalus.

BACKGROUND: Stenosis or occlusion of the nasofrontal duct with subsequent recurrent frontal sinusitis or mucocele formation may require osteoplastic surgery in combination with frontal sinus obliteration. METHODS: To study its suitability as an implant material in sinuses, granular ionomeric cement was applied for frontal sinus obliteration in 15 cats. The frontal sinus specimens were processed for histological studies after 1, 3, 6, 12, and 24 months, respectively. To obtain quantitative data from the histological sections, areas of osteoid, connective tissue, and implanted ionomeric cement …

Unexpected brain finding in pre-autopsy postmortem CT.

A case is presented in which pre-autopsy postmortem computed tomography (PMCT) revealed an unexpected brain abscess with a related frontal sinusitis and an erosion of the posterior wall of the frontal sinus. PMCT findings enabled the forensic pathologists to adapt protective measures during autopsy and protect their health from infection. Pre-autopsy PMCT has been also useful in the early differential diagnosis procedure. The complementary use of postmortem imaging and autopsy can improve the quality of forensic death investigations.

Revision endoscopic frontal sinus surgery.

Frontal sinusitis following functional endoscopic sinus surgery may represent persistent, recurrent, or iatrogenic disease. The narrow frontal recess is unforgiving of technical errors, and surgery within its confines requires ample training, skill, and patience. Revision surgery in this area, compromised by scarring and long-standing mucosal disease, demands even more of the endoscopic sinus surgeon. While successful treatment of this condition is a formidable task, strict attention to principles of proper medical and surgical therapy can increase the chances of a favorable outcome.
