Self-Managed Strength Training for Active Duty Military With a Knee Injury: A Randomized Controlled Pilot Trial.

INTRODUCTION: Knee injuries among active duty military are one of the most frequent musculoskeletal injuries and are often caused by exercise or intense physical activity or combat training. These injuries pose a threat to force readiness. Our objective was to assess feasibility (including recruitment and retention rates) of three self-managed strengthening strategies for knee injuries and determine if they resulted in improvements in lower extremity strength, function, pain, and activity compared to usual physical therapy (PT) in military members. METHODS: A pilot study using a randomized co…

An anatomical examination of iatrogenic nerve injury during inside out meniscus repair with flexion and extension of the knee.

BACKGROUND: In this study, we aim to assess the safe, risky and high-risky zones by measuring the proximity of the needles to the peroneal and saphenous nerves in millimeters for the repair of tears of the anterior, middle and posterior horns of the medial and lateral menisci at flexion and extension position during inside-out repair technique. METHODS: First, a cadaveric study was conducted on 10 cadaver knees in which both (lateral and medial) menisci were divided into anterior, corpus and posterior with the longitudinal tear simulating in each section. The next phase involved the suture of

Active muscle response contributes to increased injury risk of lower extremity in occupant-knee airbag interaction.

OBJECTIVE: Recent field data analysis has demonstrated that knee airbags (KABs) can reduce occupant femur and pelvis injuries but may be insufficient to decrease leg injuries in motor vehicle crashes. An enhanced understanding of the associated injury mechanisms requires accurate assessment of physiological-based occupant parameters, some of which are difficult or impossible to obtain from experiments. This study sought to explore how active muscle response can influence the injury risk of lower extremities during KAB deployment using computational biomechanical analysis. METHODS: A full-fact

Hydrogel implant is as effective as osteochondral autologous transplantation for treating focal cartilage knee injury in 24 months.

PURPOSE: The treatment approach for a patient with knee joint focal cartilage lesion is a difficult decision. To date, there has been no randomized clinical trial involving Hydrogel (Cartiva™). This study evaluated and compared the results of a hydrogel implant (Cartiva™) with autologous osteochondral transplantation (AOT) for treating knee joint focal cartilage lesions. METHODS: Thirty-eight symptomatic patients, with a focal cartilage lesion of Outerbridge grades III or IV, were randomized into one of two groups according to the inclusion and exclusion criteria. Group I underwent AOT, and G

Impact of treatment strategy and physical performance on future knee-related self-efficacy in individuals with ACL injury.

BACKGROUND: In people with anterior cruciate ligament (ACL) injury, high self-efficacy facilitates recovery, indicated by improved muscle function, reduced knee symptoms and increased physical activity. Impact of treatment on future self-efficacy is however not well investigated. The aims of the study were to 1) investigate knee-related self-efficacy 6 years after acute ACL injury in patients treated with exercise therapy alone or in combination with either early or the option of delayed ACL reconstruction (ACLR), and 2) to investigate associations between single-leg physical performance at va

Impact of time to surgery from injury on postoperative infection and deep vein thrombosis in periprosthetic knee fractures.

PURPOSE: Periprosthetic fracture (PPF) is a serious complication that occurs in 0.3%-2.5% of all total knee arthroplasties used to treat end-stage arthritis. To our knowledge, there are no studies in the literature that evaluate the association between time to surgery after PPF and early postoperative infections or deep vein thrombosis (DVT). This study tests our hypothesis that delayed time to surgery increases rates of postoperative infection and DVT after PPF surgery. METHODS: Our study cohort included patients undergoing PPF surgery in the American College of Surgeons National Surgical Qu

Empirical Based Modeling for the Assessment of Dynamic Knee Stability: Implications for Anterior Cruciate Ligament Injury Risk.

Anterior cruciate ligament (ACL) injuries are common sports injuries, costing the U.S. roughly $1 billion annually. To better understand the underlying injury mechanism, Nyquist and Bode stability criteria were applied to assess frontal plane dynamic knee stability among male Australian Football players during the weight-acceptance phase of single-leg jump landing. Out of 30 landings, 19 were classified as stable and 11 as unstable. Medial and lateral vasti, hamstring and gastrocnemii muscle activation waveforms were analyzed in parallel to determine if individuals with stable and unstable fro

Differential diagnosis of knee pain following a surgically induced lumbosacral plexus stretch injury. A case report.

Background and Purpose: Knee joint biomechanics requires an understanding of lower extremity (LE) segmental interactions. In some cases, knee pain may arise as a result of altered LE biomechanics; while in other cases, knee pain may stem from other causes, such as a peripheral nerve injury. Case Description: A 33-year-old woman presented via direct access for physical therapist (PT) examination with a chief complaint of left knee pain. The day after undergoing a dilation and curettage (D&C) procedure the patient had an acute onset of gait dysfunction. Over the next few days, the patient develo

Change in knee flexor torque after fatiguing exercise identifies previous hamstring injury in football players.

Muscular fatigue and interlimb strength asymmetry are factors known to influence hamstring injury risk; however, limb-specific exacerbation of knee flexor (hamstrings) torque production after fatiguing exercise has previously been ignored. To investigate changes in muscular force production before and after sport-specific (repeated-sprint) and non-specific (knee extension-flexion) fatiguing exercise, and explore the sensitivity and specificity of isokinetic endurance (ie, muscle-specific) and single-leg vertical jump (ie, whole limb) tests to identify previous hamstring injury. A novel rat model for subchondral microdamage in acute knee injury: a potential mechanism in post-traumatic osteoarthritis.

OBJECTIVE: Subchondral microdamage may play an important role in post-traumatic osteoarthritis (PTOA) development following anterior cruciate ligament (ACL) rupture. It remains unknown whether this injury mechanism causes subchondral microdamage, or whether its repair occurs by targeted osteoclast-mediated remodeling. If so these events may represent a mechanism by which subchondral bone is involved in PTOA. Our objective was to test the hypothesis that subchondral microdamage occurs, and is co-localized with remodeling, in a novel rat model of ACL rupture. DESIGN: We developed a novel non-in

ASSOCIATION OF ISOMETRIC STRENGTH OF HIP AND KNEE MUSCLES WITH INJURY RISK IN HIGH SCHOOL CROSS COUNTRY RUNNERS.

BACKGROUND: High school cross country runners have a high incidence of overuse injuries, particularly to the knee and shin. As lower extremity strength is modifiable, identification of strength attributes that contribute to anterior knee pain (AKP) and shin injuries may influence prevention and management of these injuries. PURPOSE: To determine if a relationship existed between isometric hip abductor, knee extensor and flexor strength and the incidence of AKP and shin injury in high school cross country runners. MATERIALS/METHODS: Sixty-eight high school cross country runners (47 girls, 21

Below-the-knee arterial injury: the type of vessel may be more important than the number of vessels injured.

BACKGROUND: There are few data regarding limb salvage following below-the-knee (BK) arterial trauma. Based on published data and clinical experience, we hypothesized that any single patent vessel BK would allow for limb viability and salvage. METHODS: The trauma center registry was retrospectively queried, from 2007 through 2012, for patients presenting with BK arterial injuries (BKAIs), defined as injury below the popliteal artery. Logistic regression, Pearson's χ, and Student's t test were used to analyze data. RESULTS: A total of 122 patients were identified. The mean age was 35 years, 84

Lower limb salvage surgery using Ilizarov circular external frame for a landmine injury about the knee.

Limb salvage for severe trauma has been replaced amputation as the primary treatment in many trauma centers. However, the long-term outcomes after limb reconstruction or amputation have not been fully evaluated. In this report, we present the treatment results of limb salvage surgery using Ilizarov external circular frame in a male case who had a-22-cm bone loss on the left distal femur and left proximal tibia and large soft tissue defect around the knee due to stepping on a landmine with his knee. The decision to amputate a severely injured limb, being irreversible, is challenging and signifi

The effects of core muscle activation on dynamic trunk position and knee abduction moments: implications for ACL injury.

Anterior cruciate ligament (ACL) injury is one of the most common serious lower-extremity injuries experienced by athletes participating in field and court sports and often occurs during a sudden change in direction or pivot. Both lateral trunk positioning during cutting and peak external knee abduction moments have been associated with ACL injury risk, though it is not known how core muscle activation influences these variables. In this study, the association between core muscle pre-activation and trunk position as well as the association between core muscle pre-activation and peak knee abduc

Osteogenic potential of cells in vitro derived from haemarthrosis of the knee induced by injury to the anterior cruciate ligament.

We have investigated whether cells derived from haemarthrosis caused by injury to the anterior cruciate ligament could differentiate into the osteoblast lineage in vitro. Haemarthroses associated with anterior cruciate ligament injuries were aspirated and cultured. After treatment with beta-glycerophosphate, ascorbic acid and dexamethasone or 1,25 (OH)(2)D(3), a significant increase in the activity of alkaline phosphatase was observed. Matrix mineralisation was demonstrated after 28 days and mRNA levels in osteoblast-related genes were enhanced. Our results suggest that the haemarthrosis induc

Anatomic risk of peroneal nerve injury with the "pie crust" technique for valgus release in total knee arthroplasty.

Peroneal nerve damage can occur during total knee arthroplasty because of indirect or direct injury. The potential for direct laceration injury exists when performing the "pie crust" lateral soft-tissue release in a valgus knee. To assess this risk, the axial, magnetic resonance images of 60 adult knees were evaluated. The distance from the peroneal nerve to the tibia was measured at the level of the standard tibial resection. At this level, the lateral gastrocnemius muscle is interposed between the capsule and the nerve. The mean nerve to bone distance was 1.49 cm (0.91-2.18 cm). These result

Contraction of knee flexors and extensors in skiing related to the backward fall mechanism of injury to the anterior cruciate ligament.

The purpose of this study was to examine the motion pattern and muscle activity during jumping and landing in downhill skiing to estimate the muscular involvement during the jump-landing-backward fall-anterior cruciate ligament (ACL) injury. A digitized 2-D video motion analysis and a synchronous 8-channel electromyography registration was performed over a jump on a downhill slope during 3 runs by 2 skiers and during 6 control jumps in the laboratory. The knee flexors (lateral gastrocnemius and biceps femoris) were recruited before touchdown and mean 60 ms earlier than the extensors (vastus la

A case of popliteal artery injury associated with multiligamentous knee injury successfully managed with endovascular stenting after thrombotic occlusion following vein graft revascularization.

Autologous vein grafting is a common method for revascularization in cases of traumatic limb arterial injuries. However, no clear guidelines have been established regarding the use of tourniquets following vascular reconstruction. We report a case of graft thrombosis that occurred during a planned orthopedic surgery one week after the initial revascularization. The patient was a 56-year-old man with a complex ligament injury of the left knee accompanied by a popliteal artery injury. After admission to our hospital, he underwent emergency popliteal artery reconstruction using an autologous vein

[Loss of strength and regeneration of knee extensor musculature after operations of the knee ligaments. EMG studies of the effect of the injury pattern, surgical procedure and after-care with special reference to electromyostimulation].

Clinically relevant losses of the capacity of the extensor muscles of the thigh to contract voluntarily are usual after open knee joint operations and are known to lead frequently to a major delay in healing. In the present paper, a randomized prospective comparative study is presented in which the negative effect of the injury and surgical trauma on the extensor musculature of the thigh is compared between open and arthroscopically controlled cruciate ligament operations. The surface electromyogram is measured. The values found make it evident that an equally pronounced loss of strength amoun
