Vitamin D Deficiency Meets Hill's Criteria for Causation in SARS-CoV-2 Susceptibility, Complications, and Mortality: A Systematic Review.

Clinical trials consistently demonstrate an inverse correlation between serum 25-hydroxyvitamin D [25(OH)D; calcifediol] levels and the risk of symptomatic SARS-CoV-2 disease, complications, and mortality. This systematic review (SR), guided by Bradford Hill's causality criteria, analyzed 294 peer-reviewed manuscripts published between December 2019 and November 2024, focusing on plausibility, consistency, and biological gradient. Evidence confirms that cholecalciferol (D3) and calcifediol significantly reduce symptomatic disease, complications, hospitalizations, and mortality, with optimal ef

A systematic comparison of the actual, potential, and theoretical health effects of cobalt and chromium exposures from industry and surgical implants.

Humans are exposed to cobalt (Co) and chromium (Cr) from industry and surgical devices, most notably orthopedic joint replacements. This review compares the potential health effects of exposure to Co and Cr contaminants from these two different sources, both in the locally exposed tissues and at sites distant to the primary exposure. Surgical implantation results largely in exposures to ions, corrosion products, and particles of Co and Cr. Industrial exposures are predominantly to metal compounds and particles. Although there are large literatures on industrial and surgical exposures to these

The moral battleground of a prison psychiatrist.

ObjectiveMoral distress occurs when we know what the right thing to do is in accordance with our ethical principles and values, but institutional or other constraints make this difficult1. As psychiatrists and psychiatry trainees, we are asked to assume responsibilities and balance competing obligations to our patients, organisations, and the public. I offer a first-person narrative of the moral distress experienced by a prison psychiatrist. I hope it resonates with other psychiatrists and psychiatry trainees dedicated to addressing the moral challenges in mental health and inspires those to h

Analysis of Factors Contributing to the Spread of Cholera in Developing Countries.

OBJECTIVE: Cholera has been endemic in Nigeria since the 1970s and has resulted in several early deaths, which probably could have been avoided. This study examined the various factors that contribute to the infection and spread of cholera in Benue State, Nigeria, from 2008 to 2017. We also proposed a cloud based health management system (CBHMS) for the timely identification and management of cholera in an epidemic. MATERIALS AND METHODS: A cluster random sampling in the form of a close ended questionnaire was used to collect data from a sample of 420 participants comprising farmers, traders,

Underreporting of hemorrhagic and thrombotic complications of pharmaceuticals to the U.S. Food and Drug Administration: empirical findings for warfarin, clopidogrel, ticlopidine, and thalidomide from the Southern Network on Adverse Reactions (SONAR).

The U.S. Food and Drug Administration's (FDA) Adverse Event Reporting System (AERS), familiarly known as "MedWatch," is the nation's primary tool for postmarket pharmaceutical safety surveillance. This system relies on adverse events voluntarily reported by health care providers and consumers either directly to the FDA or to drug manufacturers, which are required to prepare and forward the information to the agency. Little is known about how frequently adverse events are reported. Previous estimates range from 1 to 31% depending on the event, drug, and time period. We used published incidence

Active surveillance for the safety and effectiveness of health products for COVID-19: a scoping review.

Urgent response to the coronavirus disease 2019 (COVID-19) pandemic necessitated rapid implementation of experimental, re-purposed, or off-label treatment strategies, which can be monitored via active surveillance (AS). Using a scoping review methodology, we summarized AS systems used to assess the safety and effectiveness of health products for COVID-19 treatment. We searched electronic databases (MEDLINE, Embase, Web of Science, and Cochrane Library), global regulatory agency websites, and registries, and implemented alerts until August 2022. The records with data source, active data access,

A Modified Public Health Automated Case Event Reporting Platform for Enhancing Electronic Laboratory Reports With Clinical Data: Design and Implementation Study.

BACKGROUND: Public health reporting is the cornerstone of public health practices that inform prevention and control strategies. There is a need to leverage advances made in the past to implement an architecture that facilitates the timely and complete public health reporting of relevant case-related information that has previously not easily been available to the public health community. Electronic laboratory reporting (ELR) is a reliable method for reporting cases to public health authorities but contains very limited data. In an earlier pilot study, we designed the Public Health Automated C

Client-Level Effects of States' Mental Health Agency Expenditures and Mental Health Parity Legislation.

OBJECTIVE: Despite federal parity legislation, gaps in access to behavioral health care persist across the United States. Stronger parity implementation has been associated with an improved system-level mental health infrastructure. The authors determined whether parity and higher per capita expenditures lead to better client-level access and outcomes. METHODS: This analysis of individual-level data extends previous system-level work by examining client-level outcomes across the top 10 and bottom 10 states for per capita state mental health agency (SMHA) expenditures and parity implementation

"Keeping Control": A user-led exploratory study of mental health service user experiences of targeted violence and abuse in the context of adult safeguarding in England.

The situation for people with mental health problems as a group of disabled people who experience targeted violence and abuse is a complex one. Disabled people, particularly those with mental health problems, are at higher risk of targeted violence and hostility with few effective evidence-based prevention and protection strategies. Achieving effective safeguarding for adults with mental health problems is characterised by differential attitudes to and understandings of abuse by safeguarding practitioners, as well as systemic issues arising from multi-agency working. "Keeping Control" was a 16

Evaluation of deep space exploration risks and mitigations against radiation and microgravity.

Ionizing radiation and microgravity are two considerable health risks encountered during deep space exploration. Both have deleterious effects on the human body. On one hand, weightlessness is known to induce a weakening of the immune system, delayed wound healing and musculoskeletal, cardiovascular, and sensorimotor deconditioning. On the other hand, radiation exposure can lead to long-term health effects such as cancer and cataracts as well as have an adverse effect on the central nervous and cardiovascular systems. Ionizing radiation originates from three main sources in space: galactic cos

"A person who does not have money does not enter": a qualitative study on refugee women's experiences of respectful maternity care.

BACKGROUND: Women's childbirth experiences in health facilities is at the core of quality of care. Their perceptions of poor-quality care, including disrespectful care at health facilities during childbirth, is recognized as a significant barrier to seeking care for subsequent births. Research that explores women's perspectives of the dimensions of disrespect and mistreatment during childbirth in Arab countries is scarce, and there is none pertaining to refugee groups who carry the burden of multiple vulnerabilities and who suffer from discontinued care, especially in fragile health systems. Beyond the Grind: "Rest is Resistance" in Health Professions Education.

Rooted in spiritual energy and centered in Black liberation, Afrofuturism, and feminism, Hersey's "Rest is Resistance" framework encourages deliberate rest practices as a personal necessity and a form of political resistance. Inspired by her work, we are a group of health professions education scholars from minoritized backgrounds using our lived experiences to illustrate the application of our "Rest is Resistance" framework. The demands of academic medicine, defined by productivity in clinical, scholarly, and educational domains, combined with personal obligations, run counter to rest. Enduri
