[Stapedo-vestibular ankylosis associated with osteopathy of the promontory (author's transl)].

The authors present two unusual operative findings during surgery for stapedo-vestibular ankylosis, of which no similar cases would appear to be found in the literature. The first case posed the problem of stapedo-vestibular ankylosis as a sequel of clinically latent otitis, the only manifestations of which were radiological opacity of the mastoid and an area of osteitis of the promontory, still active, in the absence of any other progressive inflammatory phenomena or sequelae. Surgery required curettage and opening of the promontory, associated with total stapedectomy and a Teflon-inter-posit

Immediate restoration of basal sensorineural hearing (Mb Meniere) using a pressure chamber.

A new technique for the restoration of basal sensorineural hearing loss in Mb Meniere was described. Three cases with unilateral basal sensorineural hearing loss, fullness of the ear, and tinnitus were reported. In the acute stage of their disease the patients were treated in a pressure chamber where it was possible to increase or decrease the air pressure within the range +/- 110 cm H2O. The equilibration of middle ear pressures to surrounding air pressures was checked. Exposure to underpressure resulted in a rise of the hearing thresholds at low frequencies and relief of subjective symptoms.

[The peculiar features of vestibular function disorders associated with Meniere's disease].

AIM: The objective of the present study was to reveal the special aspects of vestibular dysfunction in the patients presenting with unilateral Meniere's disease (MD) during the attack-free intervals. METHODS: The study included 23 patients suffering from unilateral MD; all of them were examined with the use of tonal threshold audiometry, videonystagmography, the caloric test and video head-impulse test (vHIT). RESULTS: The study has demonstrated that the patients with MD are characterized, during the attack-free intervals, by the absence of latent spontaneous nystagmus in conjunction with th

The subjective visual vertical as a clinical parameter of vestibular function in peripheral vestibular diseases.

The subjective visual vertical, SV, was measured in the upright and side positions in 25 normal subjects and in 73 patients with various peripheral vestibular disorders. Significant deviations of SV (toward the affected ear) were found in 100% of the patients with vestibular nerve section and with Ramsay Hunt syndrome, in 89% of the patients with vestibular neuritis, and in 0% of the patients with benign paroxysmal positional vertigo. The deviation of SV gradually disappeared within a few weeks of the onset of the disease in all patients except in those with total VIIth nerve resection. SV is

The site of the lesion causing hearing loss in bacterial meningitis: a study of experimental streptococcal meningitis in guinea-pigs.

The pathological basis of hearing loss in bacterial meningitis was investigated using an animal model of Streptococcus suis meningitis. Forty guinea-pigs were infected after their hearing had been assessed by brain stem auditory evoked potentials. In 17 animals, it was possible to repeat the procedure at the onset of meningitis; this included one animal with subclinical disease. Fifteen animals showed evidence of hearing loss, which on subsequent histological examination was found to be associated invariably with suppurative labyrinthitis. The remaining two animals without hearing loss had nor

Promontory stimulation following labyrinthectomy: implications for cochlear implantation.

Transtympanic electric promontory stimulation is a psychoacoustic test used to assess residual acoustic neurons in profound sensorineural hearing loss. The test was performed in six patients who had previously undergone transmastoid (N = 5) or transcanal-oval window (N = 1) labyrinthectomy as a means of determining the feasibility of future cochlear implantation. Four patients had unilateral Meniere's disease, one had labyrinthitis, and one had delayed onset vertigo. All patients perceived a definite auditory sensation in the labyrinthectomized ear during stimulation. The results of threshold,

Induction of a rat T cell lymphoma-leukemia by magnesium deficiency--a study of fetal defense against maternal neoplasia.

If Sprague-Dawley rats, 25-30 days old, are fed a diet containing 4-5 mg% of Mg, about 25% of survivors develop a large tumor of the thymus within 6-12 weeks. The tumor is composed of lymphoblasts, which seem to arise from the thymic reticuloendothelial system and, at times, disseminate as an acute T cell lymphoma-leukemia of unknown etiology. If the tumor cells are transmitted intraperitoneally to rats, 14-16 days pregnant, a local invasive and generalized disease is established in the mother but not in the fetuses or their domain. However, if the neoplastic cells are injected into the fetal

Intrabeat relationship of postrotatory nystagmus in patients with neurological disorders.

The intrabeat relationships between velocity, amplitude and duration of the slow phase of nystagmus as well as between amplitudes of slow vs. fast phase of nystagmus were analysed during a postrotatory nystagmus response by using linear regression analysis. Three groups of patients were studied, each consisting of 10 subjects with lesions either in the peripheral vestibular system, in the frontal lobe, or in the brain stem. Irrespective of the site of the lesion, all groups exhibited the same response pattern: a reduction in amplitude control of the end-point of nystagmus and an increase in du

Treatable sensorineural hearing loss.

Sensorineural hearing loss is generally felt to be an untreatable medical condition. However, in some cases, prompt diagnosis and treatment of the underlying condition may reverse the deafness. This article summarizes various treatable forms of sensorineural hearing loss and provides illustrative cases histories of patients who have had sensorineural hearing losses that were improved by medical or surgical intervention. Patients with reversible sensorineural deafness due to inadvertent aminoglycoside over-dosage, congenital cholesteatoma, Meniere's syndrome, blood coagulopathy, and perilymphat

The Correlation of a 2D Volume-Referencing Endolymphatic-Hydrops Grading System With Extra-Tympanic Electrocochleography in Patients With Definite Ménière's Disease.

Background: Although magnetic resonance imaging (MRI) of the membranous labyrinth and electrocochleography (ECochG) have been used to diagnose endolymphatic hydrops (ELH) in patients with Ménière's disease (MD), the relationship between imaging and ECochG is not well-documented. Objectives: This study evaluates the ELH using 3D-FLAIR MRI and extra-tympanic ECochG (ET-ECochG) and correlates the results from 3D-FLAIR MRI to those from ET-ECochG. Materials and Methods: 3D-FLAIR MRI images of 50 patients were assessed using a 2D volume-referencing grading system (VR scores, relative scores accordi

Semi-quantitative vs. volumetric determination of endolymphatic space in Menière's disease using endolymphatic hydrops 3T-HR-MRI after intravenous gadolinium injection.

Magnetic resonance imaging enhances the clinical diagnosis of Menière's disease. This is accomplished by in vivo detection of endolymphatic hydrops, which are graded using different semi-quantitative grading systems. We evaluated an established, semi-quantitative endolymphatic hydrops score and with a quantitative method for volumetric assessment of the endolymphatic size. 11 patients with Menière's disease and 2 healthy subjects underwent high resolution endolymphatic hydrops 3 Tesla MRI with highly T2 weighted FLAIR and T2DRIVE sequences. The degree of endolymphatic hydrops was rated semi-qu

Patulous internal auditory canal.

OBJECTIVE: To investigate a rare anomaly of the internal auditory canal known as a patulous canal and its relationship to hearing impairment. METHODS: High-resolution computed tomographic scans of the temporal bones of patients who presented between August 2001 and August 2002 were reviewed. The patients' medical charts were evaluated for age, sex, and hearing impairment, and the computed tomographic scans were examined for the presence of a patulous canal. RESULTS: The study group included 645 patients who underwent high-resolution computed tomography of the temporal bones for various reaso

[Significance of comparing the results of caloric and vestibulometric rotation tests].

The use of modified, less laborious, easier endured vestibulometric tests made it possible to facilitate diagnosing Menière's disease and neurinomas of the vestibular nerve. Comparison of nystagmometric findings recorded in the same patient during caloric and rotation tests using the diagnostic model of a bithermal test permitted some assumptions to be made in relation to the mechanisms for nystagmogenesis associated with pathologic states in ear labyrinth and vestibular portion craniocerebral (VIII) nerve. It is found that in the majority of Meniere's disease cases (61%) there is a decrease i

Melanocyte system of the endolymphatic duct and sac.

One hundred fourteen human temporal bones from 64 individuals were studied for the presence of melanized melanocytes about the endolymphatic duct and sac. Specimens were from Caucasians (104), Orientals (6) and Negroes (4). Of the specimens from people above nine years of age 79% demonstrated melanized melanocytes about the endolymphatic duct and sac, as well as occasionally about blood vessels in the adjacent bone. These represented 74% of the Caucasian and 75% of the Negro and Oriental individuals. Pigment granules were noted in occasional epithelial lining cells and in macrophage-type cells

Bilateral vestibulopathy with dissociated deficits in the superior and inferior vestibular systems.

OBJECTIVES: The aim of this study was to reveal the clinical characteristics of dissociated bilateral vestibulopathy (BV) affecting the superior vestibular nerve system on one side and the inferior vestibular nerve system on the other side. It is probable that BV does not always show bilateral deficits of the same vestibular nerve system. METHODS: In a retrospective study of 2,752 consecutive new patients, 1,560 patients had undergone both caloric testing and cervical vestibular evoked myogenic potential testing. All 1,560 patients had also undergone a battery of tests including standardized

Histologic and statistical studies on hyalin bodies in the human endolymphatic sac.

Hyalin bodies are amorphous, eosinophilic masses that protrude from the subepithelial connective tissue into the lumen of the endolymphatic sac. In this study, hyalin bodies were analyzed in two groups of temporal bones: normal bones and bones with cochlear otosclerosis. The results revealed that bones with cochlear otosclerosis had significantly more and larger hyalin bodies that did normal bones. In addition, the hyalin bodies in cochlear otosclerosis were denser and associated with more edema and loose connective tissue in the surrounding areas. Foamy macrophages, concentric calcific struct

Vertigo after head injury--a five year follow-up.

Three hundred and twenty-one head injury patients investigated at the Workmen's Compensation Board Hospital and Rehabilitation Centre were studied. The patients were classified into two groups, minor and moderate according to the duration of post-traumatic amnesia. Post-traumatic vertigo was a significant symptom in 34 per cent and 50 per cent of the minor and moderate groups respectively. Based on the findings of full otoneurologic and vestibular examination, objective vestibular disorder was noted in 40 per cent and 65 per cent of the two vertiginous groups respectively. An approach to the i
