Four-strand hamstring tendon autograft versus LARS artificial ligament for anterior cruciate ligament reconstruction.

This retrospective study compared the results after anterior cruciate ligament (ACL) reconstruction using a four-strand hamstring tendon graft (4SHG) versus Ligament Advanced Reinforcement System (LARS) artificial ligament in 60 patients between January 2003 and July 2004 with a minimum four-year follow-up. The KT-1000 examination, the International Knee Documentation Committee (IKDC) scoring systems and Lysholm knee scoring scale were used to evaluate the clinical results. The mean side-to-side difference was 2.4 +/- 0.5 mm and 1.2 +/- 0.3 mm in the 4SHG group and LARS group, respectively (P

The role of the posterior oblique ligament in controlling posterior tibial translation in the posterior cruciate ligament-deficient knee.

BACKGROUND: Posterior cruciate ligament injuries are often associated with injuries to other structures. The role of the posteromedial structures of the knee in these injuries has received little attention. HYPOTHESIS: The posterior oblique ligament is an important restraint to posterior tibial translation in the posterior cruciate ligament-deficient knee. STUDY DESIGN: Controlled laboratory study. METHODS: Kinematic studies were performed on 10 cadaveric knees to test 3 external loading conditions at 0 degrees , 30 degrees , 60 degrees , and 90 degrees of flexion (134 N posterior tibial lo

Median arcuate ligament syndrome: open celiac artery reconstruction and ligament division after endovascular failure.

Median arcuate ligament syndrome (MALS) is a rare disorder resulting from extrinsic compression and narrowing of the celiac artery, and--less often--the superior mesenteric artery, by the relatively low insertion of the ligament and/or prominent fibrous bands or ganglionic periaortic tissue of the celiac nervous plexus. We report on a young woman who after three consecutive attempts of endovascular therapy with balloon angioplasty and stenting for MALS, each followed by gross symptom recurrence and a cumulative weight loss of 10 kg, underwent open surgical division of the ligament and reconstr

Hybrid anterior cruciate ligament reconstruction: introduction of a new technique for anatomic anterior cruciate ligament reconstruction.

Recently, anatomic or double-bundle reconstruction of the anterior cruciate ligament (ACL) has been presented in an effort to more accurately restore the native anatomy. These techniques create 2 tunnels in both the femur and tibia to reproduce the bundles of the ACL. However, the increased number of tunnels, particularly on the femoral side, has raised some concerns among authors and surgeons. We describe a technique to reconstruct the 2 distinct bundles of the ACL by using a single femoral tunnel and 2 tibial tunnels, the "hybrid" ACL reconstruction. The femoral tunnel is drilled through an

Acute anterior cruciate ligament stump entrapment in anterior cruciate ligament tears: MR imaging appearance.

PURPOSE: To describe the magnetic resonance (MR) imaging appearance of anterior cruciate ligament (ACL) stump entrapment in patients who presented with decreased knee extension after rupturing the ACL. MATERIALS AND METHODS: MR images in 15 patients who had presented with a block to terminal knee extension after sustaining an ACL tear were retrospectively reviewed. The MR imaging appearances of entrapped ACL stumps were defined and correlated with arthroscopic and histologic findings. RESULTS: The entrapped ACL stumps had two distinct appearances. Type 1 stumps (n = 11) were characterized by

Ligament lengthening compared with simple division of the transverse carpal ligament in the open treatment of carpal tunnel syndrome.

It has been suggested that the new (and controversial) endoscopic techniques are more successful than standard operation for the preservation of the ligamentous pulley function across the carpal tunnel and for the separation of the gliding structures from the subcutaneous tissues after release of the carpal ligament in carpal tunnel syndrome. We therefore decided to study the possible importance of preserving the continuity of the carpal ligament by doing an open lengthening of the ligament. This retrospective, unrandomised study included 99 patients with carpal tunnel syndrome who underwent o

Indirect forces and patella fracture after anterior cruciate ligament reconstruction with the patellar ligament. Case report.

Patella fracture is either an infrequent or underreported complication after anterior cruciate ligament (ACL) reconstruction using a portion of the patellar ligament. Patella fracture pattern has been related to the mechanism of injury. Direct fractures resulting from impaction forces delivered to the patella are typically stellate or comminuted. Indirect fractures resulting from tensile stresses applied across the extensor mechanism typically are displaced transverse disruptions. After ACL reconstruction using a portion of the patellar ligament, Y-shaped stellate fractures can occur. Fracture

Prospective preoperative evaluation of anterior cruciate ligament instability of the knee joint and results of reconstruction with patellar ligament.

Of 54 patients with chronic (n = 49) or acute (n = 5) anterior cruciate ligament instability, 52 were evaluated after mean 16 months' follow-up interval after open (n = 18) or arthroscopic assisted (n = 36) bone-tendon-bone patellar ligament reconstruction. Preoperative Knee Signature System side-to-side difference in anterior displacement decreased from 7.9 mm to 3.5 min at follow-up evaluation (p < 0.0001). An objectively satisfactory limit of 5 mm in side-to-side difference was achieved in 73% of the patients. At follow-up evaluation, there was a positive pivot shift sign in eight knees (on

Reconstruction of chronic anterior cruciate ligament rupture using the ligament advanced reinforcement system artificial ligament-comparisons between patients over 50 years and under 50 years.

BACKGROUND: With the increasing physical activity level in elderly population, anterior cruciate ligament (ACL) injuries are becoming more frequent. Due to the possible surgery complications, treatment for ACL rupture in patients with advanced age is still controversial. The purpose of this study was to compare the therapeutic effects of reconstruction using the ligament advanced reinforcement system (LARS) artificial ligament in patients older than 50 and patients younger than 50 with chronic ACL rupture. METHODS: Indications included: (I) concurrent history of subjective symptomatic anterio

Double Posterior Cruciate Ligament Sign.

A 34-year-old male presented with persistent medial knee pain and mechanical symptoms three months after a rotational injury, with limited knee extension on examination. Magnetic resonance imaging demonstrated the double posterior cruciate ligament (PCL) sign, produced by a displaced bucket-handle tear of the medial meniscus with the fragment lying anterior and parallel to the intact PCL within the intercondylar notch. Coronal sequences confirmed displacement and loss of normal meniscal configuration. Arthroscopy verified the diagnosis, and arthroscopic partial meniscectomy was performed due t

Scapholunate ligament injuries in the nonarthritic wrist.

The scapholunate ligament (SLL) is a critical stabilizer of the wrist joint. Disruption of the SLL leads to altered wrist kinematics, including scaphoid flexion, lunate extension, and increased stress on surrounding cartilage and ligaments, which can progress to dorsal intercalated segment instability and scapholunate advanced collapse. Despite predictable biomechanical changes, clinical outcomes vary widely because some patients develop significant functional limitations, whereas others remain asymptomatic. Nonoperative management, including immobilization, therapy, and bracing, remains the f…

Management of median arcuate ligament syndrome.

Median arcuate ligament syndrome (MALS) is a diagnostically challenging vascular compression disorder resulting from external compression of the celiac artery by the diaphragmatic crura. We report a unique case of a 51-year-old woman who developed multiple visceral aneurysms involving the pancreaticoduodenal arcade, attributed to hemodynamic stress from chronic celiac artery occlusion owing to MALS-related compression. This case underscores the importance of early recognition and intervention in MALS to prevent serious downstream complications, including aneurysm formation and potentially life

A Case Report of Falciform Ligament Appendagitis.

Falciform ligament appendagitis is an extremely rare form of intra-abdominal focal fat infarction. It usually presents with vigorous abdominal pain and mimics other more common acute abdominal pain-associated diseases. Better recognition of this entity avoids misdiagnoses and unnecessary surgical treatment. We present the case of a 73-year-old woman admitted to the emergency department for abdominal pain, nausea, and vomiting. She had a fever and a diffuse tender abdomen with upper right quadrant pain. Laboratory investigation showed leukocytosis and high C-reactive protein. CT revealed a hete

The Ligament of Marshall: Far From Vestigial!

BACKGROUND: The ligament of Marshall (LOM) is a vestigial fold of the left atrium that contains the remnant left cardinal vein, muscle bundles, and nerve fibers. Despite its description as an embryologic remnant, the LOM carries significant clinical importance as an important focus for atrial fibrillation propagation as well as a therapeutic target. METHODS: In this review, we discuss the embryologic origins of the LOM, its physiology and importance in atrial fibrillation pathogenesis, as well as the role of targeting and ablating the LOM surgically during concomitant cardiac surgery includin

Timing of Anterior Cruciate Ligament Surgery.

This narrative review examines the current literature for the influence of the surgical timing in the setting of anterior cruciate ligament (ACL) reconstruction on various outcomes. Although the exact definition of early and delayed ACL reconstruction (ACLR) is a subject of controversy, surgical timing influences arthrofibrosis and postoperative stiffness, quadriceps strength, postoperative knee function, and the incidence of intra-articular injuries to the menisci and cartilage. Additionally, there is a shortage of evidence regarding the role of ACLR timing in the setting of multiligament kne

Sprain of the Medial Ankle Ligament Complex.

Injuries of the medial ankle ligament complex (MALC; deltoid and spring ligament) are more common following ankle sprains than expected, especially in eversion-external rotation mechanisms. Often these injuries are associated with concomitant osteochondral lesions, syndesmotic lesions, or fractures of the ankle joint. The clinical assessment of the medial ankle instability together with a conventional radiological and MR imaging is the basis for the definition of the diagnosis and therefore the optimal treatment. This review aims to provide an overview as well as a basis to successfully manage

Histological characterization of the human scapholunate ligament.

The scapholunate interosseous ligament (SLIL) plays a fundamental role in stabilizing the wrist bones, and its disruption is a frequent cause of wrist arthrosis and disfunction. Traditionally, this structure is considered to be a variety of fibrocartilaginous tissue and consists of three regions: dorsal, membranous and palmar. Despite its functional relevance, the exact composition of the human SLIL is not well understood. In the present work, we have analyzed the human SLIL and control tissues from the human hand using an array of histological, histochemical and immunohistochemical methods to

A Rare Case of a Lisfranc Ligament Transection.

Ligamentous Lisfranc injuries, which mimic a surgical transection are rare. Left untreated or missed, these injuries may result in severe complications including long-term disability. The present case describes a unique mechanism of injury via a sharp object puncture through the plantar surface of the midfoot. It highlights the utility of planning staged procedures to remove a large foreign body object, reports notable intraoperative techniques, and provides a decision management guide for treating this type of injury. Extensive surgical planning may be required when encountering similar cases

Anatomy of the anterolateral ligament of the knee joint.

Despite remarkable improvements in clinical outcomes after anterior cruciate ligament reconstruction, the residual rotational instability of knee joints remains a major concern. The anterolateral ligament (ALL) has recently gained attention as a distinct ligamentous structure on the anterolateral aspect of the knee joint. Numerous studies investigated the anatomy, function, and biomechanics of ALL to establish its potential role as a stabilizer for anterolateral rotational instability. However, controversies regarding its existence, prevalence, and femoral and tibial insertions need to be addr
