Congenital dislocation of the patella is a rare condition characterized by lateral dislocation of the patella that is irreducible without surgical correction. Although there is no clear inheritance pattern, it is associated with several congenital syndromes. Patients often demonstrate flexion contracture, loss of active knee extension, increased tibial external rotation, and absent patella in the trochlea. In 26 knees treated by semitendinosus tenodesis, there was no recurrence of dislocation, but fair and poor results totaled 38% due to persistent pain because of chondromalacia of the patella. Poor results are also more common in the patients with generalized ligamentous laxity. Postoperative complications in wound healing suggest that the medial parapatellar skin incision is less than adequate. Semitendinosus tenodesis with or without modifications is a useful procedure to prevent recurrent dislocations and subluxation during the growth period.

We analysed the postoperative position of the patella and the degenerative changes in 80 implanted Guepar, Tillmann and GSB-prostheses of the knee. We did not found differences between the types of endoprostheses. We saw many dislocations of the patella, especially in patients with rheumatism, but only some patients had trouble with the operated knee. We can diminish these problems by modelling of the patella and by careful reconstruction of the muscle strength.

OBJECTIVE: To evaluate clinical effect of micro-modified Fulkerson osteotomy under arthroscope in treating habituation patellar dislocation.
METHODS: Twenty patients of habituation patellar dislocation (25 knees) were treated by the medial retinaculum plication, lateral retinaculum releasing and modified Fulkerson osteotomy (tibial tubercle anteromedial transfer) under arthroscope. There were 5 males and 15 females with the age from 16- to 28-years-old (average of 21 years). The subjective symptoms and the joint function were evaluated according to Lysholm and Tegner scoring system.

Twenty-five dislocations occurred after 561 total hip arthroplasties performed by the author from 1971 to 1992. Acetabular shell and liner loosening must be differentiated from true dislocations.
