[Road accidents in a city in southern Brazil].

The study was conducted in the municipality of Londrina, a medium-sized city of southern Brazil. Analysis was performed in the characteristics of road accidents that had occurred during the first semester of 1996, resulting in 3,643 victims. Car or small truck collisions were the most common type of accident. The rate of motorcycle-related injuries (per 1,000 registered vehicles) was seven times higher than that resulting from cars or small trucks. Fatality rates were higher for motorcyclists who collided with stationary objects (29.4%) and pedestrians who were injured by lorries or buses (22.

Intraoperative rescue extracorporeal membrane oxygenation and damage control during repair of a traumatic aortic injury.

A 21-year-old male involved in a motorcycle crash presented with abdominal hemorrhage and a widened mediastinum on chest radiograph. He required an initial exploratory laparotomy for mesenteric injuries. An aortic injury was confirmed on computed tomography imaging and he subsequently underwent attempted endovascular repair. During that procedure, the patient experienced massive thoracic hemorrhage with cardiac arrest and profound respiratory failure. Damage-control thoracic surgery and rescue extracorporeal membrane oxygenation were successfully utilized with a favorable outcome.

Left Internal Thoracic Artery Graft to Left Anterior Descending Coronary Artery after Blunt-Chest-Trauma Myocardial Infarction: 14-Year Outcome.

In 2005, we reported an acute myocardial infarction secondary to a left anterior descending coronary artery injury sustained in a motorcycle accident. The treatment was late myocardial revascularization with in situ left internal thoracic artery-to-left anterior descending coronary artery anastomosis. There is little information available about the natural history of acute myocardial infarction after blunt chest trauma, especially when treated in this manner. This present communication reports the 14-year outcome in our patient.

Diffuse trauma of the right lobe of the liver successfully treated without resection. Case report.

The victim of a motorcycle frontal crash was treated in a high reference teaching hospital for disruption of the right lobe of the liver (IV grade AAST/Moore). Primary packing, secondary biloma and abscess treatment including papillotomy and biliary stent, led, after 8 months, to cicatrisation of a damaged liver parenchyma. In conclusion, the authors stress that such injuries should be treated in highest level reference centres where all modern noninvasive interventional modalities are available, and that surgical resection is not always the operation of best choice.

Spontaneous fracture of the femoral neck in preexisting avascular necrosis of femoral head in sickle cell disease.

The presence of anterior hip dislocation along with contralateral posterior hip dislocation in the absence of other major traumas is a distinctly rare injury pattern. We report such a case, along with a review of previous cases. A 40-year-old male patient after motorcycle skidding had posterior dislocation of the left hip and anterior dislocation of the right one without other associated injuries. The patient underwent successful closed reduction of both hips. The clinical course and follow-up assessment of the patient was uneventful.

Volar dislocation of the index carpometacarpal joint in association with a Bennett's fracture of the thumb: a rare injury pattern.

We describe a case of volar dislocation of the index carpometacarpal (CMC) joint in association with a Bennett's fracture of the thumb following a motorcycle accident. Volar dislocation of the index carpometacarpal joint is an exceedingly rare but easily missed injury, with only a few reported cases in the literature. This report highlights the importance of a true lateral radiograph and close scrutiny of the film to detect this injury. Closed reduction supplemented with Kirschner wire fixation restored normal anatomical relations and achieved an excellent clinical result.

Incomplete decapitation of a motorcyclist from hyperextension by inertia: a case report.

This is a rare case involving a motorcyclist. A 57 year old motorcycle rider wearing a full face type helmet, suffered incomplete decapitation. The autopsy findings revealed a wide lacerated wound accompanied by extension marks in the front neck, atlanto-occipital dislocation and complete transection of the brainstem. According to the police traffic report, the man's head was stationary at the moment of impact and the remainder of the body continued in a backward motion. We concluded that the inertia of the torso caused hyperextension of the neck and subsequent incomplete decapitation.

Acute myocardial infarction following blunt chest trauma.

A 32-year-old male riding a motorcycle was involved in an accident resulting in blunt torso trauma. He had fractures of the skull, first right rib, and left femoral bone. Electrocardiogram showed acute anteroseptal myocardial infarction (MI). Subsequent coronary angiogram demonstrated laceration at the proximal portion of the left anterior descending coronary artery. Echocardiogram showed depressed left ventricular function as well as hypokinesis over septal and apical areas. His recovery was smooth.

A 50-cm fillet flap for preservation of maximal lower extremity residual limb length.

A 50-cm fillet flap permitted above-knee amputation in lieu of hip disarticulation in a 20-year-old woman who had sustained severe injuries in a motorcycle accident. Although the flap was unusually long, the principles followed were the same as those for posterior flaps used in below-knee amputations and fillet flaps of the digits. The flap allowed preservation of maximal length in the residual limb and fully sensible skin coverage, both of which facilitated prosthetic usage.

[Injury and death in motorcyclists: Umbrella review].

The growing use of motorcycles has improved urban mobility, but has also increased morbidity, mortality, and accident-related costs, especially in low- and middle-income countries. The main consequences include traumatic brain injury, disability, hospital expenditures, and loss of productivity among young adults. The objective was to assess the impact of motorcycle crashes on injuries, healthcare costs, and economic losses through an umbrella review (2000-2025) and to identify the most effective preventive interventions. The review followed PRISMA 2020 guidelines and was registered in PROSPERO…

Helmet Clasp Cracks Larynx? A Case Series and Literature Review.

INTRODUCTION: Helmet use is well known to greatly reduce the risk of head injury in both bicycle and motorcycle riding. However, helmet buckle position may increase the risk of injury to the neck cartilages. We present a series of cases in which thyroid cartilage fracture is presumed to arise from the position of the helmet buckle during a crash. Our objective with this case series is to outline an unusual series of cases and review the literature regarding laryngeal injuries secondary to helmet impact. DESIGN: Single institution case series. MATERIALS AND METHODS: We present 3 adult patient
