Will biventricular pacing replace right ventricular pacing for antibradycardia therapy?

Evaluation of: Curtis AB, Worley SJ, Adamson PB, et al; Biventricular versus Right Ventricular Pacing in Heart Failure Patients with Atrioventricular Block (BLOCK HF) Trial Investigators. Biventricular pacing for atrioventricular block and systolic dysfunction. N. Engl. J. Med. 368(17), 1585-1593 (2013). Recent trials have shown the benefit of cardiac resynchronization (CRT) in mild or moderate heart failure with a left bundle branch block or intraventricular conduction delay. Randomized trials (including the BLOCK-HF study) have now shown that irrespective of baseline QRS duration, long-term

Second-degree atrioventricular block: Mobitz type II.

Acute atrioventricular (AV) block occurs frequently in patients with myocardial infarction. Atrioventricular block is also a common manifestation of sclerodegenerative conduction system disease. Occasionally, heart block results from drug toxicity, hyperkalemia, cardiac valvular calcification, myocarditis, or infiltrative cardiomyopathy. Second-degree AV block is a form of "incomplete" heart block, in which some, but not all, atrial beats are blocked before reaching the ventricles. Mobitz type II second-degree block is an old term, which refers to periodic atrioventricular block with constant

Permanent His Bundle Pacing for Cardiac Resynchronization.

His bundle pacing (HBP) has been shown to be a feasible, beneficial, and safe way to achieve cardiac resynchronization therapy (CRT) with recruitment of the heart's physiological conduction system. HBP should be considered for those with unfavorable coronary sinus (CS) anatomy, and nonresponders to biventricular (BiV) pacing. HBP CRT may also help patients with the nonleft bundle branch block form of conduction delay and heart failure (HF). HBP CRT should be considered strongly in preventing right ventricular (RV) pacing-induced cardiomyopathy, especially after atrioventricular nodal ablation

Air Entrapment in a Pacemaker Pocket in a Child.

BACKGROUND: Advances in pediatric electrophysiology have revolutionized cardiac care by offering patients treatments for increasingly complex cardiac rhythm and conduction disorders. However, despite these innovations, there are a number of potential complications that clinicians have to deal with. CASE PRESENTATION: This clinical case study describes a rare complication in a child following pacemaker implantation, namely the appearance and accumulation of air in the pacemaker pocket. The child underwent multiple cardiac surgery for a complex congenital heart defect (CHD). Unfortunately, duri

Wolff-Parkinson-White Syndrome in a Term Infant Presenting With Cardiopulmonary Arrest.

BACKGROUND: Wolff-Parkinson-White syndrome is a congenital abnormality of the cardiac conduction system caused by the presence of an abnormal accessory electrical pathway between the atria and the ventricles. This can result in intermittent tachyarrhythmias such as supraventricular tachycardia. In rare occasions, sudden death may occur from atrial fibrillation with rapid ventricular conduction. Supraventricular tachycardia typically has a sudden onset and offset, classified as a paroxysmal arrhythmia. Because of the variable occurrence, Wolff-Parkinson-White syndrome may go undiagnosed in the

Cardiac resynchronization therapy for heart failure management.

Heart failure (HF) is responsible for an immense burden on our healthcare system. The prevalence of this disease continues to increase as a result of an aging population, successful treatment of acute coronary syndrome, and the use of new pharmacotherapies. Although pharmacotherapy with angiotensin converting enzyme inhibitors and beta-blockers shows improvement in morbidity and mortality, the overall prognosis of these patients remains poor. Cardiac transplant has limited applicability. Conduction disturbances are frequent in patients with advanced heart failure and may cause worsening systol

The genetic origin of atrioventricular conduction disturbance in humans.

Atrioventricular (AV) conduction disturbance (block) describes impairment of the electrical continuity between the atria and ventricles. Clinical classification of AV block has utilized biophysical characteristics, usually the extent (1st, 2nd, 3rd degree) and site of block (above or below His bundle recording site). The genetic significance of this classification is not known. In some cases AV block occurrence is associated with intrauterine exposure to maternal antibody (anti-Ro, anti-La), and other cases are associated with injury (e.g. surgery). Based on familial clustering of idiopathic A

Electrocardiographic repolarization and conduction abnormalities in pediatric epilepsy.

OBJECTIVE: To evaluate subclinical alterations in cardiac conduction and ventricular repolarization in children with epilepsy by comparing corrected QT dispersion (QTcd), Tpeak-Tend (Tp-e) interval, Tp-e-based ratios, and P-wave dispersion (PWD) between pediatric epilepsy patients and healthy controls. A secondary aim was to explore electrophysiological differences between electrocardiographic recordings obtained during emergency department (ED) presentations and interictal outpatient evaluations. METHODS: In this prospective case-control study, 149 children with epilepsy and 75 age- and sex-

Late Potential Abolition in Ventricular Tachycardia Ablation.

Ventricular tachycardia (VT) substrate-based ablation has become the gold standard treatment for patients with structural heart disease-related VT. VT is linked to re-entry in relation to myocardial scarring, with areas of conduction block (core scar) and of slow conduction (border zone). Slow conduction areas can be detected in sinus rhythm as late potentials (LPs). LP abolition has been shown to be the best end point to avoid long-term recurrences. Our study aimed to analyze the challenges of LP abolition and the predictors of failure. We analyzed 169 consecutive patients with structural hea

Sudden cardiac death-update.

Sudden cardiac death (SCD) is one of the most common causes of death worldwide with a higher frequency especially in the young. Therefore, SCD is represented frequently in forensic autopsy practice, whereupon pathological findings in the heart can explain acute death. These pathological changes may not only include myocardial infarction, coronary thrombosis, or all forms of myocarditis/endocarditis but also rare diseases such as hereditary structural or arrythmogenic anomalies, lesions of the cardiac conduction system, or primary cardiac tumours.

Isolated tricuspid valve repair for Libman-Sacks endocarditis.

Cardiac involvement is a well-known complication of systemic lupus erythematosus (SLE), which can involve most cardiac components, including pericardium, conduction system, myocardium, heart valves, and coronary arteries. Libman-Sacks (verrucous) endocarditis is the characteristic cardiac valvular manifestation. Although isolated tricuspid valve involvement is quite rare, we report a patient with SLE who had tricuspid stenosis caused by Libman-Sacks endocarditis. The patient underwent successful commisurotomy and Kay annuloplasty on the tricuspid valve under cardiopulmonary bypass.

Effect of the autonomic blockade on the automaticity of the A-V junctional pacemaker in awake dogs.

The effect of the autonomic blockade on the automaticity of the A-V junctional pacemaker was evaluated in 15 awake dogs with experimentally induced A-V junctional rhythm. The duration of asystole after overdrive (D.A.O.) in these dogs was prolonged significantly in accordance with increase in the drive rate, and the mean +/- SD of the D.A.O. reached 4.7 +/- 1.1 seconds (N = 15) after overdrive at 2.5 times the spontaneous heart rate. After administration of atropine (0.4 mg/kg; i.v.) to eight dogs, the mean +/- SD of the D.A.O. at the same rate decreased from 4.5 +/- 0.9 to 3.4 +/- 1.2 seconds

Lamin and the heart.

Lamins A and C are intermediate filament nuclear envelope proteins encoded by the LMNA gene. Mutations in LMNA cause autosomal dominant severe heart disease, accounting for 10% of dilated cardiomyopathy (DCM). Characterised by progressive conduction system disease, arrhythmia and systolic impairment, lamin A/C heart disease is more malignant than other common DCMs due to high event rates even when the left ventricular impairment is mild. It has several phenotypic mimics, but overall it is likely to be an under-recognised cause of DCM. In certain clinical scenarios, particularly familial DCM wi

Apoptosis in congenital heart disease.

Morphogenesis in the human body is mediated to an important, but not exclusive, extent by apoptosis. That this is true in the heart is illustrated by primary pulmonary hypertension, which includes medial degeneration in both coronary and pulmonary arteries, normal and abnormal postnatal involution of the right ventricle, familial progressive heart block ending in fatal arrhythmias, and the long QT syndrome. Apoptosis occurs in ventricular myocardium, specialized myocytes of the conduction system, endothelium and medial smooth muscle of small arteries, cardiac nerves and ganglia and fibroblasts

Collateral injury of the conduction system during catheter ablation of septal substrate in nonischemic cardiomyopathy.

INTRODUCTION: In patients with nonischemic cardiomyopathy (NICM) little is known about the clinical impact of catheter ablation (CA) of septal ventricular tachycardia (VT) resulting in the collateral injury of the conduction system (CICS). METHODS AND RESULTS: Ninety-five consecutive patients with NICM underwent CA of septal VT. Outcomes in patients with no baseline conduction abnormalities who developed CICS (group 1, n = 28 [29%]) were compared to patients with no CICS (group 2, n = 17 [18%]) and to patients with preexisting conduction abnormalities or biventricular pacing (group 3, n = 50

Conduction system pacing and defibrillation support using a single-chamber defibrillator - A case report.

We present a case of a 67-year-old male with heart failure with reduced ejection fraction due to a previous myocardial infarction, slow-conducting atrial fibrillation with an indication of pacemaker implantation, and sustained ventricular tachycardia, requiring defibrillation support in prevention of sudden cardiac death. Current guidelines recommend biventricular over conventional right ventricular pacing for patients with ejection fraction <40 % and atrioventricular conduction disorders in either sinus rhythm or atrial fibrillation, but in patients with a narrow baseline QRS complex, biventr

[Congenital Intermittent Third-degree Atrioventricular Block Associated with Retinoid Exposure in Pregnancy].

BACKGROUND: The retinoid acitretin, which has been approved as an effective therapy for severe keratinization disorders, is highly teratogenic, and exposure in the first trimester of pregnancy is associated with the risk of miscarriage and various malformations, including congenital heart defects. Cardiac conduction system disorders have not been described so far. CASE REPORT: A 24-year-old woman was treated with acitretin for dyskeratosis follicularis until pregnancy was diagnosed at 12 weeks of gestation. The female infant was born after 35 weeks gestation by cesarean section because of int

Automatic classification of heartbeats using neural network classifier based on a Bayesian framework.

This paper presents a method of automatic processing the electrocardiogram (ECG) signal for the classification of heart beats. Data were obtained from 48 records of the MIT-BIH arrhythmia database (only one lead). Five types of arrhythmic beats were classified using our method, Premature Ventricular Conduction beat (PVC), Atrial Premature Conduction beat (APC), Right Bundle Branch Block beat (RBBB), Left Bundle Branch Block beat (LBBB), and Paced Rhythm Beat (PRB), in addition to the Normal Beat (NB). A learning dataset for the neural network was obtained from a five records set (124, 214, 111

Influence of temperature on the atrioventricular conduction time for spontaneous and imposed rhythms.

In the present work data are obtained about the changes in the atrioventricular conduction time under the effect of two parameters: temperature and frequency of the sinus node. The experiments are carried out on preparations from the right atrium of rabbit heart. Temperature changes are obtained by regulation of the temperature of the nutrient medium in the 23--36 degrees C range. The excitation frequency of the sinus node was changed by imposition of artificial (external) rhythms. Data are obtained about the presence of frequency- and temperature-dependent components of the atrioventricular (
