Providing a Nursing Care Plan as a Requirement for Secondary Prevention for People Living With HTLV-1.

INTRODUCTION: The absence of nursing care plans aimed at people living with HTLV-1 (PLHTLV) led us to develop and test a proposed nursing care plan based on the evaluation of 55 PLHTLV to outline interventions according to the clinical stage. METHODS: After interviews with symptomatic patients, nursing diagnoses were made using the NANDA International Nursing Diagnoses (The International Nursing Knowledge Association). Subsequently, interventions were selected through the Classification of Nursing Interventions (NIC), and expected results were selected through the Classification of Nursing Ou

Evaluation of mode of birth in pregnant women with heart disease.

OBJECTIVE: Maternal heart disease (HD) complicates 1-4 % of pregnancies and is associated with adverse maternal and fetal outcomes. Although vaginal birth is generally recommended in the guidelines, cesarean section (CS) rates in women with HD are often high. Aim of the present study was to evaluate mode of birth and pregnancy outcomes in women with HD in a tertiary care hospital in the Netherlands. STUDY DESIGN: The study population consisted of 128 consecutive pregnancies in 99 women with HD, managed by a pregnancy heart team between 2012-2017 and ending in births after 24 weeks' gestation.

Using the critical path method to rollout and optimise new PMTCT guidelines to eliminate mother-to-child transmission of HIV in Zimbabwe: a descriptive analysis.

BACKGROUND: Achievement of the elimination target for mother-to-child transmission (MTCT) of HIV in selected countries has increased hope to end the HIV epidemic in children across the world. However, MTCT rates remain well above the 5% elimination target in most sub-Saharan Africa countries. These countries require innovative strategies to scale-up their interventions to end paediatric HIV. We describe how the Elizabeth Glaser Paediatric AIDS Foundation (EGPAF) consortium and the Children's Investment Fund Foundation (CIFF) used the critical path method to facilitate rapid expansion and optim

Representation of women in cardiovascular disease management: a systematic analysis of ESC guidelines.

OBJECTIVE: Sex differences play a critical role in the presentation, progression and treatment outcomes of cardiac diseases. However, historical male predominance in clinical studies has led to disparities in evidence supporting care for both sexes. Clinical guidelines are essential for cardiovascular care, shaping practice and influencing patient outcomes. In this study, we reviewed 34 European Society of Cardiology (ESC) guidelines between 2002 and 2024 to evaluate the representation of women and the inclusion of female-specific recommendations. METHODS: We compiled 136 gender-related keywo

Assessing concentrations and health impacts of air quality management strategies: Framework for Rapid Emissions Scenario and Health impact ESTimation (FRESH-EST).

In air quality management, reducing emissions from pollutant sources often forms the primary response to attaining air quality standards and guidelines. Despite the broad success of air quality management in the US, challenges remain. As examples: allocating emissions reductions among multiple sources is complex and can require many rounds of negotiation; health impacts associated with emissions, the ultimate driver for the standards, are not explicitly assessed; and long dispersion model run-times, which result from the increasing size and complexity of model inputs, limit the number of scena

Antimicrobial Resistance: a One Health Perspective.

One Health is the collaborative effort of multiple health science professions to attain optimal health for people, domestic animals, wildlife, plants, and our environment. The drivers of antimicrobial resistance include antimicrobial use and abuse in human, animal, and environmental sectors and the spread of resistant bacteria and resistance determinants within and between these sectors and around the globe. Most of the classes of antimicrobials used to treat bacterial infections in humans are also used in animals. Given the important and interdependent human, animal, and environmental dimensi

Safely Sedating Children and Adolescents Who Are Neurodivergent.

Children who are neurodivergent have higher rates of injury, number of health care visits, and perceived need for sedation compared with their peers who are not neurodivergent, resulting in unique medical care challenges for this patient population. Sedation of children who are neurodivergent is challenging due to increased risk of adverse events, medication interactions, and behavioral complexities. Safe and effective sedation requires individualized planning, careful agent selection, and incorporation of nonpharmacologic strategies. Significant variability exists in sedation practices for ch

An Environmental Scan of Alternate Level of Care (ALC) Policies in Ontario, Canada.

BACKGROUND: Alternate level of care (ALC) patients occupy hospital beds despite no longer requiring acute services, impacting patient flow and contributing to overcrowding. Ontario hospitals have updated legislation and policies to address this issue, yet protocols vary across centres. This study conducted an environmental scan of ALC policies to examine differences in patient identification, management, and support. METHODS: The research employed a multi-phase approach, beginning with a review of ALC policies obtained through direct inquiry from relevant institutions and publicly available d

A collaborative clinic between contraception and sexual health services and an adult congenital heart disease clinic.

BACKGROUND: The success of cardiac surgery has created a new group of patients: those with 'adult congenital heart disease' (CHD) who may need specialist advice about contraception and pregnancy. The study objective was to investigate whether women with CHD were receiving appropriate advice on contraception. METHODS: The study setting comprised a combined adult CHD and contraception and sexual health clinic operating alongside each other, once a month, at Freeman Hospital, Newcastle-upon-Tyne, UK. Data were collected on 46 consecutive female patients attending the clinic between April 2002 an

Standardizing Opioid Prescribing Practices for Cancer-Related Pain Via a Novel Interactive Documentation Template at a Public Hospital.

PURPOSE: Opioid misuse during cancer pain management places patients at risk for harm and physicians for legal liability. Identifying and monitoring patients who are at risk is challenging given the lack of validated clinical tools and evidence-based guidelines. In the current study, we aimed to standardize opioid prescribing practices at a community oncology clinic to help ensure patient safety and physician compliance with Texas state regulations. METHODS: We used the Plan-Do-Study-Act methodology. In the planning phase, current practices of assessing opioid efficacy, toxicity, and misuse w

Towards a goal-oriented methodology for clinical-guideline-based management recommendations for patients with multimorbidity: GoCom and its preliminary evaluation.

Patients with chronic multimorbidity are becoming more common as life expectancy increases, making it necessary for physicians to develop complex management plans. We are looking at the patient management process as a goal-attainment problem. Hence, our aim is to develop a goal-oriented methodology for providing decision support for managing patients with multimorbidity continuously, as the patient's health state is progressing and new goals arise (e.g., treat ulcer, prevent osteoporosis). Our methodology allows us to detect and mitigate inconsistencies among guideline recommendations stemming

KInetics of Procalcitonin to Reduce Unnecessary aNtibiotic use (KIPRUN): protocol for a multicentre, randomised, superiority trial to compare the efficacy and safety of procalcitonin kinetics-guided and absolute procalcitonin value-guided antibiotic initiation in critically ill patients.

INTRODUCTION: Appropriate antibiotic (AB) therapy remains a challenge in intensive care units (ICUs). Guidelines recommend against using procalcitonin (PCT) to guide the initiation of AB therapy, although with low certainty and a very low level of evidence. Recent studies suggest that changes in PCT levels may be more accurate than a single measurement; however, this has never been tested in a randomised trial. METHODS AND ANALYSIS: In this multicentre, randomised controlled trial (RCT), we aim to compare the efficacy and safety of two different PCT protocols for initiating AB therapy. Hemody

MSCA HOPE - Physical education in hospital schools: A protocol paper for systematic and umbrella reviews with policy framework analysis.

Hospital school programs support educational continuity for children and adolescents during hospitalization, but the provision of physical education (PE) remains inconsistent and is rarely addressed by unified standards. This protocol aims to describe the methods for three complementary components designed to synthesise evidence on PE and related movement interventions in hospital settings and to map the policy frameworks governing PE provision in hospital schools. Tier 1 is an umbrella review of existing systematic reviews on physical activity interventions for hospitalised youth. Tier 2 is a

Improving Access to the Transportation Alternatives Program for Rural Communities.

Rural small towns with small main streets and compact downtown development can be ideal locations to create walkable communities. The Centers for Disease Controls and Prevention (CDC)'s High Obesity Program (HOP) funds Cooperative Extension programs to implement strategies to improve food access and support active transportation in high obesity (often rural) counties. The Louisiana State University (LSU) AgCenter HOP program had previously partnered with rural low-income communities to create Complete Streets plans but was challenged to find ways to implement those plans. Factors Influencing Outpatient Cardiac Rehabilitation Attendance.

PURPOSE: In the current health climate, the length of stay of cardiac patients in hospital has been decreasing, and this has significantly reduced the time nurses and colleagues have for providing inpatient cardiac rehabilitation (CR). The purpose of this research was to determine if inpatient CR has an influence on outpatient cardiac rehabilitation attendance for women, Māori, and older people. METHODS: An audit of patients discharged from hospital between November 2011 and July 2012 with a diagnosis of acute coronary syndrome were sent a postal questionnaire. FINDINGS: The survey was compl

Preconception Health Behavioral Patterns and Predictors in Chinese Reproductive-Aged Men: A Latent Profile Analysis.

Preconception health behaviors significantly influence pregnancy outcomes, yet patterns of these behaviors and their determinants have received limited attention, particularly among men. Understanding behavioral profiles is essential for developing targeted interventions to improve paternal reproductive health. We conducted a cross-sectional study examining over 20 guideline-recommended preconception health behaviors among 952 Chinese men. Latent profile analysis was utilized to identify different behavioral profiles. Multinomial logistic regressions assessed associations of profile membership
