Recurrence of acute colonic pseudo-obstruction in selective adrenergic dysautonomia associated with infectious toxoplasmosis.

BACKGROUND: Acute colonic pseudo-obstruction is a life-threatening condition associated with several pathologic conditions, whose pathophysiology is still uncertain. CASE: Autonomic function in a young patient operated on for acute colonic pseudo-obstruction was carefully evaluated; none of the common clinical conditions described in the literature was found to have caused the syndrome. Selective adrenergic failure was suggested by the presence of severe orthostatic hypotension, low basal plasma catecholamine level, and absence of the expected increase on standing and by the findings of provo

Sarcopenia or muscle modifications in neurologic diseases: a lexical or patophysiological difference?

Sarcopenia is a condition characterized by a decrease in muscle mass and function (strength and mobility) that is frequently observed in the elderly. In people with paresis and altered mobility due to central nervous system (CNS) diseases, this definition then may not be applicable. In CNS diseases, mainly stroke and spinal cord injury, different and specific patterns of muscle loss and muscle changes have been described, due to denervation, disuse atrophy, spasticity and myosteatosis. The main observations available about these phenomena in CNS diseases are reviewed, and a broad view on the s

Muscular pathology in equine laryngeal neuropathy.

Three adductor muscles of the larynx, the cricoarytenoideus lateralis (CAL), the arytenoideus transversus (AT) and the ventricularis (Ve), from 36 horses were examined histologically. The neurogenic changes seen in each muscle were evaluated qualitatively. In addition, in 6 horses with clinical and subclinical signs of neurogenic atrophy, measurements of muscle fibre area were performed. Neurogenic changes observed in the Ve were less than in CAL and AT. Measurements of muscle fibre area also demonstrated that CAL and AT showed a wider range of pathological changes than did Ve. The results sho

Monitoring Progression of Amyotrophic Lateral Sclerosis Using Ultrasound Morpho-Textural Muscle Biomarkers: A Pilot Study.

The need is increasing for progression biomarkers that allow the loss of motor neurons in amyotrophic lateral sclerosis (ALS) to be monitored in clinical trials. In this prospective longitudinal study, muscle thickness, echointensity, echovariation and gray level co-occurrence matrix textural features are examined as possible progression ultrasound biomarkers in ALS patients during a 5-mo follow-up period. We subjected 13 patients to 3 measurements for 20 wk. They showed a significant loss of muscle, an evident tendency to loss of thickness and increased echointensity and echovariation. In reg

Fiber density in congenital muscle fiber type disproportion. I. Congenital myopathies.

In 15 cases with congenital muscle fiber type disproportion, concentric needle EMG (CNEMG) and single fiber EMG (SFEMG) with fiber density (FD) estimation was performed. The aim of the work was to establish the nature of congenital muscle fiber type disproportion. In 11 cases CNEMG revealed interference pattern on maximal effort and low, polyphasic motor unit action potentials (MUAPs) of short duration indicating a myopathic lesion. In four other cases electromyographic changes were less evident. The FD values were slightly increased in 9 cases. This finding could be explained by pronounced mu

[Association of post-radiation focal muscular atrophy and hypertrophy].

We report a 48 year old woman who had radiotherapy for uterine carcinoma and who developed amyotrophy and muscle hypertrophy in one lower limb. Very few cases of post-radiation monomelic amyotrophy have been reported. On the other hand denervation hypertrophy was presumed to be well known. The seat of the lesions was presumed to be radicular and spinal. The mechanism of atrophy and hypertrophy is discussed.

Anterior shoulder instability: histomorphometric study of the subscapularis and deltoid muscles.

Recurrent traumatic anterior shoulder dislocation results in soft tissues lesions around the glenohumeral joint. The subscapularis muscle is a major active stabilizer of the shoulder and the hypothesis of the current study is that one would expect pathologic changes within its substance secondary to the trauma. A histomorphometric study of the subscapularis muscle was done of 52 patients operated on for recurrent traumatic anterior shoulder dislocation. At the time of surgery biopsy specimens were taken of the subscapularis muscle and the ipsilateral deltoid muscle as a comparison and to see i

Human muscle proteins: analysis by two-dimensional electrophoresis.

Proteins from single frozen sections of human muscle were separated by two-dimensional gel electrophoresis and detected by fluorography or Coomassie Blue staining. The major proteins were identical in different normal muscles obtained from either sex at different ages, and in Duchenne and myotonic dystrophy samples. Congenital myopathy, denervation atrophy, polymyositis, and Becker's muscular dystrophy samples, however, showed abnormal myosin light chain compositions, some with a decrease of fast-fiber myosin light chains and others with a decrease of slow-fiber light chains. These protein alt

Restoration of diaphragmatic function after diaphragm reinnervation by inferior laryngeal nerve; experimental study in rabbits.

OBJECTIVES: To assess the possibilities of reinnervation in a paralyzed hemidiaphragm via an anastomosis between phrenic nerve and inferior laryngeal nerve in rabbits. Reinnervation of a paralyzed diaphragm could be an alternative to treat patients with ventilatory insufficiency due to upper cervical spine injuries. MATERIAL AND METHOD: Rabbits were divided into five groups of seven rabbits each. Groups I and II were respectively the healthy and the denervated control groups. The 3 other groups were all reinnervated using three different surgical procedures. In groups III and IV, phrenic nerv

Verapamil substantially increases the chemomyectomy effect of doxorubicin injected into rabbit or monkey eyelid.

Local doxorubicin injections have been used clinically to treat blepharospasm, hemifacial spasm, and other related disorders permanently and nonsurgically. Doxorubicin is an effective myotoxic agent for the removal of the orbicularis oculi muscle in the eyelid after local injection. Injections of this drug alone resulted in removal of up to 70% of the muscle fibers from the treated eyelids in monkeys. The authors attempted to optimize the conditions for doxorubicin myotoxicity of the orbicularis oculi. Doxorubicin was injected shortly after local verapamil injection in rabbits and a monkey in

Respiratory muscle activity during asphyxic apnoea and opisthotonus in the rabbit.

The behaviour of submandibular, cervical, thoracic and abdominal respiratory muscles was examined in the pentobarbitone-urethane-anaesthetized rabbit during progressive asphyxia induced by rebreathing. During asphyxic hyperpnoea the external intercostal, interchondral and scalene inspiratory activities augmented until succeeded by the apnoeic period, in which all were inhibited with the diaphragm. Likewise, the genioglossus, sternohyoid and thyrohyoid muscles exhibited inspiratory augmentation during asphyxic hyperpnoea until the onset of apnoeic inhibition. However, late in the apnoea these m

Electrophysiology and electrodiagnosis of the post-polio motor unit.

Post-poliomyelitis syndrome refers to new symptoms that may occur years after recovery from poliomyelitis. The most common of these symptoms are new weakness, fatigue, and pain. This article describes electrodiagnostic studies--conventional electromyography (EMG), single fiber electromyography (SFEMG), and macroelectromyography (macro-EMG)--that have provided information on the post-polio motor unit and on the possible etiology of some post-polio syndrome symptoms. Muscular fatigue, and indirectly, general fatigue, may be due to neuromuscular junction transmission defects in some post-polio in

The cellular events of injured muscle regeneration depend on the nature of the injury.

The cellular events of muscle degeneration and regeneration and their time course were studied in two experimental models of muscle injury mice; (i) the denervation-devascularization (DD) of the extensor digitorum longus (EDL) muscle, which is an ischaemic lesion; (ii) the injection of notexin (NOT), a snake venom, in the tibialis anterior (TA) muscle, resulting in a toxic lesion. Compared to the ischaemic lesion, the toxic lesion was characterized by a more extensive inflammatory infiltrate and a shortened phase of phagocytosis of the damaged myofibres. This allowed the proliferation and diff

Surgical Management of Spasticity of the Shoulder.

Although spastic conditions often involve the shoulder, it is rare for surgical intervention to be required. In cases in which chemodenervation and therapy are insufficient to optimize the patient's function or minimize their care requirements, surgical options, such as tendon and joint releases, can be considered. Tendon transfers are rarely indicated. Nerve transfers, particularly contralateral C7, may play a larger role in the future as we gain further understanding into the risks, indications, and contraindications of this exciting technique.

Management of Spinal Cord Injury-Induced Upper Extremity Spasticity.

Spasticity affects more than 80% of patients with spinal cord injury. Neural mechanisms and musculotendinous alterations lead to typical upper extremity features including shoulder adduction/internal rotation, forearm pronation, and elbow, wrist, and finger flexion. Long-standing spasticity may lead to soft tissue and joint contractures and further impairment of upper extremity function. Surgical management involves tendon lengthening, release, and transfer, as well as selective neurotomy, in an effort to reduce spastic muscle hypertonicity, restore balance, prevent further contracture, and im

MR imaging of muscles of mastication.

High-field MR imaging was used to study structural and physiologic alterations involving the muscles of mastication in 46 patients. Muscular abnormalities were often detected incidentally in conjunction with lesions of the CNS, cranial nerves, facial bones, and/or temporomandibular joint (TMJ). Specific pathologic alterations observed included anomalies of musculoskeletal development, muscle hypertrophy, atrophy (disuse and denervation), inflammatory disorders, injuries (including contusions, tears, and muscle rupture), posttraumatic musculoskeletal deformities, and reflex sympathetic dystroph

Spastic dysphonia and denervation signs in a young man with tardive dyskinesia.

Pathophysiological theories of tardive dyskinesia (TD) suggest the possibility of structural changes in the central nervous system of patients with TD. This report describes a case of choreoathetoid dyskinesia and spastic dysphonia associated with clinical and electromyographic signs of muscle denervation. The findings of this case suggest that the neurological syndrome originates within basal ganglia nuclei but may also extend to the peripheral neuromuscular system.

Ileal absorptive adaptation to jejunal resection and extrinsic denervation: implications for living-related small bowel transplantation.

Net absorption of water, electrolytes, and simple nutrients decreases early after jejunoileal autotransplantation (extrinsic denervation) in a canine model but recovers toward normal by 8 weeks. However, the ability of the extrinsically denervated ileum to adapt after total jejunectomy, which would be relevant as a model of segmental small bowel transplantation, remains unknown. Two groups of five dogs each were studied before and 2 weeks and 12 weeks after 50% proximal enterectomy. A control group remained neurally intact, whereas the other group underwent extrinsic denervation (Ext Den) of t

[Morphological study of muscle fibers stained red by modified Gomori trichrome staining with special reference to smooth red fibers].

The modified Gomori trichrome stain of muscles can demonstrate ragged red fibers which are irregular in outline and display a thick and irregular red subsarcolemmal layer and intermyofibrillar red deposits. Typical ragged red fibers are often encountered in mitochondrial myopathy. On the other hand, we have noticed fibers outlined by a thin red subsarcolemmal layer. These fibers are smooth in outline. The sarcoplasm shows normal intermyofibrillar network. We defined these fibers as "smooth red fibers". To investigate the pathological significance of the smooth red fibers, we studied morphologi
