Chronic suppurative otitis media and related complications at the University Clinic of Kinshasa.

During the five-year period from 1998 to 2002, the ENT department of the University Clinic of Kinshasa treated 343 patients suffering from c … Hearing loss was moderate to severe in 2/3 and slight in 1/3 of the audiometrically tested ears. A mastoidectomy was performed on 55% of the patients. Roughly 1/3 of the patients presented with complications, the majority of which (69) were subperiostal abscesses. Complications other than subperiostal abscesses were associated with a high morbitidy and a few p …

Tegmen tympani cerebrospinal fluid leak repair.

OBJECTIVE: To describe the transmastoid approach for closing cerebrospinal fluid leaks in the tegmen tympani using autologous materials (temporal muscle fascia and abdominal fat) and heterologous materials (bovine collagen), together with its advantages and disadvantages. MATERIAL AND METHODS: We present the cases of three patients who underwent closure of spontaneous cerebrospinal fluid leaks in the tegmen tympani in which we used the transmastoid approach and autologous and heterologous materials. The three patients underwent canal wall-up mastoidectomy, posterior tympanotomy, removal of th…

[Parapharyngeal chondrosarcoma and Ollier's disease: a case report and review of the literature].

OBJECTIVES: Ollier's disease is a constitutional pathology of unknown etiology. It is characterized by bone dysplasia generating numerous enchondromas. The malignant degeneration of this dysplasia is well known. The aim of this article is to study the diagnostic, therapeutic and prognostic characteristics of these lesions. MATERIAL AND METHODS: We report a case of parapharyngeal chondrosarcoma extended to the base of the skull in a patient with Ollier's disease. The treatment was a surgical removal by a cervicotransoral incision combined with a preauricular incision and with a mastoidectomy. …

[A case of pediatric recurrent acute mastoiditis caused by penicillin-resistant Streptococcus pneumonia complicated by primary immunodeficiency].

Penicillin-resistant Streptococcus pneumoniae (PRSP) is a frequently detected pathogen of intractable acute otitis media and is associated with prolonged or recurrent infection. The use of antibiotics has made the incidence of secondary acute mastoiditis following acute otitis media relatively rare, but when it does occur, its severe complications may be life-threatening. We report a case of pediatric recurrent acute mastoiditis caused by PRSP in a 6-year-old boy suffering from PRSP acute mastoiditis on 4 occasions, twice undergoing simple mastoidectomy. Although we initially suspected PRSP to…

[The retro-auricular, transmastoid, infralabyrinth approach. A simple route for excision of tumors of the jugular foramen].

BACKGROUND: The jugular foramen is a complex area of the skull base. Its contents, the anatomical relationships in the region, and its location at the skull base, are responsible for problematic surgical approaches. The classical infratemporal surgical routes remain complex and difficult to perform. The lateral approach through a mastoidectomy is almost always associated with an anterior transposition of the facial nerve although that transposition is usually unnecessary. METHODS: Progressive drilling along the sinusojugular axis, inferior to the labyrinth and medial to the third portion of t…

Mastoid abscess: underlying disease and management.

Despite a reduction in the incidence, subperiosteal mastoid abscesses (MAs) are still seen today with significant potentials for morbidity a … se with CSOM were older than 10 years. Fifty percent of the MAs that resulted from AOM had cortical mastoidectomy. The other five patients were treated with a less invasive method: antibiotics and incision and drainage without mastoid surgery. In eight MA cases caused by CSOM, the management of the abscess as well as the underlying disease was performed in one …

Long-term results of treatment for children's cholesteatoma.

Fifty-seven of 98 children with cholesteatoma treated between 1976 and 1985 were available for long-term follow-up. The clinical records of the 57 children were analyzed retrospectively. The purpose of this study was to compare the incidence of recurrent and/or residual cholesteatoma, postoperative otorrhea, and hearing results between two groups of patients. One group (n = 40) of children was treated with a radical mastoidoepitympanectomy (RMET) and the other group (n = 17) was treated with intact canal wall tympanoplasty/mastoidectomy (ICWT). Residual cholesteatomas were twice as frequent (1…

The use of computerized tomographic imaging in revision mastoid surgery for chronic otitis media.

Recurrent otorrhea, pain, and a conductive hearing deficit are common indicators of possible recurrent disease following mastoidectomy for chronic otitis media. High-resolution computed tomography (CT) provides an excellent method for examination of the surgically altered mastoid. Areas of residual infected air cells, cholesteatoma, cholesterol granuloma, and other surgical conditions within the mastoid are localized by this technique prior to reexploration. The technique provides information regarding the extent of disease as well as possible anatomic variations and potential complications th…

Acute and latent mastoiditis in children.

During the period from 1974 to 1981, surgery for acute mastoiditis was performed on 12 ears, giving an annual incidence of 0.004 per cent among cases of acute otitis media. All ears made a full long-term recovery. The low incidence is ascribed to the world-wide early use of antibiotics. During the same period 52 ears with secretory otitis media (SOM) underwent mastoidectomy. Histologically extensive mastoid inflammation was found in 96 per cent. The changes appeared as formation of secretory cells and cysts, resorption of bone, and infiltration of soft tissues with both mononuclear cells and p…

[A female patient with earache, polyuria and polydipsia].

INTRODUCTION: Langerhans cell histiocytosis is a rare inflammatory bone marrow neoplasia that frequently affects bone, lung, skin and pituitary gland. Due to its broad spectrum of clinical presentation, an appropriate diagnosis might be difficult. HISTORY: A 54-year-old female patient complained of pain in her right ear for 5 months. On account of similar complaints, a mastoidectomy had already been performed 3 years ago. Histology at that time revealed nonspecific inflammation. Furthermore, she reported excessive thirst. FINDINGS AND DIAGNOSIS: Computed tomography of the temporal bones show…

Does Dehiscence of the Facial Nerve Canal Affect Tympanoplasty Results?

This study aimed to evaluate the prevalence of facial nerve canal dehiscence in tympanoplasty patients and its influence on the need for revision surgery and on hearing results and anatomical outcomes. Patients who underwent tympanoplasty with/without mastoidectomy at Ankara University Otolaryngology Department from 2006 through 2013 with a minimum follow-up period of 6 months were reviewed retrospectively in this original study. Patients were divided into those with and without cholesteatoma. Numbers and frequencies of dehiscence were recorded according to disease type, the need for revision …
