Teneurin-3 and latrophilin-2 are required for somatotopic map development and somatosensory topognosis.

Somatotopy is a recurring organizational feature of the somatosensory system where adjacent neurons and their connections represent adjacent regions of the body. The molecular mechanisms governing the formation of such "body maps" remain largely unknown. Here, we demonstrate that the cell surface proteins teneurin-3 and latrophilin-2 are expressed in opposing gradients aligning with the somatotopic map in the dorsal horn of the mouse spinal cord. Genetic manipulation of these proteins in the spinal dorsal horn or sensory neurons distorts the somatotopy of neuronal connections and impairs the a

Biceps tendon properties worsen initially but improve over time following rotator cuff tears in a rat model.

Damage to the biceps tendon is often seen in conjunction with rotator cuff tears. However, controversy exists regarding its role in the shoulder and its optimal treatment. A previous study determined that biceps tendons were detrimentally affected in the presence of rotator cuff tears in the rat model and this damage worsened over time. However, whether this damage progresses at later time points to provide a chronic model is unknown. The objective of this study was to determine the changes in the biceps tendon in the presence of a cuff tear over time. Our hypothesis was that histological, com

Implementation of the PARTNERS model of care within two UK community mental health transformation systems: a qualitative realist evaluation.

INTRODUCTION: PARTNERS is a model of care that was co-designed with service users and carers to address the needs of people with severe mental illness to receive better support in primary care. Taking learning from a randomised controlled trial and process evaluation of this complex person-centred goal and coaching-based approach to care, our aim was to understand better the challenges of implementing PARTNERS in complex dynamic service delivery systems. METHODS: We identified two Integrated Care Systems that were interested in adopting the PARTNERS model of care. We trained practitioners to

Destitute and dying: interventions and models of palliative and end of life care for homeless adults - a systematic review.

BACKGROUND: Homeless adults experience a significant symptom burden when living with a life-limiting illness and nearing the end of life. This increases the inequalities that homeless adults face while coping with a loss of rootedness in the world. There is a lack of palliative and end of life care provision specifically adapted to meet their needs, exacerbating their illness and worsening the quality of their remaining life. AIM: To identify interventions and models of care used to address the palliative and end of life care needs of homeless adults, and to determine their effectiveness. ME

The patient journey: a voyage from diagnosis to hidradenitis suppurativa multidisciplinary unit.

BACKGROUND: Hidradenitis suppurativa (HS) is an inflammatory, recurrent and debilitating chronic skin disease that is often worsened by comorbidities, with a destructive impact on the social and working life of the patient. The low awareness of patients and clinicians regarding HS, together with weak coordination between specialists who manage the clinical care of these patients, may result in a burdening diagnostic and therapeutic delay, lowering efficacy of therapies and worsening prognosis and patients' QoL. OBJECTIVES: To rule out these critical aspects, a HS-Multidisciplinary Unit with a

ProvenCare-Psoriasis: A disease management model to optimize care.

There are a variety of evidence-based treatments available for psoriasis. The transition of this evidence into practice is challenging. In this article, we describe the design of our disease management approach for Psoriasis (ProvenCare®) and present preliminary evidence of the effect of its implementation. In designing our approach, we identified three barriers to optimal care: 1) lack of a standardized and discrete disease activity measure within the electronic health record, 2) lack of a system-wide, standardized approach to care, and 3) non-uniform financial access to appropriate non-pharm

Impact of an acute surgical unit on outcomes in acute cholecystitis.

BACKGROUND: The acute surgical unit (ASU) model has been associated with improved outcomes for emergency general surgical patients. Few Australasian studies have investigated patients with cholecystitis and none from South Australia. METHODS: A retrospective cohort study compared patients admitted to our institution with acute cholecystitis during the 2 years before (traditional period) and after (ASU period) introduction of an ASU on 1 August 2012. Primary outcomes were length of stay, rates of definitive surgery on index admission, time to definitive surgery and proportion of cases performe

[Mobile dementia counseling as low-threshold assistance for caregiving relatives].

BACKGROUND: Comprehensive and easily accessible information and counseling for people with dementia and their caregivers can improve home care in many ways; however, for various reasons the affected persons rarely seek help and advice. When advice is sought the caregiving situation at home is mostly already in a crisis. The Rhine-Erft district, a rural area in Germany, improved its network of dementia care services by establishing a mobile gerontopsychiatric counseling service with a special focus on dementia. A multiprofessional and interdisciplinary team consisting of professional and volunt

[Follow-up Care in the Field of Medical Rehabilitation Concerning Psychosomatic Indications Based on the New Conceptual Framework of the German Pension Insurance].

Mental disorders rank first amongst all causes for disability pensions and second in the field of medical rehabilitation. Especially alarming is the significantly lower age of entry of the mentally ill disability pensioners, compared to those with other indications. Mentally ill people often look back at a long history of diseases before getting in contact with the German pension insurance. In this regard the German pension insurance, which is obligated to effectively support people in order to keep them in working life until reaching the regular retirement age, is facing a big challenge, whic

Economic evaluation of 'Return to Country': A remote Australian initiative to address indigenous homelessness.

BACKGROUND: An increase in the number of Indigenous homeless persons in Cairns, Northern Australia, prompted the Queensland Police Service (QPS) to commence a pilot 'Return to Country' (R2C) program. The program was designed to assist homeless people who were voluntarily seeking to return to their home communities. This study assesses the costs of running the program and evaluates its net economic impact. METHODS: Retrospective uncontrolled cost, cost-effectiveness and cost-offset analyses were undertaken from a societal perspective. All costs were expressed in 2014 AU$. RESULTS: The R2C pro

[FLS - three letters alter secondary fracture prevention].

The increasing importance of preventive measures in the field of orthopedics and trauma surgery becomes apparent because of the demographic changes and the high risk for secondary fractures following osteoporotic fractures. Within the fracture treatment chain, orthopedics and trauma surgery are in the "pole position" to initiate these measures in geriatric patients. In the past orthopedists and trauma surgeons have constantly accused of neglecting secondary fracture prevention in fragility fracture patients. There are several reasons that speak in favor of us undertaking a role in secondary fr

A Platform to Collect Structured Data from Multiple EMRs.

Adoption and use of Electronic Medical Records (EMRs) is continuing to rise across Canada, leading to more data being generated. These data, however, are not being captured in a standardized manner, they are not available for research, surveillance or health system management, and they are not having a real-time impact on healthcare providers at the point of care. Multiple stakeholders, including researchers and system evaluators, require easy access to high quality, structured data. As current EMRs are not able to effectively meet their needs, we engaged multiple stakeholders to assist in des

Time banking and health: the role of a community currency organization in enhancing well-being.

Time banking is an international movement that seeks to transform traditional asymmetric social service models into social networks in which members both provide and receive services that are assigned equal value. Time banks have been shown to enhance social capital, and there is some evidence for improved health. This article, based on a survey of 160 members of a hospital-affiliated time bank, examines the likelihood and predictors of improvement in physical and mental health as a result of membership. Men, people with lower income, and those who were not working full-time reported highest l

An exploration of social support as a factor in the return-to-work process.

Despite evidence that inter-personal relationships are important in human resource management, little is understood about the nature of workplace social support in a disability context, or what features of support are important to the success of return-to-work programs. The purpose of this qualitative study was to explore workplace disability support from worker and supervisory perspectives and to identify salient features for work re-entry. A total of 8 supervisors and 18 previously injured workers from a range of work units in a Canadian municipality were interviewed, and their views concern

Radiology and the Cancer Services Collaborative--an opportunity awaits.

The principles and methodologies used by the Cancer Services Collaborative (CSC) are particularly relevant for radiology departments. A radiology project looking at the provision of barium enema examinations is used to highlight how the principles can be applied to a radiology department. Advice on how to access available CSC literature is offered. The CSC principles and methodologies are an important part of the NHS modernization agenda, and offer an exciting vehicle to improve patient care. It is important that radiologists understand the opportunities offered and the challenges posed by the

The multidisciplinary approach to prevention and treatment of cardiovascular disease: creation of a vascular care network.

Vascular care, diagnosis, and intervention can be very complex. Multiple specialists commonly are involved in dealing with patients with systemic atherosclerotic disease. Although each specialist may provide state-of-the-art care with good result, it would be advantageous to minimize duplication of effort and thereby improve cost efficiency. Patients still could reap the benefit of all these disciplines if evaluated for their vascular complaint in a coordinated system. In doing so, atherosclerosis prevention with risk factor modification, as well as diagnosis and therapy for the presenting pro

Graduate student marriages: an organizational/interactional view.

This article describes an organizational/interactional model for understanding and working with graduate student marriages. The model first distinguishes symmetrical (student/student) from asymmetrical (student/working spouse) marriages. Then, the major focus is on asymmetrical marriages and the disengagement process that is typical among these couples. Asymmetrical organization, especially within today's ethos of equal roles and equal responsibilities, presents graduate student couples with several dilemmas: (a) it promotes hierarchical confusion, (b) it makes for a problematic marital quid p
