[Catheter treatment of congenital heart defects. Arterial septal defects and open ductus arteriosus].

The results of new methods for catheter treatment of congenital heart defects are presented. Between 1989 and 1996 closure of a patent ductus arteriosus was performed in 66 instances on 63 patients, eight of which were with coils. Three patients were treated twice, one with an additional umbrella, two with coils. The overall complete closure rate for umbrellas was 75%, after two ducts, which were initially totally occluded, recanalized. In six more patients the procedure was either aborted or indication was not present. All six ducts treated with coils as the first procedure were completely cl

A novel mutation in exon 1 of GATA4 in Egyptian patients with congenital heart disease.

BACKGROUND/AIM: Congenital heart disease (CHD) is a common birth defect. Many studies have reported GATA4 mutations in patients with CHD, mainly septal defects. This study aimed to investigate the GATA4 exon 1 mutation in Egyptian patients with isolated congenital heart defects as a possible causative mutation. MATERIALS AND METHODS: Screening for mutations or any sequence variations in exon 1 of the GATA4 gene was carried out by PCR amplification followed by direct sequencings in 165 Egyptian patients with different nonsyndromic congenital heart diseases and 93 controls who were matched in t

Three new patients with congenital unilateral facial nerve palsy due to chromosome 22q11 deletion.

We report three unrelated patients with congenital facial nerve palsy and chromosome 22q11 deletion, a condition hitherto poorly recognized. In the first case, facial palsy was associated with aortic coarctation, ductus arteriosus, and ostium secundum atrial septal defect. In the second case, facial palsy was associated with ostium secundum atrial septal defect, obstruction of the ureteropelvic junction, double ureteropelvic-calicial system, and distal metaphyseal widening of the forearm and leg bones. In both cases, facial palsy was the presenting feature. In the third case, an ostium secundu

Congenital defects of the pericardium.

Congenital defects of the pericardium are rare, but when they are reported they are frequently associated with other cardiac lesions. We describe a case of partial pericardial defect found incidentally at surgery for closure of an ostium primum atrial septal defect. Proposed mechanisms of pericardial defect development are discussed and we suggest that associations with congenital and acquired heart disease are mostly circumstantial.

[Three different types of atrial septal defects in the same family].

Most of the time atrial septal defects (ASD) are sporadic. Familial forms are characterised by the same type of the ASD, and are frequently associated by other cardiac, osteoarticular (Holt-Oram syndrome) or atrioventricular conductions abnormalities (ostium secundum ASD with prolonged PR, sinus venosus ASD with sinusal bradyarrhythmia). This report describes a family in which the father and his two children present an ASD, each one of a different type (ostium primum ASD, ostium secundum ASD, sinus venosus ASD), without any other associated malformations. To the best of our knowledge, this is

[Gallbladder agenesis: a diagnostic problem?].

A rare case of an agenesis of the gallbladder in combination with cardiac malformation (anomalous pulmonary venous return with associated atrial septal defect) is reported. Despite modern diagnostic methods this agenesis which is often combined with a variety of malformations is first realized during operation or postmortem. Combinations of atypical clinical signs and sonografical or radiological absence of the gallbladder ("vésicule exclue") should point at agenesia of the gallbladder and to look for further malformations.

Congenital Complete Heart Block Complicated by Atrial Flutter Diagnosis and Management.

Seventeen-month-old child was diagnosed in utero to have congenital complete heart block. The mother has Sjogren's syndrome with high Anti Ro antibodies. The baby was delivered at term with a heart rate of 55-60 beats per minute. Echocardiography revealed a structurally normal heart with a small atrial septal defect and moderate patent ductus arteriosus. At the age of 17 months, he developed atrial flutter which was aborted using electrical cardioversion in the Cath lab. No recurrence of the atrial flutter during a one-year follow-up.

[Proceedings: Surgical closure of an atrial septal defect in a patient of over 40 years of age].

Between 1961 and 1972, 354 patients with atrial septal defect were treated surgically. Of these, 80 patients were aged over 40 years at the time of operation. The hospital mortality was 5%; 2 patients had increased preoperative pulmonary artery systolic pressure (50-75 mm Hg), while the other 2 had mitral or tricuspid incompetence. At late follow-up 82% of patients were free of symptoms and 18% had slight clinical signs of breathlessness on exertion. The heart size decreased in 37 of 10 patients, the electrocardiographic signs of right ventricular hypertrophy receded in 23 patients. The operat

Higher preoperative Qp/Qs ratio is associated with lower preoperative regional cerebral oxygen saturation in children with ventricular septal defect.

PURPOSE: The relationship between regional cerebral oxygen saturation (rSO2) and the amount of left-to-right shunt in ventricular septal defect (VSD) patients has not yet been investigated. The purpose of this study was to identify the association of preoperative pulmonary to systemic blood flow (Qp/Qs) ratio and preoperative rSO2 in patients with VSD. METHODS: We retrospectively evaluated 49 VSD surgical closure candidates at a single institution. Preoperative Qp/Qs ratio was compared with rSO2 measurements at the time of VSD closure surgery. RESULTS: Forty-nine were eligible for the final

[Recent strategies for a cyanotic heart disease].

Minimally invasive cardiac surgery has been performed for patients with patent ductus arteriosus (PDA), atrial septal defect (ASD), and ventricular septal defect (VSD). Video-assisted thoracoscopic interruption of PDA (VAT-SPDA) is effective for infants and neonates, especially for low birth weight babies. On the other hand, catheter intervention using PDA and ASD occluder devices have been widely used in our country. However, serious complications were reported in some papers. Recently, the device occlude has been applied to VSD. In percutaneous approach, there have been serious complications

Cor triatriatum sinister with incomplete atrioventricular septal defect in a cat.

An 11-month-old, 3 kg, female domestic shorthair cat was referred to evaluate cardiac structure and function. Echocardiography revealed the membrane dividing the left atrium into two chambers, a large defect in the lower part of the atrial septum, and turbulent blood flow from the distal left atrium into the right atrium. These findings suggested cor triatriatum sinister (CTS) with incomplete atrioventricular septal defect (AVSD). The cat was treated with medications for management of congestive heart failure. In the end, she died from right-sided heart failure 17 months after the initial pres

Absent pulmonary valve associated with tetralogy of Fallot and complete atrioventricular septal defect: report of a case.

A male infant with an extremely rare combination of absent pulmonary valve, tetralogy of Fallot and atrioventricular septal defect presented without symptoms of respiratory distress or congestive heart failure. He underwent successful primary repair at the age of 5 months. The procedure consisted of double-patch repair of the atrioventricular septal defect and right ventricular outflow tract reconstruction with a monocusp transannular patch. Resection or plication of a dilated pulmonary artery was not required. The patient is doing well without any symptoms 5 years after repair.

Successful repair of complete sternal cleft associated with congenital heart disease. Report of one case.

Total failure of sternal fusion without other developmental anomalies of the chest and abdominal wall is rare. We report on a 52-year-old man with a total cleft sternum associated exclusively with an ostium secundum type atrial septal defect, a large cono-ventricular septal defect and pulmonary stenosis, who underwent successful surgical repair of the congenital heart disease and of the sternal anomaly by direct approximation of the freshened sternal remnants to the midline without interposition of tissue graft or inert prosthesis. Detection of intracardiac left-to-right shunts in children by contrast echocardiography and the radionuclide technique.

This study was carried out in the Departments of Pediatrics and Nuclear Medicine at Hacettepe University Faculty of Medicine between April 1986 and March 1988 with the aim of detecting left-to-right shunts in 70 patients with isolated ventricular septal (VSD) defect, isolated atrial septal defect (ASD) and patent ductus arteriosus, by using the radionuclide technique together with contrast echocardiography. In some patients, the results were confirmed by both cardiac catheterization and angiocardiography. Most of the data were confirmed during surgery. A good correlation was found between the

Long-term follow-up of a fenestrated Amplatzer atrial septal occluder in pulmonary arterial hypertension.

Pulmonary arterial hypertension (PAH) is a progressive disease, with right-heart failure being the main cause of death. In patients refractory to conventional drug therapy, atrial septostomy can serve as palliative treatment or as a bridge to transplantation. A 41-year-old woman with a 15-year history of PAH associated with a corrected atrial septal defect presented with severe deterioration of symptoms. Echocardiography confirmed reocclusion of an atrial septal stoma that had been created several months before. After performing a repeat atrial septostomy, we implanted a custom-made atrial sep

Identification of TBX5 mutations in a series of 94 patients with Tetralogy of Fallot.

Tetralogy of Fallot (TOF) (OMIM #187500) is the most frequent conotruncal congenital heart defect (CHD) with a range of intra- and extracardiac phenotypes. TBX5 is a transcription factor with well-defined roles in heart and forelimb development, and mutations in TBX5 are associated with Holt-Oram syndrome (HOS) (OMIM#142900). Here we report on the screening of 94 TOF patients for mutations in TBX5, NKX2.5 and GATA4 genes. We identified two heterozygous mutations in TBX5. One mutation was detected in a Moroccan patient with TOF, a large ostium secundum atrial septal defect and complete atrioven

[Surgery of atrial septal defect in patients aged over 40 years: comparative study of direct suture and patch closure].

Thirty patients over 40 years of age with atrial septal defects were operated on between 1981 and 1989. They were divided into two groups according to operative technique, group A of patients with direct suture of defect (17 cases) and group B patch closure (13 cases), in order to investigate the incidence of postoperative complications. Patient age and size of defect in group B were both greater than in group A. One group A patient had pulmonary stenosis, two group B patients had mitral valvular disease and six group B patients had pulmonary hypertension. The incidence of postoperative heart
