Drug checking services: barriers and facilitators to implementation by U.S. syringe services programs.

BACKGROUND: Syringe services programs (SSPs) provide evidence-based services to prevent the spread of disease and reduce overdose mortality among people who use drugs. To address unacceptably high levels of overdose, some SSPs have implemented advanced drug checking services (DCS) to identify illicitly manufactured fentanyl, xylazine, and other unexpected substances in local drug supplies. Advanced DCS improve people's knowledge of the illicit drug market and can support widescale dissemination of information for monitoring and evaluation of drug supply trends. OBJECTIVE: To investigate the b

Public drug use, policing, and the limits of decriminalization in British Columbia: A mixed methods study.

In January 2023, British Columbia implemented a province-wide decriminalization policy for simple drug possession that aimed to reduce criminal penalties for people who use drugs. However, the policy emerged alongside intense public and political scrutiny of visible drug use which led to new restrictions on public consumption. This study uses a mixed-methods approach that draws on quantitative data from the 2024 Harm Reduction Client Survey (N = 622) and in-depth interviews with people who use drugs (N = 38) to examine how people who use drugs have experienced decriminalization in relation to …

Impact of lab-based drug checking services on syringe service program engagement in Maryland, USA.

PURPOSE: The introduction of synthetic substances into the unregulated drug supply has driven a surge in overdose deaths, posing significant public health challenges in timely substance identification, engagement with people who use drugs (PWUD), and access to treatment. Maryland's rapid analysis of drugs (RAD) program, piloted through syringe service programs (SSPs), provides a proactive harm reduction strategy by integrating mail-in, laboratory based drug checking with broader public health services to enhance surveillance and improve engagement with PWUD. We aim to investigate the associati…

Tribally-affiliated syringe services programs in the United States: a brief report.

BACKGROUND: Few studies have examined the implementation of syringe services programs (SSPs) that serve Indigenous people who inject drugs i … searches included several synonyms for SSPs, including "needle exchange," "syringe exchange," and "harm reduction." In instances where "harm reduction programs" were identified, we reviewed website contents to determine if the program was inclusive of a SSP. We considered SSPs to be affiliated with Federally-recognized tribe(s) if they were housed in a tribal h …

Advancing research on strategies to reduce drug use and overdose-related harms: a community informed approach to establishing common data elements.

With the overdose crisis continuing to pose significant challenges in North America, harm reduction strategies are critical for public health systems to reduce mortality and morbidity. Despite the considerable strides in harm reduction research, high-quality evidence for decision-making is limited. This is compounded by a variation in reported outcomes, drug supply, administration changes, and policy and social impacts, which further challenge researchers and practitioners in their efforts to implement effective, nimble harm reduction interventions. Adoption of common data elements (CDEs) and …

A qualitative study of health, social, and community professionals' practices to reduce alcohol-related risks among people living in social precarity in the Greater Paris region.

Alcohol-related harms constitute a major global public health challenge. Despite similar consumption levels to their more affluent counterparts, alcohol-related morbidity and mortality is disproportionately higher in socioeconomically deprived populations. People living in social precarity face multiple barriers to accessing traditional treatment services, making alcohol harm reduction (AHR) practices particularly relevant for them. This qualitative study, conducted in the Greater Paris region, described and analyzed AHR practices adopted by health, social, and community professionals working …

Re-examining mandatory drug testing in Australian prisons.

Mandatory drug testing is commonly used in Australian prisons to detect and deter drug use. In this commentary, we review the limited evidence for mandatory drug testing programs, highlight potential harms associated with their implementation and provide recommendations for drug surveillance in prisons concordant with a harm minimisation framework.

Bridging institutional logics: Implementing naloxone distribution for people exiting jail in three California counties.

Drug overdose is the leading cause of death among formerly incarcerated people. Distribution of the opioid overdose medication naloxone to people who use drugs reduces overdose mortality, and officials in many jurisdictions are now considering or implementing programs to offer naloxone to people exiting jails and prisons. The principles and practices of harm reduction programs such as naloxone distribution conflict with those of penal institutions, raising the question of how organizations based on opposing institutional logics can collaborate on lifesaving programs. Using in-depth interviews …

Cognitive neuroscience can support public health approaches to minimise the harm of 'losses disguised as wins' in multiline slot machines.

Video slot machines are associated with both accelerated transition into problematic forms of gambling, as well as psychosocial harm above and beyond other forms of gambling. A growing body of evidence is uncovering how key design features of multiline slot machines produce an inflated experience of reward, despite the fact that these features offer no overall financial benefit to the player. A pernicious example of this are 'losses disguised as wins' (LDWs), which occur when simultaneous bets placed on multiple lines result in a winning combination that returns an amount greater than zero, bu

The use of messages in altering risky gambling behavior in experienced gamblers.

The present study was an experimental analogue that examined the relationship between gambling-related irrational beliefs and risky gambling behavior. Eighty high-frequency gamblers were randomly assigned to four conditions and played a chance-based computer game in a laboratory setting. Depending on the condition, during the game a pop-up screen repeatedly displayed either accurate or inaccurate messages concerning the game, neutral messages, or no messages. Consistent with a cognitive-behavioral model of gambling, accurate messages that correctly described the random contingencies governing
