Digitising controlled substance accountability: implementation and use of a dashboard-based monitoring system in an university hospital.

OBJECTIVES: Opioids are the cornerstone of severe pain management but they have a strong potential for abuse. To mitigate this risk, hospitals are required to track and account for these controlled substances (CS). Despite widespread adoption of electronic medication administration records (eMAR), nurses also performed paper-based registrations for CS. We hypothesised that a validated dashboard combining dispensing and eMAR data could be implemented for CS accountability. This study describes trends in accountability of dispenses through administrations before, during and after implementation

Prehospital fluid resuscitation in patients with severe burns - A nationwide cohort study.

BACKGROUND: Prehospital fluid administration is widely used in patients with severe burns to stabilize hemodynamics and organ perfusion, but its effect on outcomes remains unclear. We examined the association between prehospital fluid administration and early mortality in patients with severe burns. METHODS: In this retrospective observational study using Japan's nationwide trauma registry, we identified patients with severe burns - defined as burns involving ≥ 20% of the total body surface area (TBSA) in patients 5 years of age or older, or ≥ 10% in younger children - who were transported di

Prehospital tranexamic acid shortens the interval to administration by half in Major Trauma Networks: a service evaluation.

INTRODUCTION: Tranexamic acid (TXA) reduces bleeding and mortality. Recent trials have demonstrated improved survival with shorter intervals to TXA administration. The aims of this service evaluation were to assess the interval from injury to TXA administration and describe the characteristics of patients who received TXA pre-hospital and in-hospital. METHODS: We reviewed Trauma and Audit Research Network records and local trauma registries to identify patients of any age that received TXA at all London Major Trauma Centres and Queen's Medical Centre, Nottingham, during 2017. We used the 2016

The Association of Residential Instability and Hospitalizations among Homeless and Vulnerably Housed Individuals: Results from a Prospective Cohort Study.

The present study examined the association of residential instability with hospitalizations among homeless and vulnerably housed individuals over a 4-year time period. Survey data were linked to administrative records on hospitalizations. Specifically, we used data from the Health and Housing in Transition study, a prospective cohort study that tracked the health and housing status of homeless and vulnerably housed individuals in Canada. Responses from Vancouver-based participants (n = 378) from baseline and 3 follow-ups were linked to their administrative health records on hospitalizations (D

Cost savings of home bortezomib injection in patients with multiple myeloma treated by a combination care in Outpatient Hospital and Hospital care at Home.

At home injectable chemotherapy for patients receiving treatment for hematological diseases is still in debate. Given the expense of new innovative medicines, at home treatment has been proposed as a suitable option for improving patient quality of life and decreasing treatment costs. We decided to assess the cost of bortezomib administration in France among multiple myeloma patients from an economic standpoint. Patients in this study were treated within a regional hematological network combining outpatient hospital care and Hospital care at Home administration. To make the cost comparison, ou

Trends in methicillin-resistant Staphylococcus aureus bloodstream infections using statewide population-based surveillance and hospital discharge data, Connecticut, 2010-2018.

We compared methicillin-resistant Staphylococcus aureus (MRSA) bloodstream infections (BSIs) captured by culture-based surveillance and MRSA septicemia hospitalizations captured by administrative coding using statewide hospital discharge data in Connecticut from 2010 to 2018. Observed discrepancies between identification methods suggest administrative coding is inappropriate for assessing trends in MRSA BSIs.

Safety evaluation of outpatient ifosfamide regimens in adult sarcoma patients.

INTRODUCTION: Transitioning historically inpatient chemotherapy regimens to outpatient administration can reduce hospital stays, resource use, and healthcare expenditures while improving patient quality of life. However, agents like ifosfamide, commonly used in sarcoma, often necessitate inpatient administration for close monitoring of adverse effects. The Fred Hutchinson Cancer Center (FHCC) sarcoma group has developed criteria for outpatient ifosfamide administration after one successful inpatient administration. Nevertheless, there remains a paucity of literature characterizing the safety p

The Additive Effect of Atropine Sulfate during Cardiopulmonary Resuscitation in Out-of-hospital Non-traumatic Cardiac Arrest Patients with Non-shockable Rhythm.

Objective The updated guidelines of 2015 for cardiopulmonary resuscitation (CPR) do not recommend the routine use of atropine for cardiopulmonary arrest. Methods The study population included out-of-hospital cardiac arrest (OHCA) patients with non-shockable rhythm who were encountered at a Japanese community hospital between October 1, 2012 and April 30, 2017. Results At the outcome, the epinephrine with atropine and epinephrine-only groups had a similar survival rate to that at hospital admission (28.7% vs. 26.7%: p=0.723). The odds ratio (OR) for the survival to hospital admission after the

Developing a Value Framework: Utilizing Administrative Data to Assess an Enhanced Care Initiative.

BACKGROUND: There remains no tool to quantify the total value of comparative processes in health care. Hospital administrative data sets are emerging as valuable sources to evaluate performance. Thus, we use a framework to simultaneously assess multiple domains of value associated with an enhanced recovery initiative using national administrative data. MATERIALS AND METHODS: Risk-stratified clinical pathways for patients undergoing pancreatic surgery were implemented in 2016 at our institution. We used a national administrative database to characterize changes in value associated with this in

[Perceptions of the organization and the practice of dental healthcare by the professionals working in prisons].

INTRODUCTION: Few studies have been conducted on the role and position of dental surgeons in prisons. Interestingly though, dental surgeons perform in a peculiar working environment, since the organizations are divided into two administrative structures: penitentiary and hospital administrations. Stakeholders’ perceptions of both the current oral healthcare organization in prisons and the interactions between prison and hospital administrations’ professionals will be at the core of this study. PURPOSE OF RESEARCH: This article aims at assessing hospital and penitentiary administra

Liquid risperidone in the treatment of rages in psychiatrically hospitalized children with possible bipolar disorder.

OBJECTIVE: To examine the safety and efficacy of liquid risperidone to reduce duration of rages in children with severe mood dysregulation (SMD) or possible bipolar disorder (BP). METHOD: The sample included 151 consecutive admissions of 5-12 year old children to a psychiatric inpatient unit. Diagnostic information and history of prior rage outbursts were obtained at admission. In hospital, a first rage was treated with seclusion. If a second rage occurred, the child was offered liquid risperidone to help him/her regain control. Durations of unmedicated and last medicated rage were compared.

Responses of mental health professionals to lay-offs.

In this study, 39 laid-off mental health professionals responded to a questionnaire that focused on the emotional stress of being laid off and the respondents' efforts to cope with the stress. In the early weeks after lay-off, shock, disbelief, anger, and sadness gave way to depression, anxiety, and feelings of being betrayed and discounted. Most respondents coped with these feelings by talking to others who were being laid off or by focusing their energies on seeking a new job. Many respondents felt that mismanagement of the lay-off process was a major factor in their emotional distress. The

Diversification of health care services: the effects of ownership, environment, and strategy.

The present findings suggest that the trend toward greater diversification of hospital services is likely to be most strongly influenced by state Medicaid policies and certain hospital characteristics. Increasing Medicaid eligibility and payment levels is likely to have a positive effect on services diversification. Growth in the number of inpatient services provided and a more severe case mix are also likely to be involved with greater service diversification. Affiliation with a not-for-profit hospital system is likely to be associated with more diversified hospital services but not affiliati

Atypical crusted "Norwegian" scabies: report of nosocomial transmission in a community hospital and an approach to control.

During August, 1981, a person with an unrecognized case of atypical Norwegian scabies was admitted to a community hospital in Chariton, Iowa. Twenty cases of symptomatic scabies were reported among hospital staff; mites were recovered from four. Subsequent evaluation confirmed scabies transmission to family and friends of this patient before hospitalization; twelve cases of symptomatic scabies, three of them slide positive, were identified in this group. The patient was treated sequentially with 1 percent lindane lotion, 10 percent crotamiton lotion, and 6 percent sulfur ointment to successful
