Oral rehydration in hypernatremic and hyponatremic diarrheal dehydration.

Ninety-four well-nourished, bottle-fed infants with hypernatremic (N = 61) or hyponatremic (N = 33) diarrheal dehydration were treated with oral rehydration. In 61 hypernatremic and 25 hyponatremic infants, two thirds of the fluid volume were given as glucose/electrolyte solution containing 90 mmole of sodium per liter and one third as plain water; the other eight hyponatremic infants were given glucose/electrolyte solution alone. Fluid deficits were successfully and rapidly replaced with oral therapy alone in all 61 hypernatremic infants (mean +/- SEM, 8.5 +/- 0.6 hours) and in 31 of those wi

Preoperative sodium abnormalities and adverse postoperative outcomes in older surgical patients: association and incremental predictive value in the BioCog cohort.

PURPOSE: To determine whether preoperative sodium abnormalities provide incremental predictive value for estimating postoperative delirium (POD), mortality, and complications. METHODS: For this observational cohort study, we included older patients (≥ 65 years) without dementia who were scheduled for elective surgery with an anticipated duration of ≥ 60 min. Routinely obtained sodium values below 135 mmol/L defined hyponatremia and that above 145 mmol/L hypernatremia. We assessed our primary outcome, postoperative delirium, by using a set of four validated tests. Our secondary, exploratory ou…

Exercise-associated hyponatremia was uncommon (<2%), whereas hypernatremia was substantially more prevalent. Early race editions reported isolated deaths, likely due to ischemic heart disease. Use of hypertonic saline in the treatment of severe refractory posttraumatic intracranial hypertension in pediatric traumatic brain injury.

OBJECTIVES: To evaluate the effect of prolonged infusion of 3% hypertonic saline (514 mEq/L) and sustained hypernatremia on refractory intracranial hypertension in pediatric traumatic brain injury patients. DESIGN: A prospective study. SETTING: A 24-bed Pediatric Intensive Care Unit (Level III) at Children's Hospital. PATIENTS: We present ten children with increased intracranial pressure (ICP) resistant to conventional therapy (head elevation at 30 degrees, normothermia, sedation, paralysis and analgesia, osmolar therapy with mannitol, loop diuretic, external ventricular drainage in five pa…

Effects of prenatal steroids on water and sodium homeostasis in extremely low birth weight neonates.

OBJECTIVE: We sought to determine if prenatal steroid (PNS) treatment affects water and sodium (Na) balance in extremely low birth weight in … : PNS treatment was associated with lower estimated insensible water loss, a decreased incidence of hypernatremia, and an earlier diuresis and natriuresis in extremely low birth weight neonates. We speculate that PNS effects these changes through enhancement of epithelial cell maturation improving skin barrier function. PNS treatment may also enhance lung Na, K …

Diabetic Ketoacidosis-Associated Brain Injury: A Rare but Serious Complication.

Diabetic ketoacidosis (DKA)-associated brain injury is a rare but serious complication, typically occurring early during metabolic correction and associated with a poor prognosis. We report the case of an 18-year-old college student admitted to the intensive care unit with severe DKA, characterized by profound metabolic acidosis and marked electrolyte imbalances, including hypokalemia, hypernatremia, and hyperchloremia. Within hours, she developed altered mental status and generalized seizures. Brain magnetic resonance imaging (MRI) revealed diffuse cortical and brainstem abnormalities, sugges…

However, it is important to note that the use of HTS comes with potential risks, such as hypernatremia, hyperchloremia, and fluid overload. Due to its unique properties, HTS is a crucial agent in the management of IH, and understanding its appropriate use is essential to optimize patient outcomes.

A case of ifosfamide-induced acute kidney injury, Fanconi syndrome, and nephrogenic diabetes insipidus.

Ifosfamide, a cytotoxic antineoplastic drug, can induce rare complications of Fanconi syndrome and nephrogenic diabetes insipidus (DI). Ifos … Fanconi syndrome, which were managed by electrolyte replacement therapy. In addition, polyuria and hypernatremia were considered due to nephrogenic DI, which partially responded to desmopressin treatment. This case highlights the importance of the routine electrolytes monitoring after ifosfamide treatment.

Central Diabetes Insipidus Masked by Uncontrolled Diabetes Mellitus: A Challenging Case Managed With Indapamide.

A 44-year-old man with a history of traumatic brain injury (TBI) presented to the emergency room (ER) with diabetic ketoacidosis (DKA). After resolution of DKA, the patient had persistent polyuria (up to 5.5 L/24 h) associated with low specific gravity (1.002-1.005) and severe hypernatremia (up to 186 mmol/L) that led us to consider the possibility of central diabetes insipidus (DI). Due to the lack of desmopressin availability in our country, we managed the patient using indapamide. Polydipsia and polyuria in a patient with controlled diabetes mellitus (DM) should raise suspicion for alternat…

Idiopathic Fanconi's syndrome with nephrogenic diabetes insipidus in a child who presented as vitamin D resistant rickets--a case report and review of literature.

Fanconi's syndrome is a complex of multiple tubular dysfunctions of proximal tubular cells occurring alone or in association with a variety of inherited (primary) or acquired (secondary) disorders. It is characterized by aminoaciduria, normoglycemic glycosuria, tubular proteinuria without hematuria, metabolic acidosis without anion gap and excessive urinary excretion of phosphorous, calcium, uric acid, bicarbonate, sodium, potassium and magnesium. Diabetes insipidus is a disease of collecting tubules and a child mainly presents with dehydration and hypernatremia. We report the first case of id…

METHODS: Thirty-seven HH patients, 26 males and 11 females, aged 8.7 underwent resection of the tumor via the transcallosal transseptal interforniceal approach. The symptoms and blood electrolytes were observed. RESULTS: After operation blood electrolyte test showed that 8 cases were normal, 19 cases showed hyponatremia as prime abnormality, and 10 cases had hypernatremia primarily. Among the 19 cases with hyponatremia transient apath…
