Clinical and hemodynamic results of intraortic balloon counterpulsation and surgery for cardiogenic shock.

Forty-two patients with cardiogenic shock (CS) secondary to myocardial infarction were treated with intra-aortic balloon pumping (I.A.B.P.). In 14 patients C.S. was associated with ventricular septal defect (V.S.D.) and in four with mitral regurgitation (M.R.) secondary to rupture of the posterior papillary muscle. All patients were resistant to conventional medical therapy. Shock was reversed in 20 of the 24 patients in C.S. without mechanical complications. After 24 to 48 hours of I.A.B.P., cardiax index (C.I.) increased from 1.38 to 2.00 L./min./M2, systolic arterial pressure (S.A.P.) from …

Total Left Main Occlusion ST-Elevation Myocardial Infarction Complicated by Cardiogenic Shock and Delayed Sinus Node Dysfunction.

BACKGROUND Acute ST-segment elevation myocardial infarction (STEMI) due to total occlusion of an unprotected left main coronary artery (LMCA) is rare and often fatal, particularly when complicated by cardiogenic shock. CASE REPORT We report the case of a 58-year-old man who presented with sudden, severe chest pain due to an extensive anterolateral STEMI. His condition rapidly deteriorated, with the development of severe hypoxemia, flash pulmonary edema, and cardiogenic shock. Endotracheal intubation was performed, followed by urgent primary percutaneous coronary intervention and initiation of

Managing Cardiogenic Shock in the Era of Multimodal Mechanical Support: Bridging to Transplant or Recovery.

PURPOSE OF REVIEW: To review the contemporary management of cardiogenic shock in the era of multimodal temporary mechanical circulatory support, with emphasis on phenotype-guided device selection, serial trajectory assessment, and bridging toward recovery, durable left ventricular assist device implantation, or heart transplantation. RECENT FINDINGS: Temporary mechanical circulatory support is now used to influence clinical outcomes, not just as rescue therapy. Devices like intra-aortic balloon pumps, microaxial flow pumps, and venoarterial extracorporeal membrane oxygenation affect heart fun

Mechanical circulatory support after cardiac arrest.

PURPOSE OF REVIEW: Mechanical circulatory support (MCS) is increasingly used in cardiogenic shock, yet evidence for its benefit in postcardiac arrest patients remains limited and controversial. This review discusses recent randomized trials and evolving concepts in hemodynamic phenotyping and patient selection. RECENT FINDINGS: MCS devices - such as intra-aortic balloon pump (IABP), microaxial flow pump (mAFP), and veno-arterial extracorporeal membrane oxygenation (VA-ECMO) - distinct indications, risks, and limitations. Although mAFP demonstrated improved survival in infarct-related cardioge

Impact of Left Ventricular Venting on Acute Brain Injury in Patients With Cardiogenic Shock: An Extracorporeal Life Support Organization Registry Analysis.

OBJECTIVES: While left ventricular (LV) venting reduces LV distension in cardiogenic shock patients on venoarterial extracorporeal membrane oxygenation (ECMO), it may also amplify risk of acute brain injury (ABI). We investigated the hypothesis that LV venting is associated with increased risk of ABI. We also compared ABI risk of the two most common LV venting strategies, percutaneous microaxial flow pump (mAFP) and intra-aortic balloon pump (IABP). DESIGN: Retrospective observational cohort study. SETTING: The Extracorporeal Life Support Organization registry. PATIENTS: Adult patients on p

The Effect of Different Types of Mechanical Circulatory Support on Mortality of Patients after Adult Cardiac Surgery: A Systematic Review and Meta-Analysis.

OBJECTIVES: Sample size may limit the ability of individual studies to detect differences in clinical outcomes between extracorporeal membrane oxygenation (ECMO) alone and ECMO plus intra-aortic balloon pump (IABP) after adult cardiac surgery. Therefore, we undertook a meta-analysis of the best evidence available on the comparison of clinical outcomes of ECMO alone and ECMO plus IABP after adult cardiac surgery. METHODS: PubMed, EMBASE, Web of Science, and Cochrane Center Registry of Controlled Trials were searched for studies comparing the use of ECMO alone and ECMO plus IABP after adult car

Incidence and clinical outcomes of stroke in ST-elevation myocardial infarction and cardiogenic shock.

OBJECTIVE: The authors sought to evaluate 10-year national trends, incidence and clinical outcomes of stroke in CS-STEMI. BACKGROUND: Stroke is a devastating complication among patients with ST-elevation myocardial infarction (STEMI). Concomitant cardiogenic shock (CS) may further increase the risk of stroke. Use of percutaneous mechanical circulatory support (pMCS) devices may further increase stroke risk in CS-STEMI. No studies have evaluated the risk of stroke in contemporary CS-STEMI. METHODS: We performed a retrospective cohort study of CS-STEMI patients from a large U.S. national datab

Ventricular septal rupture with right ventricular wall dissection after inferior ventricular infarction: A case report and literature review.

Right ventricular (RV) wall dissection following ventricular septal rupture related to inferior myocardial infarction (MI) is an extremely rare complication with a high mortality rate. We report the case of a 61-year-old man who was admitted to our hospital because of syncope and intermittent chest pain with a precordial murmur. Transthoracic echocardiography showed a rupture at the basal infero-posterior septum and RV free-wall dissection forming an echolucent cavity that extended beyond the septum and subsequently re-entered into RV chamber. The patient's overall cardiac and renal functions

[Modified David-Komeda Repair for Ventricular Septal Perforation Complicated with Severe Postoperative Acute Respiratory Distress Syndrome;Report of a Case].

A 61-year-old man presented by ambulance with dyspnea. He was diagnosed with myocardial infarction complicated with ventricular septal perforation (VSP), and intraaortic balloon pumping support and intensive care were started. Because of instability of hemodynamic status, modified David-Komeda operation with double patch was performed in the subacute phase of VSP. Although he developed acute respiratory distress syndrome( ARDS) on the 21st day after operation, he was successfully treated with corticosteroid pulse therapy and artificial ventilation. He was transferred to a rehabilitation hospit

[Emergent coronary artery bypass grafting in a survivor of out-of-hospital cardiac arrest].

We report a case of emergent coronary artery bypass grafting (CABG) in a survivor of an out-of-hospital cardiac arrest. A 64-year-old male driver lost consciousness and collapsed in a rice paddy field. A bystander placed him in a car and immediately started cardiopulmonary resuscitation after confirming the presence of pulselessness and apnea. Emergency medical service providers performed a defibrillation of ventricular fibrillation by using an automated external defibrillator (AED), and the patient was transferred to the critical care center in our hospital. Coronary angiography revealed a th

Effect of preoperative hemodynamic support on survival after cardiac transplantation.

The accessibility and success of cardiac transplantation promotes transplantation for a broad range of recipients, including those requiring intravenous inotropes or mechanical-assist devices. To determine if survival is dependent on preoperative requirements for hemodynamic support, we studied 230 patients who underwent transplant at the Loyola, Stanford, and UTAH programs from December 1, 1984 through November 30, 1986, and who were followed up for 34 months postoperatively. Group 1 (n = 132 of 230, 57%) patients required only oral medical therapy to maintain hemodynamic compensation; Group

Vascular trauma secondary to diagnostic and therapeutic procedures: cardiopulmonary bypass and intraaortic balloon assist.

Significant arterial trauma can result from femoral arterial cannulation for cardiopulmonary bypass or intraaortic balloon pumping. Threat of imminent loss of limb or suture line disruption requires prompt surgical intervention. Delayed appearance of claudication, characteristically at 1 to 2 weeks postoperatively, is highly suggestive of iatrogenic iliofemoral injury. Arterial reconstructive surgery was necessary in three of five such patients.

[Rupture of the papillary muscles of the mitrale valve during myocardial infarction].

Between 1967 and 1976, 13 cases of papillary muscle rupture during the acute phase of myocardial infraction were collected. Six patients have been treated surgically during the past three years. Three could undergo surgery by virtue of the use of an intra-aortic counterpulsion balloon. There were no operative deaths. One patient died ten days after surgery as a result of a neurological complication; while a second died 4 months after a successful operation of an undetermined cause. For very satisfactory results were obtained with a follow-up of 7, 16, 18 and 18 months.

Steady Flow Left Ventricle Unloading Is Superior to Pulsatile Pressure Augmentation Venting During Venoarterial Extracorporeal Membrane Oxygenation Support.

Venoarterial extracorporeal membrane oxygenation (VA-ECMO) shunts venous blood to the systemic arterial circulation to provide end-organ perfusion while increasing afterload that may impede left ventricle (LV) ejection and impair cardiac recovery. To maintain flow across the aortic valve and reduce risk of lethal clot formation, secondary mechanical circulatory support (MCS) devices are increasingly used despite limited understanding of their effects on cardiac function. This study sought to quantify the effects of VA-ECMO and combined with either intraaortic balloon pump (IABP) or percutaneou

Ten-year trends, predictors and outcomes of mechanical circulatory support in percutaneous coronary intervention for acute myocardial infarction with cardiogenic shock.

AIMS: There are limited data on the trends and outcomes of mechanical circulatory support (MCS)-assisted early percutaneous coronary intervention (PCI) in acute myocardial infarction with cardiogenic shock (AMI-CS). In this study, we sought to assess the use, temporal trends, and outcomes of percutaneous MCS-assisted early PCI in AMI-CS. METHODS AND RESULTS: Using the National Inpatient Sample database from 2005-2014, a retrospective cohort of AMI-CS admissions receiving early PCI (hospital day zero) was identified. MCS use was defined as intra-aortic balloon pump (IABP), percutaneous left ve

The novel cystatin C, lactate, interleukin-6, and N-terminal pro-B-type natriuretic peptide (CLIP)-based mortality risk score in cardiogenic shock after acute myocardial infarction.

BACKGROUND: Cardiogenic shock (CS) complicating acute myocardial infarction (AMI) still reaches excessively high mortality rates. This analysis is aimed to develop a new easily applicable biomarker-based risk score. METHODS AND RESULTS: A biomarker-based risk score for 30-day mortality was developed from 458 patients with CS complicating AMI included in the randomized CULPRIT-SHOCK trial. The selection of relevant predictors and the coefficient estimation for the prognostic model were performed by a penalized multivariate logistic regression analysis. Validation was performed internally, inte
