OBJECTIVE: To determine the relationship between axial and peripheral bone density in patients with rheumatoid arthritis (RA) compared to controls. To study risk factors for axial and peripheral bone loss in RA.
METHODS: Twenty women with RA and 20 age matched controls underwent dual energy x-ray absorptiometry (DXA) of the spine and hip and microradioscopy of the hands in order to quantify the combined cortical thickness (CCT) of the 2nd metacarpal. Sharp's method was used to assess the degree of joint space narrowing and erosions.
RESULTS: Compared to controls, the RA group had significant

Seventy-six patients who had sustained Bennett's fractures before 1985 comprised the patient base. Of these, 21 could be contacted and were reviewed at an average of 7 years 7 months after fracture to determine the effect of the quality of reduction on clinical and radiological outcome. Those cases that healed with up to 1 mm of fracture displacement were found to have superior results to cases where the reduction was less accurate. However even in those cases where a good reduction was not achieved the results were generally satisfactory with only one of 76 patients being known to have requir

In a prospective study, 43 patients with 47 metacarpal or phalangeal fractures with significant displacement, rotation, angulation and/or instability were treated with internal fixation using AO minifragment screws and plates. In all fractures an anatomical reduction was achieved. Postoperatively all metacarpal fractures were protected by means of a plaster, in which motion was allowed, whereas all phalangeal fractures were treated by means of a soft bandage. At a review after a mean period of 28 months, 31 patients with 35 fractures out of 34 patients with 38 fractures had regained excellent

At the same time as the paper by P. Vives in July 1981, we proposed, in June 1981, a fixation technique with two perpendicular K-wires for correction of rotated malunions of fractures of the neck of the fifth metacarpal. Longitudinal rotation of the distal fragment, either in pronation or in supination, interferes with thumb-little finger opposition and grip. It cannot always be corrected by orthopaedic reduction and osteosynthesis is sometimes necessary: an intramedullary axial K-wire with a curved tip maintains the proximal fragment and reduces its angulation in flexion. A transverse K-wire,

A prospective study of the management of 30 patients with 37 unstable or complex metacarpal or phalangeal fractures treated with the "Shearer" micro-external fixator was carried out over an 18-month period. The stability achieved, with minimal soft-tissue tethering, allowed early joint mobilisation with good or excellent function in 94% of metacarpal and 85% of phalangeal fractures by nine weeks. There were no cases of non-union and few complications.

The treatment of 64 intra-articular fractures at the base of the fifth metacarpal was studied. 11 fractures with minimal displacement had been immobilised in a plaster cast without reduction. The position was improved in five of 25 fractures treated by closed reduction and a plaster cast, six of nine fractures after percutaneous pinning, and 12 of 19 fractures after open reduction and internal fixation. At follow-up after a median of 4.3 years, 19 of 50 patients answering a questionnaire had intermittent pain, especially on firm grip. 43 patients were re-examined clinically and radiographicall

The authors present a radiological assessment of palmar subluxation of the carpus in rheumatoid disease. The radiological measurements have been taken with reference to the long axis of the radius, the radiological centres of the lunate and the capitate and expressing their reciprocal distances as a ratio which we have termed the radio-carpal and inter-carpal indices. Using a control group of a 100 serial radiographs of normal wrists it was possible to determine the normal indices and their normal range. A subsequent analysis of 100 rheumatoid wrists enabled them to accurately determine the de

Composite wiring techniques using various configurations of Kirschner pins and stainless steel wire sutures have been applied to the treatment of 63 fractures of the long bones of the hand. The secure fixation achieved allowed active motion within 1 week of operation. Thirty-three metacarpal fractures achieved a final mean total active motion of 256 degrees (standard deviation 13.4) (normal total active motion -260 degrees). Twenty-one phalangeal fractures achieved a mean total active motion of 215 degrees (standard deviation, 46 degrees). There were no instances of infection, malunion, nonuni

The treatment and results of a retrospective study are presented on 23 patients who underwent surgical treatment for a fracture of the base of the thumb metacarpal. The 12 patients with a Bennett fracture were not found to be suffering from limitations in their daily activities, work, sport or hobbies. In the majority of cases an anatomical or almost anatomical position was achieved and maintained using closed reduction and extra-articular Kirschner wire fixation. Two of the seven patients with a Rolando fracture were found to have complaints which gave rise to limitations in the above-mention

1. Oxygen consumption, respiratory frequency, and temperatures of the rectum, common carotid artery, external auditory meatus, and skin on the ears, near the distal end of the metacarpus and metatarsus, upper thigh and mid-side of the body, were measured in five rams before, during and after heating the scrotum. Effects on the woolly animal exposed to ambient temperatures of 17-19 degrees C, and on the shorn animal exposed to ambient temperatures of 5.6-24.5 degrees C, were determined.2. In eight experiments on woolly animals scrotal heating resulted in vigorous panting and a lowering of deep …

Twelve patients (eleven males, one female; mean age 36 years) with Bennett's fracture having a gap or a step-off of more than 2 mm and in whom maintenance of the reduced position was difficult were treated by percutaneous leverage pinning. Bone union was obtained in all patients, and the wire was removed 37 days on average after the initial treatment. On the final follow-up examination, mild pain with motion was noted in two patients, and moderate to severe pain was noted in one patient. The reduction of the articular surface of the carpometacarpal joint of the thumb was confirmed by radiograp

This multicenter study analyzes data on metacarpal fractures in 1260 patients to determine optimal treatment concepts. Of the 740 patients followed up, 487 (66%) presented with fractures of a single metacarpal in one of the three most frequently observed locations (distal fractures of the 4th and 5th metacarpal, shaft fractures of the 2nd to 5th metacarpal, and fractures of the base of the first metacarpal). The overall clinical and radiological results were good to excellent. The "path" analysis used to identify baseline parameters and treatment options influencing outcome led to recommendati

We describe a 2-year-old boy with chondrodysplasia punctata (CDP). The boy was exposed to phenytoin, in combination with carbamazepine, during pregnancy. There has been previous evidence for a connection between phenytoin exposure during pregnancy and chondrodysplasia punctata. The boy had clinical and some radiological characteristic features of CDP, of the tibia-metacarpal type. We know of no other report on a child exposed to phenytoin during pregnancy who developed CDP of this type.

Diaphyseal fractures of the metacarpals and phalanges are common injuries that can lead to impairment of hand function. The fracture pattern and soft-tissue injury vary with the mechanism of injury. The imbalance of the flexor and extensor forces created by displaced fractures will often produce a secondary angulatory deformity. Nonoperative treatment is indicated for reducible and stable fracture configurations. Irreducible or unstable fracture patterns require open or closed reduction and fixation. Reduction must be assessed in flexion and extension to ensure correct rotatory alignment. Frac

The radiographs and clinical and surgical histories of 50 skeletally immature patients with histologically verified giant cell tumor (GCT) of the long and short tubular bones were retrospectively studied to determine the prevalence, location within bone (eg, epiphysis, metaphysis), skeletal distribution, radiographic appearance, and pathogenesis of GCTs. Skeletal immaturity was determined radiographically by the presence of open epiphyses. Patients were selected from a group of 876 patients who were seen in consultation with documented GCT of the tubular bones. Approximately 5.7% of all GCTs o

A patient whose cardiac and skeletal malformations are compatible with Holt-Oram syndrome is presented. The interest lays in the fact that cytogenetic shows and excess of chromosomic material in the long arm of the sixth chromosome. Authors consider this to be a "de novo" finding as the study practiced on the parents has been normal. This chromosomic anomaly has not been, to their knowledge, reported in the literature. All known cases have a normal karyotype.

Fusion of selected bones of the carpus, occasionally combined with the radius or metacarpal bases, has been used to treat a wide variety of serious pathologic conditions of the wrist. Indications include severe localized degenerative arthritis and posttraumatic changes secondary to subluxation, dislocation, or residual instability resulting in significant pain. The procedure was performed on 28 wrists in 26 patients. There were three failures to obtain union initially, two of which united after secondary grafting procedures. All patients had less pain and most had functional, though limited, r

Defining replantation as the restoration of a completely amputated part as opposed to simply restoring circulation to an incompletely severed part, the results of replantation of 86 completely amputated parts in 71 patients performed from January, 1970, to December, 1975, were studied. Twenty-eight, or 32.5 percent, were the result of sharp severances of the part; localized crushing accounted for 56, or 65.1 percent. Two were classified as degloving injuries. Twelve amputations were transmetacarpal, six were at the metacarpophalangeal joints, 14 through the proximal phalanx, 15 at the proximal
