The effect of inhaled hypertonic saline on lung structure in children aged 3-6 years with cystic fibrosis (SHIP-CT): a multicentre, randomised, double-blind, controlled trial.

BACKGROUND: In the Saline Hypertonic in Preschoolers (SHIP) study, inhaled 7% hypertonic saline improved the lung clearance index in children aged 3-6 years with cystic fibrosis, but it remained unclear whether improvement is also seen in structural lung disease. We aimed to assess the effect of inhaled hypertonic saline on chest CT imaging in children aged 3-6 years with cystic fibrosis. METHODS: Children with cystic fibrosis were enrolled in this multicentre, randomised, double-blind, controlled study at 23 cystic fibrosis centres in Spain, Denmark, the Netherlands, Italy, France, Belgium,

Nebulized hypertonic saline to prevent ventilator associated pneumonia in premature infants, a randomized trial.

BACKGROUND/OBJECTIVES: Ventilator associated pneumonia (VAP) is a leading cause of death. Nebulized hypertonic saline solution (HSS) has been used to improve pulmonary clearance and reduce infection in intubated patients. This study examines whether nebulized HSS may reduce VAP in intubated premature infants. METHODS: We analyzed results of 100 intubated premature infants who completed the study. Infants were divided into: (a) the "Control" group, where VAP prevention protocol was implemented and (b) the "Intervention" group, where nebulized HSS twice daily was added to the package of care. [Treatment of bronchiectasias].

Typical symptoms of bronchiectasis include cough, sliminess of the airways and recurrent respiratory infections which lead to lung injury and impaired quality of life when untreated. High-resolution computer tomography is the most important examination to detect bronchiectasias. Cornerstones of the treatment are elucidation of the etiology of the disease, taking care of vaccinations, management of physical fitness, treating sliminess, and careful treatment of periods of exacerbation with antibiotics according to bacterial growth and susceptibility testing determined from sputum. Hypertonic sal

Cardiovascular adjustments induced by hypertonic saline in hemorrhagic rats: Involvement of carotid body chemoreceptors.

The peripheral hyperosmolarity elicited by intravenous infusion of hypertonic saline brings potential benefits to the treatment of hemorrhage. The neural mechanisms involved in these beneficial effects remain unknown. The present study examines the role of carotid chemoreceptors in cardiovascular responses induced by hypertonic saline after hypovolemic hemorrhage in rats. Male Wistar rats (300-400 g) were anesthetized with thiopental, and instrumented for recording of mean arterial pressure. Arterial pressure was reduced to 60 mm Hg by withdrawal of arterial blood over 10 min, and maintained a

Rapid intravenous infusion of 20 ml/kg saline does not impair resting pulmonary gas exchange in the healthy human lung.

Rapid infusion of intravenous saline, a model of pulmonary interstitial edema, alters the distribution of pulmonary perfusion, raises pulmonary capillary blood volume, and increases bronchial wall thickness in humans. We hypothesized that infusion would disrupt pulmonary gas exchange by increasing ventilation/perfusion ((.)VA/(.)Q) inequality as opposed to a diffusive impairment in O2 exchange. Seven males (26 +/- 3 yr; FEV1: 110 +/- 16% predicted.) performed spirometry and had (.)VA/(.)Q mismatch measured using the multiple inert gas elimination technique, before and after 20 ml/kg iv of norm

Aldosteronoma in a dog with polyuria as the leading symptom.

In a 10-year-old castrated male shorthaired German pointer polyuria was associated with slight hypokalemia, hypophosphatemia and alkalosis, as well as elevated plasma concentrations of a glucocorticoid-inducible iso-enzyme of alkaline phosphatase. Repeated measurements of urinary corticoids and normal suppressibility of the hypothalamus-pituitary-adrenocorticial axis excluded glucocorticoid excess. Urine osmolality (Uosm) did not increase during administration of the vasopressin analogue desmopressin. At the time water deprivation had caused Uosm to rise from 300 to 788 mOsm/kg, there was also

[Hypernatremia and kidney function].

Hypernatremia is caused by a water deficit. Cases with hypernatremia and dehydration appear to cluster among children and the elderly with alterations in the level of consciousness thus with no independent access to water. In general, central nervous symptoms prevail. However, thorough examination reveals impaired renal function in many such cases. Animal experiments have shown that rapid increases of the sodium concentration in the renal artery will cause a reduction of renal blood flow (RBF), glomerular filtration rate (GFR) and inhibition of renin secretion, particularly during states of so

[Mechanism of erythrocyte cryohemolysis, induced by cationic amphipaths: synergism of induction of the "discocyte-stomatocyte III" transition due to chlorpromazine and medium tonicity].

It is determined that chlorpromazine, a cationic amphipath usually protecting erythrocytes under conditions of hypertonic cryohemolysis is an efficient inductor of the cell lysis in case of cooling in media with tonicity close to the physiological (the isotonic cryohemolysis). Both chlorpromazine and tonicity of the medium influence the alterations in the state of cells, which is confirmed by synergy of the "discocyte-stomatocyte III" transition induction. The above process may be considered as a critical stage of structural modification of erythrocytes. Transition through this stage coincides

[Failure of spinal anesthesia. Evaluation of the practice at a university hospital].

There is little data concerning failures in spinal anaesthesia. A retrospective analysis of 337 spinal anaesthesias performed in a teaching hospital gave a 9.8% failure rate. A failure was defined as the need to carry out part or all of a surgical act under general anaesthesia when spinal anaesthesia had been carried out. The main causes of failure were insufficiently or excessively extended neural blockade, insufficient duration or a poor quality sensory blockade with the patient feeling surgical or tourniquet pain. The local anaesthetics used were hyperbaric 0.5% tetracaine with and without

Elevated pleural fluid glucose: a risk for tension hydrothorax.

We have presented a case of tension hydrothorax that developed after hyperosmolar hyperalimentation fluids were infused into the pleural space through a misplaced jugular venous line. The hyperosmolar state of the pleural fluid appears to have played a prominent role in the development of the tension hydrothorax. We believe there is a risk of tension hydrothorax in any clinical situation in which the pleural fluid glucose concentration is significantly higher than the serum concentration.

Chronicity of hyponatremia as a factor in experimental myelinolysis.

A recent analysis of hyponatremic patients disclosed that those who were chronically hyponatremic prior to rapid correction subsequently developed central pontine myelinolysis. To test the view that chronicity of hyponatremia is a factor in myelinolysis, we studied a group of rats that were hyponatremic for 1 day and compared it with another group of animals maintained in a hyponatremic state for 3 days prior to the administration of hypertonic saline solution. The 3-day hyponatremic rats developed more numerous and more severe demyelinative lesions than the 1-day rats. The findings support th

Mechanisms of pancreatic hypersecretion following intraduodenal hyperosmolar glucose infusion in rats.

The effects of intraduodenal glucose infusions on pancreatic exocrine secretion were studied in anesthetized and in conscious rats with pancreatic fistula. In the anesthetized model a stimulating effect of glucose on pancreatic flow and bicarbonate output was evidenced. This effect was dose-related and depended in part upon osmolarity of the solution. No nervous mechanism could be demonstrated, while release of secretin in portal blood might be involved in the observed effect. In conscious rats an increase of bicarbonate concentration and output was observed after 30% intraduodenal glucose whi

Amniotic fluid embolism and disseminated intravascular coagulation complicating hypertonic saline-induced abortion.

Amniotic fluid embolism (AFE) associated with disseminated intravascular clotting (DIC) is usually fatal. Such a combination generally occurs at the term of pregnancy or in the immediate postpartum period. A case of AFE with DIC following a hypertonic-saline-induced abortion, is reported. The patient was in the midtrimester with a live fetus. Prompt recognition and treatment of her condition ensured her complete recovery. Such a case has not, to our knowledge, been previously reported.

A method for evaluating analgesic agents in rats.

A method is described for evaluating antinociceptive activity in rats using writhing responses induced by 4% sodium chloride solution. Narcotic and nonnarcotic analgesics, narcotic antagonists except naloxone, and antipyretic and nonsteroidal antiinflammatory drugs were effectively evaluated at relatively low doses using this method. Parenteral ED50 values of these compounds ran parallel to their clinical doses. The method detected not only the analgesic action of the above drugs but also that of methamphetamine. In spite of its high sensitivity, the method was specific in the sense that CNS-d

Critical analysis of hypertonic medium and agitation in detection of bacteremia.

Over 18,000 clinical specimens collected in Vacutainer tubes with sodium polyanethol sulfonate were inoculated into modified Columbia broth (MCB) with and without 10% sucrose. The effects of venting and shaking on recovery were studied. The volume of the blood had a definite effect on the recovery rate. When inoculum size was held constant, recovery of aerobic and facultative organisms was maximal in vented and shaken bottles; the presence of sucrose had no demonstrable effect, recovery of anaerobes was maximal using an unvented bottle incubated under stationary conditions; a significantly gre
