Students are not customers: a better model for medical education.

The author argues that the student-as-customer model of medical education has many failings that result in interactions that are educationally dysfunctional. Ten "pathologies" resulting from the adoption of this model are presented (e.g., "The student-customer model seduces students into believing that they know what is best for them"). Part of the reason for the unprofessional conduct often demonstrated by students and faculty alike may be a result of the influence of this model on medical education and the consequent inappropriate empowerment of students in the role of customers, the diminis

Global grassland productivity and carbon storage benefit from functional diversity under climate change.

Functional diversity plays an important role for ecosystem services of managed grasslands such as their gross primary productivity (GPP) and soil organic carbon (SOC) stocks. Yet vegetation models assessing functional diversity have been limited to small spatial scales. We used the Lund-Potsdam-Jena managed land (LPJmL) competitor stress-tolerant ruderal (CSR) model to assess the role of functional diversity for managed grassland GPP and SOC under climate change at the global scale. LPJmL-CSR simulates functional diversity using plant functional types representing different ecological strategi

Specialty care at the crossroads: electrophysiology practice in the managed-care era.

The economics of the managed-care era have declared that there is an o ver supply of specialists. Especially vulnerable to the changing marketplace are electrophysiologists whose technology is poorly understood by managed-care plans. To be successful, electrophysiology (EP) practices must develop strategies to contend with the new reality. Over the past decade, electrophysiologists at Good Samaritan Hospital (Los Angeles, CA) have worked with the leadership of a large managed-care organization (Family Health Plan [FHP]) to develop guidelines for clinical EP procedures that optimize clinical ou

The obstetrical anaesthesia assessment clinic: a review of six years experience.

We reviewed the out-patient consultation notes of 136 pregnant women seen at the Ottawa Civic Hospital from 1985 to 1991 to evaluate the efficacy of an Obstetric Anaesthesia Assessment Clinic (OAC). In addition, their anaesthetic records from labour and delivery were reviewed. For each patient the reason for referral was recorded according to the involved organ system. The anaesthetic management at delivery was compared with the proposed anaesthetic plan by the OAC consultant (obstetric anaesthetist). The majority of women 84 (62%) had complaints related to the musculo-skeletal system. In addi

Working toward sustainability: Transitioning HIV programs from a USA-based organization to a local partner in Zimbabwe.

BACKGROUND: Despite the history of United States of America (USA)-based partners implementing global health programs in low- and middle-income countries (LMIC), future models for sustainable healthcare rely on local country ownership and leadership. Transition is the process of shifting programs towards country ownership, where local stakeholders plan, manage, and deliver health programs. Transition is not a singular event but a process which may include a phase where health programs are led and managed by local entities but still reliant on awards from international partners. This phase is sc

Parental Experiences in Caring for Children with Surgically Treated Hydrocephalus: A Phenomenological Study from Eastern India.

BACKGROUND: Congenital anomalies may be structural or functional and manifest at birth or be detected later in life. One such condition, hydrocephalus, is commonly managed with a Ventriculo-peritoneal (VP) shunt. As children grow with a shunt in situ, parents play a vital role in recognizing the symptoms of shunt malfunction and reporting for medical care. The possible complications are unpredictable and require a keen awareness of the associated symptoms and signs. MATERIALS AND METHODS: A qualitative descriptive phenomenological design explored experiences of parents caring for children wit

The effect of appropriate bladder management on urinary tract infection rate in patients with a new spinal cord injury: a prospective observational study.

PURPOSE: This study aimed to determine the rate of urinary tract infection (UTI) in patients with a new spinal cord injury (SCI) and identify which bladder management technique is associated with the lowest rate of UTI. METHODS: Adults admitted to the Victorian Spinal Cord Service with a new SCI from 2012 to 2014 were enrolled. Data collected included patient characteristics, SCI level, bladder management and diagnosis of UTI. Bacteriuria (≥ 102 colony-forming organisms/mL) with clinical signs of infection was used to define a UTI. RESULTS: 143 patients were enrolled. 36 (25%) were female; t

Academic managed care organizations and adverse selection under Medicaid managed care in Tennessee.

CONTEXT: Health plans competing in a managed care system may face serious financial consequences if they are disproportionately selected by enrollees with expensive health conditions. Academic medical centers (AMCs) have traditionally provided medical care for the sickest patients and may be at particularly high risk for adverse selection, but whether this occurs is not known. OBJECTIVE: To determine whether managed care organizations (MCOs) representing AMCs are adversely selected by Medicaid managed care (MMC) enrollees with expensive chronic health conditions. DESIGN AND SETTING: Observat

Highlighting Healthcare Disparities in Sickle Cell Disease Management in Resource-Constrained Settings: A Cross-Sectional Study in Lebanon.

BACKGROUND: Sickle cell disease (SCD) is a genetic disorder characterized by crescent-shaped red blood cells, which can result in a number of complications. SCD prevalence is higher in countries with limited resources and, in certain regions such as Lebanon, may be compounded by high consanguinity rates, posing significant challenges for both healthcare systems and patients. This study aimed to assess patient needs and the organization of SCD management centers, to identify service gaps, and enhance care. METHODS: SCD patients, caregivers, or legal representatives completed questionnaires abo

Exploring potential barriers and facilitators to integrate tuberculosis, diabetes mellitus, and tobacco control programmes in India.

BACKGROUND: Co-integrating tuberculosis (TB), diabetes mellitus (DM), and tobacco control (TC) programmes in India could help address the triple burden of these diseases. However, limited information exists regarding the feasibility and determining factors of such integration. We explored potential barriers and facilitators to integrating TB, DM, and TC programmes in Ambegaon Block of Pune District, Maharashtra, and Ballabgarh Block of Faridabad District, Haryana, in India. METHODS: We conducted a qualitative study based on in-depth interviews with health workers, programme managers, and stak
