Symmetric Lipomatosis Arising in the Tongue Presenting as Macroglossia and Articulatory Disorder.

Symmetric lipomatosis is definitively characterized by symmetric, tumorous lipomatous proliferation of adipose tissue that often develops in the head and neck, shoulders, and upper trunk. However, in the oral region, symmetric lipomatosis of the tongue (SLT) is an extremely rare condition related to generalized lipidosis that is often caused by chronic alcoholism. It is characterized by multiple symmetric lipomatous nodules and diffuse bilateral swelling located within the tongue. We report an extremely rare case of SLT arising in an 80-year-old man with a long history of alcoholic liver cirrh

The pathological mechanisms and potential therapeutic strategies for renal anemia.

Renal anemia (RA) is a common complication associated with chronic kidney diseases. Its pathological mechanism is attributed to the combined effects of insufficient production of erythropoietin by the kidneys, iron metabolism disorders, and chronic inflammatory conditions. The main pillars of medicine treatment for these aspects include erythropoiesis-stimulating agents, iron supplementation, and novel hypoxia-inducible factor prolyl hydroxylase inhibitors. Traditional Chinese medicine shows potential advantages across multiple targets in improving anemia and renal function in patients with RA

Clonal hematopoiesis of indeterminate potential: a multisystem hub bridging hematopoietic dysfunction with non-hematopoietic diseases.

Clonal hematopoiesis of indeterminate potential (CHIP), driven by leukemia-related somatic mutations in hematopoietic stem cells, previously recognized as a major risk factor for hematological malignancies, has now emerged as a potent risk factor for chronic inflammation and diverse non-hematologic diseases. CHIP-associated DNA methyltransferase 3 alpha (DNMT3A), tet methylcytosine dioxygenase 2 (TET2), and additional sex combs like 1 (ASXL1) mutations alter epigenetic programs, skew myelopoiesis, and increase proinflammatory cytokines, resulting in chronic inflammation and immune imbalance. Takayasu Arteritis with Fistulizing Crohn's Disease: A Rare Presentation.

Crohn's disease (CD) is a chronic, recurrent, transmural inflammatory disease with lesions anywhere in the gastrointestinal (GI) tract. Takayasu arteritis (TA) is an idiopathic, chronic, granulomatous inflammatory panarteritis that involves the aorta and its branches, known as "pulseless disease." Crohn's disease and TA are both associated, as both are granulomatous disorders, and multiple cases of simultaneous occurrence of both diseases have been reported. Here we are reporting a rare case of simultaneous CD and TA in a young female who had a large enteroenteric fistula between the colon and

Functional neurological disorder, physical activity and exercise: What we know and what we can learn from comorbid disorders.

Functional neurological disorder (FND) is a common neurologic disorder associated with many comorbid symptoms including fatigue, pain, headache, and orthostasis. These concurrent symptoms lead patients to accumulate multiple diagnoses comorbid with FND, including fibromyalgia, chronic fatigue syndrome, postural orthostatic tachycardia syndrome, persistent post-concussive symptoms, and chronic pain. The role of physical activity and exercise has not been evaluated in FND populations, though has been studied in certain comorbid conditions. In this traditional narrative literature review, we high

Care of People Experiencing Homelessness.

Homelessness affects more than 580,000 Americans on any given night. Risk factors for homelessness include extreme poverty, substance use, and mental illness. People experiencing homelessness are likely to have multiple chronic medical or mental health conditions. Homelessness increases morbidity associated with cardiovascular, respiratory, and infectious diseases and all-cause mortality. A trauma-informed approach to the examination of people experiencing homelessness is imperative because previous exposure to physical or sexual trauma is common in this population, especially among women. Use of health services by chronically ill and disabled children.

Hospitalization and use of outpatient health care services during a 1-year period by 369 pediatric patients with cystic fibrosis, cerebral palsy, myelodysplasia, or multiple physical handicaps and 456 randomly selected children without congenital conditions from the Cleveland area were examined. Use of hospitalization and outpatient services by the average chronically ill or disabled child was 10 times that of the average comparison child. Physician specialists, occupational and physical therapists, and school nurses were the major outpatient categories used disproportionately by children with

Misperception of Body Weight After Childhood Cancer.

BACKGROUND: Misperception of body weight can negatively impact the weight management efforts of childhood cancer survivors (CCSs). Both being overweight or underweight are associated with chronic health conditions commonly observed in CCS; therefore, accurate weight perception is critical for reducing long-term health risks. AIM: To assess the agreement between objectively measured body mass index (BMI) and self-perceived body weight in adult CCS, and to identify predictors of BMI underestimation and overestimation. METHODS: In this prospective, multicenter cohort study, we invited CCS aged

Multisystemic eosinophilic epitheliotropic disease in three donkeys.

Multisystemic eosinophilic epitheliotropic disease (MEED) is a rare condition of equids characterized by eosinophilic infiltration of multiple organs. Clinical signs are variable depending on the affected organs. The most common clinical signs include chronic weight loss, diarrhoea and exfoliative dermatitis. Respiratory distress and raised liver enzymes are less frequently seen. The cause is unknown and the pathogenesis is poorly understood. There are less than 50 reported cases of horses with MEED. We now document the lesions in three donkeys with fluctuating or chronic loss of weight, letha

Cutaneous botryomycosis in a free-living short-beaked echidna (Tachyglossus aculeatus).

Botryomycosis is a chronic bacterial infection of the skin and viscera caused by nonbranching bacteria such as Staphylococcus spp. It results in the formation of chronic, suppurative and often granulomatous lesions that may resemble multiple abscesses. It has been diagnosed worldwide in both humans and animals. It is a rare disease, usually requiring surgical excision combined with long-term antibiotics to achieve a good resolution. A wild echidna (Tachyglossus aculeatus) was presented with a soft tissue mass at the base of the right distal beak with multiple discharging fistulas arising from

The role of transcutaneous auricular vagus nerve stimulation in chronic pain: from neurobiological mechanisms to clinical applications.

Chronic pain is a prevalent health issue with high disability rates, and traditional pharmacological treatments often come with limitations such as dependency and side effects. Transcutaneous auricular vagus nerve stimulation (taVNS), as an emerging non-invasive neuromodulation technique, has demonstrated broad application prospects in chronic pain management in recent years. This systematic review examines the clinical efficacy of taVNS across multiple chronic pain conditions, including neuropathic pain, autoimmune disease-related pain, gastrointestinal pain, and musculoskeletal pain. It also

Insights into Personalised Medicine in Bronchiectasis.

Bronchiectasis is a heterogenous disease with multiple aetiologies resulting in inflammation and dilatation of the airways with associated mucus production and chronic respiratory infection. The condition is being recognised ever more frequently as the availability of computed tomography increases. It is associated with significant morbidity and healthcare-related costs. With new understanding of the disease process, varying endotypes, identification of underlying causes and treatable traits, the management of bronchiectasis can be increasingly personalised.

Gastrointestinal malignancies as a complication of chronic myeloid leukaemia.

A 70-year-old patient is presented who, after chemotherapy for chronic myeloid leukaemia, developed two separate gastrointestinal malignancies. His family history indicated a strong family tendency towards neoplasia, and it is speculated that this predisposition may have been potentiated by the chemotherapy or the leukaemic process itself. It is stressed that, in such cases of multiple malignancy, each condition should be treated on its own merits.
