A prospective assessment of bowel habit in irritable bowel syndrome in women: defining an alternator.

BACKGROUND & AIMS: Irritable bowel syndrome (IBS) is subtyped as IBS with diarrhea (IBS-D) or IBS with constipation (IBS-C) based on Rome II guidelines. The remaining group is considered as having mixed IBS (IBS-M). There is no standard definition of an alternator (IBS-A), in which bowel habit changes over time. Our aim was to use Rome II criteria to prospectively assess change in bowel habit for more than 1 year to understand IBS-A. METHODS: Female patients (n=317) with IBS entering a National Institutes of Health treatment trial were studied at baseline with questionnaires and 2-week daily

Exposure with response prevention versus habit reversal in Tourettes's syndrome: a controlled study.

The intentional nature of tics provides the opportunity to apply behavioural interventions aimed at tic reduction through interruption of stimulus-response sequences. The aim of this study has been to evaluate the effect of exposure and response prevention (ER) versus habit reversal (HR) in 43 Tourette's syndrome (TS) patients. The three outcome measures were: the Yale Global Tic Severity Scale (YGTSS), 15-min tic frequency registrations monitored at the institute and 15-min home tic frequency registrations. Both treatment conditions resulted in statistically significant improvements on all ou

[Clinical study on the relationship between tooth abrasion and the habits of chewing betel nut].

OBJECTIVE: To investigate the relationship between tooth abrasion and long-term chewing betel nut. METHODS: The time, frequency, clinical features of occlusal abrasion, and results of pantomography were studied in 64 patients with the habits of chewing betel nut. RESULTS: The occlusal surfaces of all patients had abrasion from mild to severe. The longer the chewing time, the more severe the occlusal abrasion (P < 0.01). The occlusal abrasion became more severe with the increase in chewing frequency (P < 0.01). The severe abrasion was accompanied with periapical periodontitis and the resorpti…

Diagnosing the patient with abdominal pain and altered bowel habits: is it irritable bowel syndrome?

Diagnosing a patient who presents with abdominal pain and altered bowel habits can be challenging. Although serious organic illnesses can cause these symptoms, irritable bowel syndrome is commonly responsible. It can be difficult to properly evaluate these patients without overusing diagnostic tests and consultation. A practical approach for diagnosing irritable bowel syndrome is suggested, using the Rome II criteria and the presence of alarm symptoms such as weight loss, gastrointestinal bleeding, anemia, fever, or frequent nocturnal symptoms as starting points. If there are no alarm symptoms…

Dissociation between attentional set shifting and habit learning: a longitudinal case study.

We report a patient (ST) with predominant damage to the right neostriatum, caused by a rare cerebral angiitis. The testing procedure was focused on attentional set shifting (Wisconsin Card Sorting Test; WCST) and habit learning (probabilistic classification learning; PCL). ST showed impairments in the WCST, digit span backward, alphabet span, and PCL procedures, whereas he exhibited spared IQ, short-term verbal memory, object recognition, episodic and semantic memory. After 1 month of steroid therapy, there was a significant improvement in the WCST, digit span backward and alphabet span tests,

Sequential application of major habit-reversal components to treat motor tics in children.

In this study, we sequentially administered up to four components of the habit-reversal treatment to 4 children with motor tics within a multiple baseline design. The habit-reversal components included (a) awareness training; (b) awareness training and self-monitoring; (c) awareness training, self-monitoring, and social support; and (d) awareness training, social support, and the use of a competing response. Results demonstrated that the combined use of awareness training, social support, and competing response training was effective in eliminating motor tics in 2 of 4 children, that awareness

Long-term follow-up of habit retraining for bladder instability in elderly patients.

Sixty patients with detrusor instability were contacted 2 to 3 years after their initial diagnosis and treatment. When first treated, 40 had improved, nine were catheterized and the remainder were unchanged. By the time of the first outpatient appointment 1 month later, a further 6% had been catheterized and 26% had lost control. Two to 3 years later, 17 of the 40 who had improved initially had maintained their improvement. The control in all the remainder had deteriorated and ten of the original 60 had died. Four of these deaths occurred in the group initially catheterized, only two of this g

[Behavior therapy self control in chronic compulsive organismic defective habit development].

Patients with chronic compulsive organismic mishabituations, which are rather difficult to treat, can be helped by self-control methods of behavioural therapy combined with suggestive measures. The author currently regards this therapeutic approach to be superior to aversion therapy, which was occasionally used for such cases in the past. The contribution presents a scheme of therapy which can be generalized and is based on the work of Meichenbaum and Leonhard. The scheme consists of two parts containing four phases each. The actual approach and the modification necessary to tailor it to the i
