Timing optimization for primary pterygium excision with conjunctival-limbal autograft to restore the corneal optical properties.

PURPOSE: To propose the optimal value of baseline corneal astigmatism and pterygial morphological profiles for primary pterygium surgery to restore the corneal optical properties. METHODS: We analysed 93 eyes from 84 subjects with nasal-only primary pterygium who underwent pterygium excision with conjunctival-limbal autograft and were assessed perioperatively using anterior segment swept-source optical coherence tomography (AS SS-OCT). We collected data on anterior corneal astigmatism (ACA) and root mean square (RMS) values for anterior corneal lower- (LoA) and higher-order aberrations (HoA)

The Effect of Amniotic Membrane Transplantation or Conjunctival Autografts on the Tear Mucins MUC5A and MUC2 After Pterygium Resection: A Six-Month Follow-Up.

PURPOSE: Pterygium is an ocular surface disease characterized by the invasion of fibrovascular tissue from the bulbar conjunctiva to the cornea and is associated with abnormal tear function caused by changes in tear composition and osmolarity. In this study, the effect of two different surgical techniques to remove primary pterygium: conjunctival autograft surgery (CAG) and amniotic membrane transplantation (AMT), on changes in MUC2 and MUC5AC tear mucins concentration were evaluated. METHODS: Forty-four patients (>18 years old) with primary unilateral pterygium (> 1.0 mm long, measured from

Shaping the Future of Limbal Stem Cell Therapy: Advances in Substrates, Techniques, and Science for Cultivated Autologous and Allogeneic Limbal Epithelial Transplantation.

Limbal stem cell deficiency (LSCD) is a rare but often progressive condition associated with significant ocular morbidity and visual loss. LSCD can be partial or total and can affect one or both eyes, important factors to consider when planning disease management. Decades of laboratory and clinical research have provided the basis for the contemporary understanding and management of this condition. In this narrative review, we highlight the important breakthroughs in limbal stem cell science which have shaped limbal stem cell therapy, current cell culture techniques, and the respective merits

Exploring the impact of human pluripotent stem cell heterogeneity on corneal limbal stem cell differentiation outcomes.

Differentiation of limbal stem cells (LSCs) from human pluripotent stem cells (hPSCs) shows promise for treating bilateral limbal stem cell deficiency. However, variation in hPSCs can compromise cell production protocols and increase unwanted heterogeneity. We hypothesize that intrinsic differences among hPSC lines affect their efficiency and consistency in LSC differentiation. Several hPSC lines were differentiated towards LSCs, and the process was analyzed in different timepoints using real-time qPCR (RT-qPCR) and immunofluorescence (IF) to assess differentiation efficiency. Flow cytometry (

Gene expression study in the siRNA based aniridia cell model and in primary aniridia limbal epithelial cells following duloxetine and ritanserin treatment.

Progressive aniridia associated keratopathy is worsening visual acuity of congenital aniridia subjects lifelong. Restoration of PAX6 expression in PAX6 haploinsufficient limbal epithelial cells could be one therapeutic option. In a previous study using aniridia-like CRISPR/Cas9 genome-edited corneal epithelial cells, the antipsychotic drugs duloxetine and ritanserin increased PAX6 mRNA and protein expression. Our purpose was to investigate the effect of duloxetine and ritanserin on cultured primary limbal epithelial cells (pLECs) without and with PAX6 knockdown. pLECs were isolated from 11 ani

Mini-simple limbal epithelial transplantation and conjunctival-limbal autograft for the treatment of primary pterygium: Study protocol for a randomized controlled trial.

BACKGROUND: Pterygium is a wing-shaped fibrovascular growth over the cornea that may require surgical removal due to recurrence and cosmetic concerns. This randomized controlled trial compares the recurrence rate of conjunctival-limbal autograft (CLAu) with a modified mini-simple limbal epithelial transplant (mini-SLET) technique. METHODS: 68 patients will be randomly assigned to two equally sized groups via simple randomization. Group I will undergo a pterygium excision with CLAu, while Group II will undergo a pterygium excision using a mini-SLET. Allocation concealment will be ensured using

Multi-parametric evaluation of autologous cultivated Limbal epithelial cell transplantation outcomes of Limbal stem cell deficiency due to chemical burn.

BACKGROUND: The sparsity of established tools for the grading of limbal stem cell deficiency hinder objective assessments of the clinical outcome of cultivated limbal epithelial cell transplantation. To advance towards the development of standards for the comparison of the outcomes of these bio-surgical protocols we have now applied a battery of recognized objective and patient-declared subjective outcome criteria to the autologous modality of cultivated limbal epithelial cell transplantation. METHODS: The prospective study involved ten patients (M/F = 9/1; mean age = 42.1 years) displaying o

Early effects of dexamethasone and anti-VEGF therapy in an inflammatory corneal neovascularization model.

Inflammatory angiogenesis is the pathogenic mechanism of various sight-threatening eye diseases, among them corneal neovascularization. Current treatment options include steroids which have undesirable side effects, or anti-VEGF which has only limited efficacy. In an inflammatory environment, however, angiogenesis can be stimulated by numerous factors not directly targeted by anti-VEGF therapy. The aim of this study was to induce corneal inflammation leading to angiogenesis, and investigate the early, differential effects of steroid and anti-VEGF therapy at the cellular, tissue, and gene expre

A circum-corneal conjunctival nevus in a child.

An amelanotic, circum-corneal nevus in a 2-year-old child is described. The nevus presented at birth as a red spot in the nasal conjunctiva that subsequently enlarged to completely encircle the cornea. The tumour was partially removed three times, but at the age of 6 years, the nevus still covers the entire limbal region. The case illustrates that circum-corneal redness in a child may be caused by a nevus and that a conjunctival limbal nevus in a child tend to recur after incomplete excision.
