OBJECTIVE: To look for factors relating to the vertigo control and hearing changes after intratympanic injections of gentamicin (GM).
STUDY DESIGN: Prospective.
SETTING: Tertiary referral medical center.
PATIENTS: Twenty-eight patients with intractable Ménière's disease.
INTERVENTIONS: Three intratympanic injections of GM (once per day for three consecutive days).

Single-shot transtympanic gentamicin therapy has become a popular treatment modality for Meniere's disease despite the known possible ototoxic properties of this drug. OBJECTIVE: To determine whether the prostaglandin E1 analogue, misoprostol, could preserve renal function in dogs receiving nephrotoxic doses of gentamicin.
ANIMALS: 12 (6/group) healthy sexually intact male dogs.

Septic complications after surgery for enterogenous peritonitis are minimized by adjuvant antibiotics effective against aerobes and anaerobes. Historically, "gold standard" therapy included an aminoglycoside plus clindamycin, the latter given at 600 mg intravenous piggyback (IVPB), every 6 hours. TLC G-65, a liposome-encapsulated gentamicin, was given intravenously twice weekly for 4 weeks to AIDS patients with Mycobacterium avium-M. intracellulare complex (MAC) bacteremia at 1.7 mg of gentamicin per kg of body weight per infusion (4 patients), 3.4 mg/kg (10 patients), and 5.1 mg/kg (7 patients). Ototoxicity affects the higher frequencies first and extends to the lower frequencies during continuation of the ototoxic treatment. Peritonitis due to continuous ambulatory peritoneal dialysis (CAPD) regularly demands ototoxic antibiotic treatment, e.g. with gentamicin and vancomycin. In this study ultra-high frequency audiometry (10-20 kHz), 'normal' audiometry (0.125-8 kHz) and registration of blood concentrations of the antibiotics were used to assess ototoxic hearing loss during treatment of CAPD patients with peritonitis.

Patients with uncomplicated pelvic inflammatory disease (PID) (acute salpingitis and no pelvic masses) were randomly assigned for treatment with either cefotaxime or cefoxitin. Aminooxyacetic acid (AOAA) is a transaminase inhibitor that has been shown to protect the inner ear from loud noises. This study was done to determine if it can also protect against the cochleotoxic action of gentamicin. Metronidazole (M) has potent bactericidal activity against anaerobic bacteria. This study was designed to compare its efficacy and safety when combined with gentamicin (M + G) to those of an accepted regimen, clindamycin plus gentamicin (C + G), for the treatment of intra-abdominal infection. A three drug regimen of carbenicillin, cefazolin and gentamicin was used for the treatment of patients with solid tumors, who developed fever while granulocytopenic following chemotherapy. Thirty-five (8%) of 426 cancer patients receiving various combinations of antineoplastic chemotherapy qualified for the antibiotic treatment. Nine patients (26%) had bacteriologically confirmed infections, all with gram negative microorganisms. Twenty-four patients (69%) recovered and eleven (31%) died. Increasing evidence suggests that membrane phospholipids are a major site of interaction between gentamicin and renal tubular cells. To help assess the impact of this interaction on renal tubular cell phospholipid metabolism, renal cortical phospholipid levels were assessed serially during treatment with nephrotoxic doses of gentamicin in the rat. The present work describes the effects on the spiral organ of very young guinea pigs, of continuous white noise (76 dB SPL, 7 days) combined with gentamicin, using surface preparations and both transmission and scanning electron microscopy to analyse structural changes. Tympanocentesis was performed in 32 pediatric patients with chronic recurrent suppurative otitis media. The aspirate was cultured aerobically and anaerobically.
