An open study of the effectiveness of Internet treatment for panic disorder delivered in a psychiatric setting.

Panic disorder with or without agoraphobia (PD/A) is common and can be treated effectively with selective serotonin reuptake inhibitor (SSRI) medication or cognitive-behaviour therapy (CBT). However, the lack of access to CBT services has motivated the development of self-help approaches requiring less therapist contact. A novel treatment modality in this field, showing efficacy in several randomized trials but until now not evaluated within the context of regular psychiatric care, is Internet-based treatment. The aim of the present study was to evaluate the effectiveness of Internet-based CBT…

Patient needs and sources of drug information in Singapore: is the Internet replacing former sources?

BACKGROUND: Patients are exposed to a broad range of drug information (DI) sources; among them is the Internet, which has been increasingly used over the years, especially by adolescents. OBJECTIVE: To identify patients' needs and common sources of DI; examine the differences in use of DI sources among groups, by age and sex; and better understand patients' use of the Internet as a source of DI. METHODS: A quota sample of 201 outpatients from National University Hospital in Singapore was surveyed. A questionnaire was developed to examine patients' needs for and common sources of DI as well a…

Is internet-based CBT for panic disorder and agoraphobia as effective as face-to-face CBT?

This study compared Panic Online (PO), an internet-based CBT intervention, to best-practice face-to-face CBT for people with panic disorder with or without agoraphobia. Eighty-six people with a primary diagnosis of panic disorder were recruited from Victoria, Australia. Participants were randomly assigned to either PO (n=46) or best practice face-to-face CBT (n=40). Effects of the internet-based CBT program were found to be comparable to those of face-to-face CBT. Both interventions produced significant reductions in panic disorder and agoraphobia clinician severity ratings, self reported pani…

Vaccines and Internet: characteristics of the vaccine safety net web sites and suggested improvements.

UNLABELLED: The Internet contains a large amount of useful information on many subjects, but also information of doubtful quality. To help identify Web sites on vaccine safety that fulfil good practice, the Global Advisory Committee on Vaccine Safety of the World Health Organization (WHO) has published criteria to which sites should adhere and a listing of Web sites that fulfil them. There are no studies describing the common attributes of these sites. OBJECTIVE: To examine the attributes, design characteristics and resources of Web sites belonging to the Vaccine Safety Net (VSN) of the WHO. …

Internet-based treatment for adults with depressive symptoms: the protocol of a randomized controlled trial.

BACKGROUND: Depression is a highly prevalent condition, affecting more than 15% of the adult population at least once in their lives. Guided self-help is effective in the treatment of depression. The purpose of this study is to investigate the effectiveness of two Internet-based guided self-help treatments with adults reporting elevated depressive symptoms. Other research questions concern the identification of potential mediators and the search for subgroups who respond differently to the interventions. METHODS: This study is a randomized controlled trial with three conditions: two treatment…

Characteristics of smokers reached and recruited to an internet smoking cessation trial: a case of denominators.

The Internet can deliver smoking cessation interventions to large numbers of smokers. Little is known about the feasibility, reach, or efficacy of Internet cessation interventions. Virtually no data exist on who enrolls in cessation programs or on differences between those who complete enrollment and those who do not. This paper reports recruitment and enrollment findings for the first 764 participants in an ongoing randomized controlled trial that tested the efficacy of a widely disseminated Internet smoking cessation service (www.QuitNet.com) alone and in conjunction with telephone counselin…

Internet-guided self-help with or without exposure therapy for phobic and panic disorders.

BACKGROUND: As many sufferers from phobic and panic (phobia/panic) disorders cannot get to suitable therapists, routine aspects of therapy were delegated to internet-accessed computer-aided self-help with or without exposure instructions. METHODS: Phobia/panic referrals were randomised to computer-aided self-help via the internet at home in a 2:1 ratio either by self-exposure cognitive behaviour therapy (CBT) [FearFighter (FF), n = 45] or by minimal CBT without exposure [Managing Anxiety (MA), n = 23]. All had brief backup phone advice from a clinician concerning their computer guidance. RES…

The use of the Internet by gay and bisexual male escorts: sex workers as sex educators.

While prior studies have targeted street-based male sex workers as potential vectors of disease transmission, the number of men who work independently through Internet chat-rooms and other online endeavors has steadily increased. It is likely that these men differ substantially from their street-based counterparts in terms of sexual risk behaviors with their clients. The purpose of this study was to explore the ways in which the Internet has impacted the work of male escorts and their sexual practices with clients. Semi-structured qualitative interviews and quantitative surveys were administer…

Using the internet in patient-centred diabetes care for communication, education, and decision support.

The New England Journal of Medicine 329 (1993) 977- The present paper describes two systems for communication, education, and decision support in patient-centred diabetes care. Both systems are developed under the assumption that while the clinical resources in the health care sector are limited, patients' resources and new information technology may be able to play a much more central role. With DiasNet patients can experiment with their own data. They can, retrospectively, adjust insulin doses or meals sizes, and thereby learn how to cope with various situations. Experiences of Receiving an Internet Cognitive Behaviour Therapy Stepped-Care Model: Young Person Perspectives.

ObjectiveQualitative research examining young people's experiences with stepped-care internet cognitive behaviour therapy (ICBT) for anxiety is limited. Guided by Proctor et al.'s implementation framework, this study explored children and adolescents' experiences of the BRAVE Stepped-Care programme including perceived acceptability and appropriateness of the early, mid-intervention stepped-care model, helpful and challenging aspects of treatment, experiences being stepped-up (or not stepped up) to therapist-support, and, for those stepped-up, perceptions of clinician support and support modali…

Examining the relationship between online self-efficacy and daily Internet use for older adults.

Internet use by community-dwelling older adults can increase their feelings of social connectedness and alleviate feelings of loneliness. Older adults' Internet use is increasing, yet some older adults still lack confidence in their ability to use the Internet. A lack of confidence may negatively affect their Internet use. This study incorporates constructs from technology acceptance models and theories to examine the role of online self-efficacy as a mediating process linking five sources of technical support (i.e., customer support, themselves, family, friends, and online sources), attitudes…

Solution-focused group counseling enhances attention control in Internet addiction: Behavioral and neural evidence.

OBJECTIVE: Internet addiction (IA) is often associated with impaired attention control. Nevertheless, ways to improve this deficit are lacking, and so is the understanding of the mechanisms that such interventions might impact. To address this gap, this study seeks to examine the efficacy of Solution-Focused Group Counselling (SFGC) in enhancing attention networks among individuals with IA. METHODS: A total of 32 college students with high IA scores were recruited, with 26 participants (16 females and 10 males; mean age 19.42 ± 1.10) completing a five-week SFGC intervention. Assessments condu…

Internet-based CBT for young adults with low self-esteem: a randomized, controlled study.

BACKGROUND: Low self-esteem can be disabling for young individuals, negatively impacting school performance, relationships, and mental or physical health. Cognitive-behavioral conceptualizations and treatments for low self-esteem exist but have not been applied or evaluated in a young adult population. TRIAL DESIGN: To address this gap, this study evaluated a novel Internet-based CBT (ICBT) for young adults (18-25 years) suffering from low self-esteem in a randomized controlled trial. METHODS: Eighty-six participants were recruited and randomized to either seven weeks of therapist-guided ICB…

Internet-Delivered Cognitive Behavioral Therapy for Social Anxiety Disorder: Nationwide Retrospective Cohort Observational Study.

BACKGROUND: Internet-delivered cognitive behavioral therapies (iCBTs) can address the accessibility and affordability limitations of conventional therapy. Although iCBTs for social anxiety disorder (SAD) are efficacious, their effectiveness in routine care remains less well established. Helsinki University Hospital (HUS) provides a novel nationwide iCBT program for SAD. OBJECTIVE: We aimed to assess whether this 7-session, flexible time-scheduled, therapist-supported HUS-iCBT for SAD, delivered by a specialized clinic, (1) is effective in routine care and (2) can enhance adherence, compared t…

Internet adoption and health trajectories in older adults during COVID-19: a longitudinal study.

BACKGROUND: Population aging highlights the need to understand whether late-life internet use is associated with cognitive and mental health trajectories. Using the COVID-19 period as a unique social context, we examined whether first-time internet adoption was associated with changes in cognitive function and depressive symptoms among older Chinese adults. METHODS: Using the 2018 and 2020 waves of CHARLS, we first compared three digital-social profiles in 2018: Inactive, Offline-only, and Dual-mode participants (n = 10,935). We then conducted a longitudinal change-score analysis among 3,346 …
