A point-of-care test for measles diagnosis: detection of measles-specific IgM antibodies and viral nucleic acid.

OBJECTIVE: To evaluate the performance of a newly developed point-of-care test (POCT) for the detection of measles-specific IgM antibodies in serum and oral fluid specimens and to assess if measles virus nucleic acid could be recovered from used POCT strips. METHODS: The POCT was used to test 170 serum specimens collected through measles surveillance or vaccination programmes in Ethiopia, Malaysia and the Russian Federation: 69 were positive for measles immunoglobulin M (IgM) antibodies, 74 were positive for rubella IgM antibodies and 7 were positive for both. Also tested were 282 oral fluid …

The laboratory confirmation of suspected measles cases in settings of low measles transmission: conclusions from the experience in the Americas.

The Americas have set a goal of interrupting indigenous transmission of measles using a strategy developed by the Pan American Health Organization (PAHO). This strategy includes recommendations for vaccination activities to achieve and sustain high immunity in the population and is complemented by sensitive epidemiological surveillance systems developed to monitor illnesses characterized by febrile rash, and to provide effective virological and serological surveillance. A key component in ensuring the success of the programme has been a laboratory network comprising 22 national laboratories in…

Vitamin a supplementation of vitamin a deficient measles patients lowers the risk of measles-related pneumonia in zambian children.

This is an a posteriori analysis of previously published data to assess whether improving vitamin A (VA) status resolves measles-related pneumonia (MP). Nonhospitalized acute measles patients (2 d of rash onset) had their VA status determined based on the molar ratio of retinol-binding protein to transthyretin (RBP/TTR). Using a cutoff value of </=0.36, indicative of marginal VA deficiency, 82 children were diagnosed as marginally VA deficient and 114 were diagnosed as VA sufficient. At baseline, marginally VA-deficient patients had significantly lower serum retinol and higher serum C-reactive…

Expression of measles virus RNA in peripheral blood mononuclear cells of patients with measles, SSPE, and autoimmune diseases.

In order to characterize measles virus (MV) infection in peripheral blood mononuclear cells (PBMCs), RNA was isolated from PBMCs after PHA-stimulation for 72 hr of 9 patients with acute measles, 16 patients with subacute sclerosing panencephalitis (SSPE), 13 patients with various autoimmune diseases, and 16 healthy control donors. The RNA obtained was screened for the presence of MV N (nucleocapsid) gene specific transcripts of either positive or negative orientation in a S1 nuclease protection assay. The sensitivity of this assay allowed us to detect one infected cell in 20,000 PBMCs or 0.1 t…

Measles Vaccine Boosters in Immunocompromised Adults to Reduce Morbidity and Mortality From Acute Measles Infection: A Scoping Review.

Measles is a highly contagious viral illness associated with significant global mortality, despite the availability of an effective vaccine. As vaccine hesitancy continues to increase following the COVID-19 pandemic, combined with a lack of measles-specific antiviral agents, the risk of outbreaks and epidemics remains elevated. Immunocompromised individuals have a higher risk of serious complications from measles, especially because the live-attenuated measles, mumps, and rubella vaccine may be contraindicated in severely immunocompromised patients. As such, these individuals depend on herd im…

Insights from 25 Years of Measles and Measles-Rubella Vaccination Campaigns in the WHO African Region (2001-2025).

INTRODUCTION: The WHO African Region is working to eliminate measles and rubella in 80% of countries by 2030. In countries with sub-optimal routine immunization coverage, periodic supplemental Immunization Activities (SIAs) are implemented to boost childhood immunity against measles and rubella. METHODS: We reviewed the SIA technical reports and reports from post-campaign surveys shared by countries with the WHO Regional Office for Africa, and we analyzed the coverage data from preventive measles campaigns implemented during the years 2001-2025. RESULTS: A total of 326 preventive measles/mea…

Measles Virus Bearing Measles Inclusion Body Encephalitis-Derived Fusion Protein Is Pathogenic after Infection via the Respiratory Route.

A clinical isolate of measles virus (MeV) bearing a single amino acid alteration in the viral fusion protein (F; L454W) was previously identified in two patients with lethal sequelae of MeV central nervous system (CNS) infection. The mutation dysregulated the viral fusion machinery so that the mutated F protein mediated cell fusion in the absence of known MeV cellular receptors. While this virus could feasibly have arisen via intrahost evolution of the wild-type (wt) virus, it was recently shown that the same mutation emerged under the selective pressure of small-molecule antiviral treatment. …

Reactogenicity and immunogenicity of measles-rubella combined vaccine in school-entry-aged subjects with naturally acquired measles immunity.

BACKGROUND: The reintroduction of measles-rubella combined (MR) vaccination to Japan raised concerns about adverse events as well as immunogenicity related to booster immunization in subjects with naturally acquired immunity to measles or rubella. METHODS: The time course of reactogenicity and antibody responses in recipients with pre-existing immunity to measles through natural infection was observed. Eighteen children aged 80-104 months received MR booster vaccination; 16 of them had had previous rubella vaccination. RESULTS: There were virtually no clinical reactions related to booster va…

Interleukin-4 induced by measles virus and measles-derived peptides as measured by IL-4 receptor-blocking ELISA.

Interleukin-4 (IL-4) is a signature cytokine for T-helper 2 (Th2) type immune responses in humans. However, data on antigen-specific secretion of IL-4 is limited due to difficulties detecting IL-4. We evaluated an IL-4 receptor-blocking assay for the detection of secreted IL-4 in peripheral blood mononuclear cells (PBMC) stimulated in vitro with measles virus (MV) and MV-derived nucleoprotein (N) and phosphoprotein (P) peptides. We recruited 20 healthy subjects, previously immunized with two doses of measles-mumps-rubella-II (MMR-II) vaccine. We evaluated the cellular and humoral immune status…

Measles and rubella antibody response after measles-mumps-rubella vaccination in children with afebrile upper respiratory tract infection.

The effect of concurrent afebrile viral upper respiratory tract infection (URI) on measles and rubella serum antibody response after measles-mumps-rubella (MMR) vaccination was determined in 580 1-year-old children, 369 with URI and 211 without URI, with the use of preimmunization and postimmunization sera. The presence of URI during the preceding 28 days, at the time of, or up to 7 days after immunization had no effect on the antibody response to either the measles or the rubella component of the vaccine. These findings support the recommendation that MMR vaccine be give to children at the re…

Measles-mumps-rubella immunization of susceptible hospital employees during a community measles outbreak: cost-effectiveness and protective efficacy.

OBJECTIVE: To determine cost-effectiveness and protective efficacy of a program to identify and immunize susceptible hospital employees during a measles outbreak. DESIGN: A cost analysis was made of blind measles-mumps-rubella (MMR) immunization versus directed MMR immunization based on 2,000 employees born after December 31, 1956. A directed MMR immunization program for susceptible employees was instituted. Actual costs of the program were calculated at the conclusion of the program. SETTING: A medical center complex with more than 4,000 employees, two acute care community hospitals, and a …

Subacute sclerosing panencephalitis after passive immunization and natural measles infection: role of antibody in persistence of measles virus.

Subacute sclerosing panencephalitis (SSPE) developed in a patient in whom natural measles infection was anteceded by immunization with measles immune serum globulin (ISG). This observation prompted experimental studies of the role of antibody in viral persistence. When Balb/c mice were infected with the hamster neurotropic measles virus, acute encephalopathy was fatal in 80% of the animals. When measles antibody was administered 3 days after virus inoculation, the acute disease was abolished and subacute encephalitis had a 30% mortality. The subacute disease was characterized by the presence o…

Antiviral targets in the prevention and treatment of measles.

PURPOSE OF REVIEW: Despite the efforts of national and international health agencies to eradicate measles virus (MeV), the causative agent of measles, progress has stagnated and cases are rising. Since 1963, a well tolerated and efficacious live-attenuated vaccine has been used to remarkable success, leading to the near eradication of measles virus. Nevertheless, increased vaccine hesitancy has led to a resurgence in the past decade. The development of specific antiviral therapeutics to combat measles is lagging but increased efforts are justified considering the current measles resurgence and…

Measles is misery: A brief update for paediatricians.

Measles is an important vaccine-preventable disease that has re-emerged in recent years. Since the COVID-19 pandemic, interruptions to routine immunisation programs and declining vaccine coverage have altered the incidence and patterns of respiratory virus infections. Global outbreaks have intensified, and vaccine hesitancy is recognised as major health threat. Revisiting the clinical presentation of measles is crucial for early diagnosis and to reduce transmission of this highly contagious infection. As serious respiratory and neurological complications can follow natural infection and no spe…

The Resurgence of Measles: A Rash From the Past.

Measles, or rubeola, remains a highly contagious infectious disease with a concerning resurgence in the United States. Despite previous control efforts, the number of reported cases continues to rise, surpassing the total for the previous year in just the first quarter of 2024 (CDC, 2024a). Emergency nurse practitioners and other emergency clinicians are likely to encounter patients presenting with concerns of or exposure to measles. However, given the low frequency of cases in the past, many emergency clinicians have likely not previously encountered measles, making identification more challe…

Effects of a measles outbreak on vaccination uptake.

This paper explores the effects of a measles outbreak on vaccination uptake in Austria, using administrative data with individual-level information on childhood vaccinations. I define a treatment group of children affected by the outbreak, and compare them with a control group of earlier-born children who are unaffected. Twelve months after the outbreak, the vaccination rate of the treatment group is 2.5 (first dose of the measles, mumps and rubella vaccine) and 4 (second dose) percentage points higher than the corresponding rates of the control group. The results do not indicate that families…

An Update and Review of Measles for Emergency Physicians.

BACKGROUND: It is vital for frontline emergency physicians to immediately recognize the signs and symptoms of measles to initiate appropriate therapy and prevent spread to the health care team and other patients. OBJECTIVE: This review serves as a clinically practical updated reference for when the differential diagnosis includes measles. DISCUSSION: Measles is a highly contagious illness that classically presents with a rash, fever, cough, coryza, and conjunctivitis. Cases in the United States since 2000 have been attributed mainly to travelers who are infected abroad and then spread the il…

Measles: There is No Vaccine against Vaccine Phobia.

In 2000, the United States had effectively eliminated endemic measles. Unfortunately, due to misinformation and non-scientific based concerns, the rate of measles vaccination has declined. The United States is in the midst of its largest outbreak of measles since 2014, with 1,095 confirmed cases as of June 2019. The reasons for the re-emergence of measles and what this epidemic illustrates about the anti-vaccine culture in the United States are explored in this article.
