Monitoring the respiratory rate by miniature motion sensors in premature infants: a comparative study.

OBJECTIVE: Existing respiratory rate (RR) monitors suffer from inaccuracy. The study assesses the accuracy of a novel modality that monitors lung ventilation with miniature motion sensors. STUDY DESIGN: RR was measured by three methods: impedance technology, motion sensors and visual count, in babies (n=9) that breathed spontaneously or with respiratory support and babies (n=12) that received high-frequency oscillatory ventilation (HFOV). RESULTS: A line close to equality (slope=0.96, r(2)=0.83) was obtained between the motion sensor and the visual count of the RR with narrow 95% limits of a

Impact of new treatments for neonatal pulmonary hypertension on extracorporeal membrane oxygenation use and outcome.

OBJECTIVE: To determine the impact of new treatment modalities, including high-frequency oscillatory ventilation (HFOV) and inhaled nitric oxide (INO), on extracorporeal membrane oxygenation (ECMO) use and outcome of neonatal patients with persistent pulmonary hypertension of the newborn. STUDY DESIGN: We reviewed the medical records of neonatal patients meeting established ECMO criteria from 1988 to 1995. Clinical data were gathered from this retrospective chart review. Comparison of ECMO experiences were made between the 1988-90 period (pre-HFOV and INO, or period 1) and the 1993-95 period

Neonatal respiratory support strategies for the management of extremely low gestational age infants: an Italian survey.

BACKGROUND: We aimed to survey Delivery Room and Neonatal Intensive Care Unit (NICU) respiratory strategies dedicated to the extremely low gestational age newborn (ELGAN - GA < 28 wks) in Italy. METHODS: A questionnaire was sent to 113 Italian level III centres. A lead physician and a nurse with expertise in mechanical ventilation (MV) were identified in each unit to answer. Information about those aspects of ventilatory support considered by center's staff as needing improvement was also collected. RESULTS: A 100% response rate was obtained. In the Delivery Room, sustained lung inflation wa

High versus low-dose caffeine for apnea of prematurity: a randomized controlled trial.

UNLABELLED: The optimum caffeine dose in preterm infants has not been well investigated. We aimed to compare the efficacy and safety of high versus low-dose caffeine citrate on apnea of prematurity (AOP) and successful extubation of preterm infants from mechanical ventilation. We compared high-dose (loading 40 mg/kg/day and maintenance of 20 mg/kg/day) versus low-dose (loading 20 mg/kg/day and maintenance of 10 mg/kg/day) caffeine citrate in preterm infants <32 weeks gestation, presented with AOP within the first 10 days of life. A total of 120 neonates (60 in each group) were enrolled. High-d

Characteristics of the Five Different Types of Intermittent Mandatory Ventilation.

Intermittent mandatory ventilation (IMV) has evolved substantially from its original form, developing into 5 distinct types intended to enhance patient safety, comfort, and liberation from mechanical ventilation. Each form of IMV has different potential advantages and disadvantages that should be considered when selecting the mode of ventilation. IMV(1): the set mandatory breath rate is always delivered. IMV(2): mandatory breaths may be suppressed by spontaneous breaths if their frequency is high enough. IMV(3): mandatory breaths may be suppressed by spontaneous breaths if they achieve some pr

Pulmonary interstitial emphysema after resolution of relapsing pneumothorax and discontinuation of mechanical ventilation. An atypical case in a preterm infant.

UNLABELLED: Pulmonary interstitial emphysema (PIE) is a frequent complication in preterm-ventilated infants. These newborns have an increased risk of mortality and bronchopulmonary dysplasia. Various treatments for PIE have been proposed: high-frequency ventilation, postural therapy, selective bronchial intubation or surgery. We present an atypical case of a preterm infant with respiratory distress syndrome complicated by pneumothorax, who after recovery and discontinuation of mechanical ventilation, developed unilateral PIE which was treated with selective bronchial intubation. CONCLUSIONS: …

Hunsaker Mon-Jet tube ventilation: a 15-year experience.

OBJECTIVE/HYPOTHESIS: The Hunsaker Mon-Jet tube (HMJT) (Xomed, Jacksonville, FL) has been used effectively for subglottic ventilation. We previously reported a series of 552 patients over a 10-year period with no major complications. This is a continuation of that series with an additional 5 years of cases. STUDY DESIGN: Retrospective consecutive case series. METHODS: Patients who were ventilated with the HMJT for microlaryngeal surgery at the University of Washington Medical Center over a 15-year period (1995-2010) were identified from the Voice Disorders database. Charts were reviewed for

Risk factors for pulmonary artery hypertension in preterm infants with moderate or severe bronchopulmonary dysplasia.

BACKGROUND: Despite the potential importance of pulmonary artery hypertension (PAH) in preterm infants with bronchopulmonary dysplasia (BPD) … PAH had more severe cases of BPD and underwent longer durations of oxygen therapy, conventional or high-frequency ventilation, and hospitalization compared to those without PAH. Low 5-min Apgar scores (≤6; relative risk (RR) 6.2; 95% confidence interval (CI) 1.4-28.0; p = 0.017) and oligohydramnios (RR 7.7; 95% CI 2.0-29.6; p = 0.030) were found to be significant risk fact …

Transtracheal jet ventilation in 50 patients with severe airway compromise and stridor.

BACKGROUND: Management of the airway is difficult in patients with pharyngeal or laryngeal pathology caused by malignancy, extensive surgery, or radiotherapy scarring, particularly when undergoing pharyngolaryngeal surgery. Tracheal intubation, with or without fibreoptic guidance, is often impractical because of the severe glottic stenosis and primary tracheostomy under local anaesthesia has been the preferred technique. However, complication rates as high as 30% have been reported after primary tracheostomy and there is the potential for long-term morbidity. High-frequency jet ventilation (HF

Visual and fine-motor outcomes in adolescent survivors of high-risk congenital diaphragmatic hernia who did not receive extracorporeal membrane oxygenation.

OBJECTIVE: Although survivors of congenital diaphragmatic hernia (CDH) are at high risk for brain injury, little is known about their neurodevelopment. Studies exploring short-term outcomes in children who received extracorporeal membrane oxygenation (ECMO) therapy suggest an increased risk for abnormalities in tone and/or motor development. This study provides the first detailed examination of visual and fine-motor outcomes in adolescent survivors of high-risk CDH (manifesting within the first 24 h) who did not receive ECMO. STUDY DESIGN: A total of 13 CDH survivors (mean age 12.9 years) and

Effects of open vs. closed system endotracheal suctioning on cerebral blood flow velocities in mechanically ventilated extremely low birth weight infants.

BACKGROUND: Endotracheal (ET) suctioning causes cardiovascular side effects and may impair cerebral hemodynamics. Subjectively, these effects are worse if patients are disconnected from the ventilator (open system suctioning, OSS) than if they remain connected to the ventilator during suctioning (closed system suctioning, CSS). It is uncertain whether the response to ET suctioning is similar in conventionally (CV) and high frequency (HF) ventilated patients. OBJECTIVES: To investigate if the mode of suctioning or of mechanical ventilation influences cerebral blood flow velocities (CBFVs) in e

[Surfactant therapy in severe neonatal respiratory failure--multicenter study--II. Surfactant therapy in 10 newborn infants with meconium aspiration syndrome].

As a part of a west german multicenter study (424 patients, 16 hospitals) 10 term neonates ventilated because of severe meconium aspiration syndrome were treated with 1-4 doses of 50 mg/kg/BW of a bovine surfactant (Alveofact). Before treatment respiratory distress was severe (median FiO2: 1.0, median MAD 9.9 mmHg, median OI: 20). Acute improvement ("responders") was shown in 4 patients. All patients survived. Time of mechanical ventilation was 6-26 d. High frequency ventilation was applied in two nonresponders, ECMO in one.

New parachute cuff and positive end-expiratory pressure to minimize tracheal injury and prevent aspiration.

A new parachute cuff has been tested in combination with a positive end-expiratory pressure (PEEP) on mongrel dogs. During positive-pressure ventilation the intracuff and intratracheal pressures showed synchronous, identical pressure variations, and therefore theoretically with this type of cuff the pressure against the tracheal wall would be minimal. The cuff provided a seal against gas leakage from the lungs throughout the entire test period, i.e., for up to 7 h. To avoid aspiration of mouth contents during the passive exhalation phase, different amounts of PEEP were tested. A PEEP of 4.0--8

Anteromedial pneumothorax in a neonate: 'the diagnostic dilemma' and the importance of clinical signs.

A premature neonate born at 32 weeks of gestation was admitted to the neonatal unit with respiratory distress syndrome. The infant received late rescue surfactant therapy with continued mechanical ventilation in view of continuous positive airway pressure (CPAP) failure. Owing to worsening distress and an air leak, he was switched over to high-frequency oscillatory ventilation. The air leak required drainage for possible pneumopericardium. This was initially attempted ineffectually with echo-assisted pericardial drainage, and later successfully with the use of chest tubes for anteromedial pneu

[Refractory hypoxemia treated early by HFO in a severe traumatic brain injury patient].

A 23-year-old man without previous medical history, was transferred to our surgical intensive care unit for management of a traumatic brain injury (Glasgow Coma Score of 3 on admission). He rapidly presented a refractory hypoxaemia essentially due to posterior alveolar collapse. Severe hypoxaemia and hypercarboxaemia didn't respond to conventional ventilation and complicated the management of the brain injury and the control of intracranial pressure. The introduction of high-frequency oscillatory ventilation permitted a respiratory improvement in 48 hours and a good neurological outcome.

Controlled cycles in spontaneous-timed noninvasive ventilation: Incidence and associated factors.

BACKGROUND: The most-used ventilation mode in home mechanical ventilation (HMV) is spontaneous-timed, designed to be essentially spontaneous with a programmed backup rate. RESEARCH QUESTION: We do not know the real frequency of activation of controlled cycles, nor its associated factors. STUDY DESIGN: and Methods: We conducted a single-center cohort study of patients with chronic hypoventilation who were started on HMV. We collected the clinical variables, the ventilator programming parameters and the ventilation efficacy data obtained from the built-in software. We analyzed the percentage o
