PURPOSE: Although classically classified as a non-inflammatory condition, an inflammatory basis for keratoconus (KC) appears to be a growing evidence. Recently, it has been shown that KC patients have an increased choroidal thickness (CT). Among inflammatory disorders, atopy has been associated with KC development; therefore, the aim of this study was to evaluate if the increased CT in patients with KC is related to atopy.
METHODS: This is an analytical cross-sectional study of patients with KC. Patients were classified as atopic and non-atopic according to their atopy history (allergic rhino…

PURPOSE: To characterize very asymmetric keratoconus (KC) in terms of clinical demographics and risk factors in the Japanese population.
METHODS: We retrospectively reviewed the clinical records of patients with very asymmetric KC attending a university hospital. Patients with very asymmetric KC had defined clinical KC in one eye and normal topography in the fellow eye. All patients completed a questionnaire on potential risk factors (family history of KC, atopy, asthma, allergy, heart disease, sleep apnea, Down syndrome, eye rubbing, prone sleep position, and dominant hand). These data were …

The objective of this report is to describe a case of meibomian gland dysfunction associated with keratoconus and to examine the importance of treatment for evaporative dry eye in cases of corneal ectasia. A 45-year-old man diagnosed as having keratoconus complained of burning, tearing, itching, and red eye. He had a history of penetrating corneal transplantation and wearing rigid contact lenses. The meibography revealed a severe meibomian gland dropout and normal tear meniscus height in both eyes. Objective propaedeutic tests are important tools for dry eye diagnosis and proper evaluation of …

The purpose was to review and document the methods used to calculate the power of the intraocular lens (IOL) to be implanted in cataract surgery in the specific scenario of eyes with keratoconus. This review included all scientific articles published in English that focused on the parameters and formulas used to calculate the power of the IOL to be implanted in eyes with keratoconus undergoing cataract surgery. There are few publications that show in detail how IOL power is calculated in these particular cases. If the keratometric value used was based on the standard refractive index (1.3375),…

UNLABELLED: Keratoconus (KC) is typically a bilateral asymmetric corneal condition associated with anisometropia. A detailed evaluation of binocular vision function is therefore a critical component of ocular examination in these patients for early detection and treatment of binocular vision anomalies.
PURPOSE: The purposes of this study were to measure the various parameters of binocular vision (BV) in patients with KC and to identify the factors that possibly influence them.
METHODS: This was a prospective, observational study of BV in patients with KC and with best-corrected visual acuity…

OBJECTIVES: Early identification of keratoconus is imperative for preventing iatrogenic corneal ectasia and allowing for early corneal collagen cross-linking treatments to potentially halt progression and decrease transplant burden. However, early diagnosis of keratoconus is currently a diagnostic challenge as there is no uniform screening criteria. We performed a review of the current literature to assess imaging modalities that can be used to help identify subclinical keratoconus.
METHODS: A Pubmed database search was conducted. We included primary and empirical studies for evaluating diffe…

PURPOSE: Keratoconus is considered a non-inflammatory condition. Recently however, increased proinflammatory cytokines have been detected in the tears of keratoconic patients and clinical and immunohistochemical observations reported infiltration of matured dendritic cells and leukocytes. Our laboratory utilized cytokine antibody arrays to elucidate the inflammatory aspects of keratoconus.
METHODS: Protein was extracted from 42 corneal buttons (14 keratoconic and 28 non-keratoconic) and incubated with cytokine antibody arrays scanning 120 cytokines. Mann Whitney U test with a p-value of <0.05…

The article reviews data on the so called non-surgical methods of keratoconus (KC) correction. The combined approach to KC treatment entails three main tasks: prevention of progression, optical correction of induced refractive disorders, and structural repair. Measures taken to accomplish the first two tasks should be regarded as symptomatic treatment, while addressing the last task requires pathogenetically oriented solutions. Corneal cross-linking and optical correction of refractive disorders with rigid gas permeable (RGP) scleral contact lenses should be regarded as the first choice for sy…

PURPOSE OF REVIEW: To summarize the recent advances in transcriptomics and proteomics studies of keratoconus using advanced genome-wide gene and protein expression profiling techniques.
RECENT FINDINGS: Second-generation sequencing including RNA sequencing has been widely used to characterize the genome-wide gene expression in corneal tissues or cells affected by keratoconus. Due to different sample types, sequencing platforms, and analysis pipeline, different lists of genes have been identified to be differentially expressed in KC-affected samples. Gene ontology and pathway/network analyses …
