Surgical treatment of complex distal humeral fractures: functional outcome after internal fixation using precontoured anatomic plates.

HYPOTHESIS: Several studies have shown good results with internal fixation of distal humeral fractures; however, few have focused specifically on anatomic parallel plate fixation using the same implant and postoperative regimen. The purpose of this study was to determine the functional outcome after open reduction and internal fixation of these complex fractures using parallel precontoured anatomic plates. MATERIALS AND METHODS: This was a retrospective single-surgeon series involving 16 patients (12 women, 4 men) treated with a double-column parallel plating technique. Clinical assessment in…

The reverse shoulder prosthesis for primary and secondary treatment of proximal humeral fractures: a case report.

Humeral head fractures are very common among elderly people, often requiring shoulder arthroplasty. One requirement for good function after … he right side. The primary or secondary implantation of the reverse shoulder prosthesis in proximal humeral fractures has to be planned carefully, since long-term results are still lacking and treatment options after failed reverse shoulder arthroplasty are few. Generally, primary implantation of traditional fracture prosthesis is indicated in most cases of humeral …

Does the presence of a full-thickness rotator cuff tear influence outcome after proximal humeral fractures?

BACKGROUND: Prevalence of rotator cuff tears increases with advancing age. Despite proximal humeral fractures being common in the elderly, the influence of a coexistent rotator cuff tear on outcome has, to our knowledge, not been previously investigated. This study prospectively assessed whether the presence of a rotator cuff tear in association with a proximal humeral fracture influences functional prognosis. METHODS: Eighty-five patients treated conservatively for proximal humeral fractures were evaluated prospectively with ultrasonography to determine the status of the rotator cuff. Every …

Nerve lesions in primary shoulder dislocations and humeral neck fractures. A prospective clinical and EMG study.

The incidence of nerve injuries in primary shoulder dislocation and humeral neck fracture is uncertain. We made a prospective study of 101 patients, using clinical examination and extensive electrophysiological assessment when there was suspicion of nerve damage. We found electrophysiological evidence of nerve injury in 45%, most involving the axillary, suprascapular, radial and musculocutaneous nerves. There were significantly more nerve injuries in older patients and those with a haematoma. Most patients recovered partially or completely in less than four months, and only eight had persisten

Postoperative complications of distal humeral fractures. 27/96 adults followed up for 6 (2-10) years.

Of 96 adult patients with a distal intercondylar, intraarticular fracture of the humerus treated operatively during a 10-year period, 27 patients suffered a local complication, while the total number of individual complications were 34. There were six postoperative infections, 12 neural injuries, and 16 fixation failures. Because of the complication, a reoperation was performed in 12 of the 27 patients. The final outcome was assessed at a reexamination after a mean follow-up period of 6 (2-10) years. Three patients had a permanent dysfunction of the ulnar nerve. In another 3 patients, failure

Influence of soft tissue composition and arm diameter on fracture strain in simulated humeral shaft fractures undergoing functional bracing.

INTRODUCTION: Functional bracing is a common non-operative treatment for humeral shaft fractures. The effects of patient-specific soft tissue characteristics on fracture site biomechanics during bracing are poorly understood and may alter healing. This study leveraged finite element analysis (FEA) to characterize the impact of arm diameter and muscle-adipose composition on fracture site strain during bracing. In conjunction with other factors, researchers and clinicians may apply these findings toward optimizing outcomes in patients with humeral shaft fractures. METHODS: Nine humerus FEA mode

Displaced humeral shaft fractures: Assessment of fracture union and complications following dual plate fixation using an anterior approach.

The humeral bone is subject to torsional forces. In case of displaced shaft fractures, internal fixation remains the standard of care. This retrospective two-center study assessed the fracture union rate and complications after dual 3.5 mm locking compression plate (LCP) fixation using an anterolateral approach. Over a 9-year period, 38 patients underwent surgery in two centers. They had a mean age of 53.7 years (15-97, ± standard deviation (SD) 26) and there were three open fracture cases (7.9 %). The dominant side was affected in 21 cases (55.3 %) and there were 11 polytrauma patients (29 %)

Minimally invasive anatomical fixation of distal humeral fractures: Direct reduction through the anterior MIPO distal window.

Conventional surgical treatment for distal humeral shaft fractures consists of osteosynthesis through an open posterior approach. This technique requires a wide incision, extensive muscular dissection, and direct handling of the radial nerve, resulting in considerable surgical morbidity. Although the anterior MIPO technique represents a less invasive alternative and avoids these drawbacks, it has traditionally been associated with indirect reduction and relative stability alone. In distal humeral shaft fractures, the distal window of the anterior MIPO approach is located directly above the fra

Continuous Costoclavicular Brachial Plexus Block for Humeral Fractures in Pregnancy: A Case Report and Literature Review.

Brachial plexus block is the predominant anesthetic method used for upper-limb surgical procedures in pregnant patients. The innovative method of brachial plexus block in the costoclavicular space has shown enhanced reliability and effectiveness for postoperative analgesia. We report a case of a pregnant woman who underwent surgery for a humeral fracture. Continuous costoclavicular brachial plexus block was successfully employed to facilitate the reduction and stabilization of the humeral fractures. This case report serves as an initial assessment of the efficacy of the costoclavicular brachia…

Stemmed hemiarthroplasty with a suture collar and a common platform system for acute proximal humeral fractures.

BACKGROUND: Hemiarthroplasty for acute proximal humeral fractures gives disappointing results, often due to rotator cuff insufficiency. Better tuberosity fixation might improve results. Therefore, the aim of this study was to: 1) report the outcome of a stemmed hemiarthroplasty with a common platform system and a modular suture collar; 2) compare the outcome with that of a standard stemmed hemiarthroplasty; 3) report the feasibility of revision arthroplasty with retention of the stem; and 4) to evaluate the association between tuberosity healing and functional outcome. METHODS: Forty-four fra…

Anatomically reduced fixation should always be considered when treating B and C proximal epiphyseal humeral fractures.

BACKGROUND: Proximal humeral fractures are commonly observed in elderly patients. Management of these injuries is controversial. Literature comparing locking plate fixation, arthroplasty, and conservative treatments show no clear advantages for any of these management strategies. Thus far, no study has considered anatomically reduced fractures obtained after locking plate treatment. To clarify the best surgical procedure in middle-aged patients, we considered outcomes and major complications leading to surgical revision following an anatomically reduced fracture fixed with locking plate and re…
