Recurrent shoulder stabilization with open bankart repair and long head biceps transfer.

There are several treatment options for recurrent shoulder instability. The Latarjet addresses bone loss via coracoid transfer, and creates a "sling effect" on the humeral head with the conjoint tendon. It does however carry a high potential risk of complications including graft resorption, hardware failure, and neurologic injury. It is hypothesized that the long head of the biceps can function similarly, without the donor site morbidity of a coracoid transfer. We present a case of recurrent instability and musculocutaneous nerve palsy following primary arthroscopic stabilization three years p…

[Shoulder instability: diagnostic imaging].

Shoulder instability is often diagnosed among athletes; two clinical forms are distinguished: anatomical instability, with recurrent luxation of the shoulder, and functional instability, with pain, articular "click" and sensation of instability. Lesions of periarticular soft tissues (capsula, fibrocartilaginous labrum, gleno-humeral ligaments and rotator cuff) are common in both forms, while lesions of bone structures (humeral head and glenoid of scapula) are typical of shoulder with previous dislocation. Purpose of our retrospective study was to verify the value of magnetic resonance (MRI) an…

[The unstable shoulder in young subjects. Surgical experience].

The stability of the scapulo humeral joint depends on three anatomical factors: bony: surface and integrity of the glenoid cavity. ligamentous: joint capsula and labrum glenoïdale. muscular: coordination and condition of the components of the rotator cuff that maintains the humeral head in the center of the joint, whatever be the position of the arm. Clinically, the most frequent aspect is the anterior recurrent dislocation, occurring in young patients.... Other forms are possible: posterior, multi directional, associated with a lesion of the rotator cuff in elderly patients.... Lately a new a…

Risk factors for glenoid erosion in patients with shoulder hemiarthroplasty: an analysis of 118 cases.

BACKGROUND: Glenoid erosion is one of the main concerns in shoulder hemiarthroplasty. The goal of this study was to quantify glenoid erosion … local destruction of the cartilage. The degree of erosion did not correlate with age and glenoid or humeral head size. Only 1 patient (of 30) with a fracture-type prosthesis developed progressive glenoid erosion. CONCLUSION: In this series, favorable conditions for resistance to erosion after hemiarthroplasty were lack of glenoid cysts, intact glenoid cartila …

Capsular tear in line with the inferior glenohumeral ligament: a cause of anterior glenohumeral instability in 2 patients.

Anterior glenohumeral instability typically involves lesions associated with the inferior glenohumeral ligament complex. Multiple lesions ha … of force involved in a dislocation. Arthroscopy revealed a capsular defect from the glenoid to the humeral head in the anterior inferior glenohumeral ligamentous complex in both. This lesion is an unusual circumstance, providing another pathology to include in the differential diagnosis of anterior glenohumeral instability.

BACKGROUND: The treatment of unstable displaced proximal humeral fractures, especially in the elderly, remains controversial. The objective … omplication, noted in twenty-one (14%) of 155 patients, was intraoperative screw perforation of the humeral head. Twenty-nine patients (19%) had an unplanned second operation within twelve months after the fracture. Radiological Diagnosis of Axillary Artery and Brachial Plexus Involvement in a Proximal Humerus Fracture-Dislocation: A Case Report.

Proximal humerus fracture-dislocations are uncommon in elderly patients and rarely associated with neurovascular compromise. The anatomical proximity of the axillary artery and brachial plexus to the humeral head places them at risk during trauma, but diagnosis can be challenging when distal pulses are preserved. We present the case of a 79-year-old female who sustained a fracture-dislocation of the proximal humerus following a low-energy fall. CT angiography demonstrated humeral head displacement into the axilla with bony impingement on the axillary artery compression and brachial plexus tent…

Locked posterior dislocation of the shoulder: treatment using rotational osteotomy of the humerus.

This study reports the use of rotational osteotomy in 10 patients with locked posterior dislocation of the shoulder. The average interval between injury and diagnosis was 155 days (range, 21-400 days). Patients were aged 40-78 years old, with an average age of 53 years. Articular impaction fracture of the humeral head involved 20-40% of the articular surface as determined by CT analysis. There were no postoperative complications except a transient axillary nerve palsy. Patients were started on immediate passive motion followed by an active program at 2-3 weeks. Using the Rowe/Zarins scale, six…

Validity and reliability of shoulder kinematics in typically developing children and children with hemiplegic cerebral palsy.

Shoulder motion has been mainly analysed in children based on thoraco-humeral (TH) joint kinematics, excluding the scapula-thoracic (ST) and gleno-humeral (GH) joints. In order to measure 3D scapulo-humeral motion using an optoelectronic system, we propose a protocol based on an acromion marker cluster (AMC), a functional method to determine the gleno-humeral rotation centre and different Euler sequences. This study investigated the validity of the AMC compared to the palpation of anatomical landmarks with a scapula locator, assessed the intra-session repeatability and the ability to discrimin

Transfer of pectoralis major for the treatment of irreparable tears of subscapularis: does it work?

Transfer of pectoralis major has evolved as the most favoured option for the management of the difficult problem of irreparable tears of sub … transfer was highest in group II and was associated with pre-operative anterior subluxation of the humeral head. We conclude that in patients with irreparable rupture of subscapularis after shoulder replacement there is a high risk of failure of transfer of pectoralis major, particularly if there is pre-operative anterior subluxation of the humeral head.

Unusual Variation of the Biceps Brachii with Possible Median Nerve Entrapment.

The biceps brachii is one of three muscles of the anterior compartment of arm. Variations of the biceps brachii are not rare. The most frequent is the existence of a third head called the humeral head by Le Double (1897) (Rodríguez-Vázquez et al., 1999). Our article is based on the unexpected result of a routine dissection class held for medical students. Dissection was performed according to the guidelines accepted by the anatomy department (Seichert, 1999). We describe a third (accessory) head of the biceps brachii. In addition of two regular heads, the third head originated together with th…
