A 56-year-old woman with acquired, common variable immunodeficiency was found to have persistent gastrointestinal as well as pulmonary infection with Strongyloides stercoralis. Repeated courses of treatment with thiabendazole led to marked reduction or loss of Strongyloides stercoralis larvae, but cessation of treatment always led to recurrence of Strongyloides infection. Several small bowel biopsies showed normal villous architecture and little inflammatory response to presence of larvae. The need to identify improved therapy against cystic echinococcosis (CE) has motivated pharmacology-based research. The comparative pharmacological performances of the benzimidazole compounds flubendazole (FLBZ) and albendazole (ABZ) were addressed here. The goals of the work were as follows: (i) to evaluate the ex vivo activities of FLBZ, ABZ, and their respective metabolites against Echinococcus granulosus protoscoleces, (ii) to compare the plasma and cyst disposition kinetics for the two drugs in infected mice, and (iii) to compare the clinical efficacies of FLBZ and ABZ against CE in mice.

BACKGROUND: Chemotherapy of cystic echinococcosis became a treatment option 25 years ago, when new anthelminthic drugs were introduced. Benzimidazole carbamates were shown to kill the entire metacestode stage of the parasite, and praziquantel exhibited an effect on protoscoleces. Continuous or intermittent treatment with albendazole is recommended for a period of up to 6 months, and praziquantel may enhance the effect, in particular in the case of cyst spillage. OBJECTIVE: Hydatid disease is endemic in many countries throughout the world. Although we do not have exact figures, the disease is prevalent in Turkey. A considerable number of patients have additional liver cysts as well. In this study we reviewed our experience with hydatid disease of the lung and liver and discussed the principles of treatment.

A pharmacokinetic study of flubendazole was carried out, using a radioimmunological method, in patients with hydatid disease about to undergo surgery; there was no evidence of drug penetration into the walls or contents of the cysts. Another study, carried out simultaneously in subjects without hydatid disease, showed very high concentrations of the drug in liver tissue and in bile. These results should not be taken as arguments against the effectiveness of flubendazole, but should encourage the setting up of further randomized pharmacokinetic studies.
