Malignant giant cell tumor of the sphenoid.

Malignant giant cell tumors (MGCTs) of the sphenoid sinus are extremely rare neoplasms. They are challenging to diagnose and difficult to treat because of their skull base location. To the best of our knowledge, we report the first case of a primary MGCT of the sphenoid arising in a patient with Paget's disease. A 77-year-old man presented with epistaxis and a history of Paget's disease. There was normal cranial nerve function although radiographic images disclosed a large mass centered in the sphenoid sinus and extending into the ethmoid and maxillary sinuses. Excisional biopsy revealed a MGC

Iodine-125 brachytherapy for orbital-invasive low-grade myxofibrosarcoma of the maxillary sinus: a case report challenging conventional therapeutic paradigms.

Low-grade myxofibrosarcoma (LGMFS) is a rare subtype of soft tissue sarcoma, with an estimated incidence of approximately 0.18 cases per million population, accounting for about 0.6% of all soft tissue sarcomas. It is characterized by a high local recurrence rate and malignant potential. Orbital involvement secondary to maxillary sinus LGMFS represents an exceptionally rare occurrence. The management of such cases is complicated by anatomical complexity (e.g., proximity to optic nerve, lacrimal apparatus, and orbital vasculature) and limitations of conventional therapies, including radical res

Root canal complications: 'the hypochlorite accident'.

Root canal treatment is performed routinely in dental practice, using sodium hypochlorite which serves as an effective irrigant. The literature reviewed shows that several complications following irrigation with sodium hypochlorite may occur, but few practitioners are aware of it and its management. Such complications include injury to skin, oral mucosa and eyes, damage to clothing, air emphysema, allergic reactions, and injection beyond the foramen. In this article, a case report of injection with sodium hypochlorite beyond the foramen is presented, together with a review of the recent litera

A Call for Vigilance: Trigeminocardiac Reflex Presenting As Ventricular Bigeminy During Marginal Mandibulectomy.

The trigeminocardiac reflex (TCR) is an autonomic reflex elicited by stimulation of the trigeminal nerve and is most commonly characterized by bradycardia and hypotension. In oral and maxillofacial surgery, peripheral TCR may occur during manipulation of the maxillary or mandibular divisions, occasionally presenting with atypical cardiovascular responses. We report a rare case of peripheral TCR manifesting as sudden severe hypertension with ventricular bigeminy during wide local excision and marginal mandibulectomy in a 69-year-old male with carcinoma of the lower alveolus. The hemodynamic dis

Unilateral port wine stain on the face: a case report and review.

INTRODUCTION: Sturge-Weber syndrome (SWS) is a sporadic, progressive, congenital condition that occurs due to hamartomatous malformation and is usually referred to as a "port wine stain." It is characterized by trisymptomatic forms that include facial port wine stain, glaucoma, and leptomeningeal calcifications. CASE DESCRIPTION: A 42-year-old female patient presented with a chief complaint of missing teeth in the mandibular anterior region that had recently exfoliated following mobility. A unilateral port wine stain noted on the left side of her face appeared to follow the left maxillary div

Glioblastoma, IDH-wildtype with leptomeningeal metastasis to Meckel's cave: A case report.

Meckel's cave or the trigeminal cistern is a subarachnoid space near the apex of the petrous portion of the temporal bone and contains cerebrospinal fluid and the Gasserian ganglion, which divides into the ophthalmic (V1), maxillary (V2), and mandibular (V3) nerves. Infectious, inflammatory, congenital, and neoplastic lesions can occur in Meckel's cave. Leptomeningeal metastasis of glioblastoma (GBM), IDH-wildtype to Meckel's cave is rare. We encountered a case of leptomeningeal metastasis of GBM to Meckel's cave in an elderly female patient who presented with pain around her right eye. Magnet

Interplay of Oral, Mandibular, and Facial Disorders and Migraine.

PURPOSE OF THE REVIEW: Migraine and other primary headache disorders can be localized in the face resembling facial or dental pain, indicating the influence of the trigeminovascular system in the structures innervated by the maxillary (V2) and mandibulary (V3) branches of the trigeminal nerve. Disorders of oral and craniofacial structures may influence primary headache disorders. In the current article, we review the potential links of this interplay. RECENT FINDINGS: This interplay may be related to anatomy, with the trigeminal pathway and the involvement of both peripheral and central mecha

Multidetector computed tomography in preoperative planning for temporomandibular joint ankylosis: A pictorial review and proposed structured reporting format.

Ankylosis of the temporomandibular joint (TMJ) is a disabling disease resulting from fibrous or bony fusion of the mandibular condyle and the glenoid fossa. Early diagnosis and surgical treatment are essential to prevent facial deformity and other complications. Conventional radiography has limitations in demonstrating the true extent of ankylosis. It is important for surgeons to be aware of the size and degree of bony ankylosis in order to perform complete resection of the ankylotic mass. In addition, a detailed evaluation of the relationship with adjacent vital structures such as the interna

Trifurcated external carotid artery and complete gamma-loop of its maxillary branch.

The external carotid artery (ECA) normally bifurcates terminally with the superficial temporal artery (STA) and the maxillary artery (MA). From the horizontally coursing, mandibular segment of the MA leaves the middle meningeal artery (MMA). We hereby report a previously unknown anatomic possibility, incidentally found during an angio-CT scan in an adult female patient. Unilaterally, the ECA was terminally trifurcated, sending off the MA, STA, and MMA. On that side, the mandibular segment of the MA had a gamma-loop and the contralateral one had a U-loop; both these loops were inferior to the l

Paroxysmal vascular events in Sturge-Weber syndrome: Role of aspirin.

Sturge-Weber syndrome (SWS) is a rare, sporadically occurring neurocutaneous disorder with a frequency of approximately 1 per 50,000. The hallmark is an intracranial leptomeningeal vascular angioma in association with a port wine nevus, usually involving ophthalmic or maxillary distribution of trigeminal nerve. Other clinical findings associated with SWS are seizures, glaucoma, hemiparesis and mental retardation. The radiological hallmark is "Tram-line" or "Gyri-form" calcification. 25 to 56% of patients experience recurrent episodes of paroxysmal focal neurological deficits in form of transie

An interesting case of gunshot injury to the temporomandibular joint.

The head and face are relatively common sites of gunshot injury, and the temporomandibular joint is often affected. These wounds usually produce major deformity and functional impairment, particularly when the temporomandibular joint is affected or when structures such as the facial nerve are damaged. Complications may include mandibular displacement at maximum mouth opening and in protrusion, limited mouth opening, limited lateral movement of the jaw, anterior open bite, and, more rarely, temporomandibular ankylosis. Projectiles that strike the mandible usually cause comminuted fractures; max

Intraoral presentation of multiple malignant peripheral nerve sheath tumors associated with neurofibromatosis-1.

Neurofibromatosis-1 (NF-1) is a relatively common autosomal dominant disease characterized by multiple cutaneous fibromatoses and café au lait spots. It is associated with the mutation of NF-1 gene, a tumor suppressor gene located on chromosome 17q11.2. Hence, it can be considered as a familial cancer predisposition syndrome in which the affected individuals are at increased risk of developing malignancies. Intraoral neurofibromas associated with NF-1 are quite common, but the occurrence of malignant peripheral nerve sheath tumor (MPNST) in the oral cavity is very rare. Oral MPNST can occur ei

Osteopetrosis-associated osteomyelitis of the jaws: a report of 4 cases.

Osteopetrosis represents a heterogeneous group of rare, hereditary bone disorders with variable clinical features and an increase in bone density. The common clinical findings that usually lead to the detection of the disease are fractures and osteomyelitis of the mandible. We report 4 cases of osteopetrosis, complicated by osteomyelitis of the jaws. Maxillary bone was involved in 3 of the cases and mandible in 1 of the cases. Osteomyelitis of the maxilla is extremely rare. When it occurs, there is invariably an underlying predisposing condition. All the 4 patients presented with osteomyelitis

Free vascularised iliac crest graft: an audit of 26 consecutive cases.

Over an 18 month period 26 vascularised myo-osseus, and one myo-osseus-cutaneous iliac crest flaps were used to reconstruct 18 mandibular and eight maxillary defects. During the operation one flap failed to reperfuse and was removed. Postoperatively three patients were returned to theatre for appraisal and reanastomoses, which in two cases involved thrombolysis. In all three salvage was deemed successful but in all there was partial muscle necrosis. There were 10 cases of anaesthesia of the skin supplied by the lateral cutaneous nerve of the thigh, three incisional hernias, one wound dehiscenc

Radiographic features of central giant cell granuloma of the jaws in children.

The radiographic features of ten pediatric cases of central giant cell granuloma of the jaws were studied, using plain film radiography (PFR), computed tomography (CT), and a dental CT software program (DS). The radiologic features varied from ill-defined destructive lesions to a well-defined, multilocular appearance. Teeth or root displacement was found as the most consistent feature. Root resorption was rare. The features seen on CT were clearer than those seen on PFR. DS, by its visualization of the jaw in three plans - axial, panoramic, and buccolingual - provided useful information for de

Ocular manifestations of the Smith-Lemli-Opitz syndrome.

To our knowledge, this article describes the first ocular histopathologic condition of a Smith-Lemli-Opitz proband, despite almost 60 clinical histories that exist in the literature. The sole retinal abnormality in this 1-month-old infant with congenital bilateral cataracts is the extensive dropout of peripheral ganglion axons with incipient optic nerve demyelination. Unusual amorphous cytoplasmic masses that are continuous with photoreceptor discs are prominent aspects of the peripheral subretinal space. The morphological data imply that the localized mitochondrial disintegration is restricte

Clinical and imaging findings, treatments, and outcomes in 27 dogs with imaging diagnosed trigeminal nerve sheath tumors: A multi-center study.

The clinical behavior of canine trigeminal nerve sheath tumors and benefits of previously reported treatments are incompletely defined. Aims of this retrospective, multicenter, observational study were to describe clinical signs, tumor localization characteristics, treatments, and clinical outcomes in a group of dogs with this neoplasm. Databases at four hospitals were reviewed for dogs with a trigeminal nerve sheath tumor diagnosis, magnetic resonance imaging (MRI) studies, and presentation between 2004 and 2014. A single observer recorded medical record findings and two observers recorded MR
