Growing into gender: Funerary practice of boys and girls in the Bronze Age Maros culture (2100 - 1700 BC).

BACKGROUND: Identification and preservation of the recurrent laryngeal nerve (RLN) is of major concern in surgery of the thyroid gland. The purpose of this study was to review the surgical anatomy of the nerve and to describe its relationship to other important structures. METHODS: A total of 325 patients were accrued in this prospective non-randomized study from January 1999 to December 2000. All patients who had total, subtotal and hemithyroidectomies were included in this study. Each side of the thyroid gland was considered as a separate unit in the analysis of the results. RESULTS: Two h

The Heart of the Thymus: A Rare Tale of Nonischemic Cardiomyopathy in the Absence of Myasthenia Gravis.

Thymomas are rare epithelial tumors of the anterior mediastinum known for their association with autoimmune and paraneoplastic syndromes, most notably myasthenia gravis (MG). Cardiomyopathy is an exceptionally rare paraneoplastic manifestation. Emerging case reports suggest that thymoma may exert systemic effects independent of MG, including direct or immune-mediated myocardial injury. We report the case of a 54-year-old male with known nonischemic cardiomyopathy who presented with septic shock from pneumonia and cholecystitis, complicated by ventricular arrhythmias and decompensated heart fai…

Sodium-Glucose Linked Transporter-2 (SGLT-2) Inhibitor-Associated Unmasking of Myasthenia Gravis in a Patient With Undiagnosed Thymic Follicular Hyperplasia: The First Reported Case.

Myasthenia gravis is an autoimmune disease of the neuromuscular junction, primarily caused by antibodies directed against the acetylcholine receptor. Follicular hyperplasia of the thymus can be seen in patients with acetylcholine receptor-positive myasthenia gravis and may remain asymptomatic for many years until unmasked by a specific medication. Myasthenia gravis induced by drugs has been reported with D-penicillamine and immune checkpoint inhibitors, but never with the sodium-glucose linked transporter-2 inhibitors. We report a case of a 62-year-old man with type 2 diabetes presenting with …

Rate of Autoimmune Diseases in the 10 Years Preceding Myasthenia Gravis Diagnosis: A Nationwide Cohort Study in South Korea.

BACKGROUND AND OBJECTIVES: Patients with myasthenia gravis (MG) are frequently associated with other autoimmune diseases. However, the temporal relationship between autoimmune diseases and MG remains unclear. This study aimed to evaluate the rates and temporal patterns of autoimmune diseases during the 10 years preceding MG diagnosis. METHODS: This retrospective population-based cohort study analyzed claims data from the Korean National Health Insurance Service between 2010 and 2021 and included individuals aged ≥20 years. MG was defined using the International Classification of Diseases, 10t…

Isolated pulmonary Cryptococcus neoformans infection presenting with severe pulmonary involvement: a case report.

BACKGROUND: Pulmonary cryptococcosis is an uncommon fungal infection typically seen in immunocompromised hosts. In individuals with only mild immunosuppression, the disease is rare, often mimicking bacterial pneumonia, leading to diagnostic delays and inappropriate treatment. CASE PRESENTATION: We report a 50-year-old Chinese man with poorly controlled diabetes mellitus and myasthenia gravis on low-dose corticosteroids who presented with fever, cough, and hypoxemic respiratory failure. Initial imaging suggested severe community-acquired pneumonia, and broad-spectrum antibiotics were administe…

Pseudoptosis due to acute dacryoadenitis: An unusual presentation of Epstein-Barr virus infection in a child.

Epstein-Barr virus (EBV), a common human herpesvirus, usually causes a self-limited illness in children, manifested by fever, pharyngitis, and lymphadenopathy. However, patients may have atypical presentations including rare ocular manifestations like uveitis, dacryoadenitis, ptosis, or pseudoptosis. In this report, we describe EBV presenting as pseudoptosis in a previously healthy 7-year-old boy. The child was initially worked up as having possible myasthenia gravis with neurologic and ophthalmologic consultations. Ultimately, he was diagnosed with viral dacryoadenitis due to acute EBV infect…

Case Report: Thymoma-associated myasthenia gravis, myositis, myocarditis, and anti-GAD65 autoimmune encephalitis: a unique case of paraneoplastic polyautoimmunity.

Neurological and autoimmune muscle comorbidities are rare in thymoma-associated myasthenia gravis (TAMG). The incidence of myositis is likely underestimated due to its clinical similarity. Few cases of autoimmune encephalitis (AE) have been reported in TAMG, most of which are associated with neuronal surface antibodies. We present the case of a 41-year-old woman with weakness and bulbar dysfunction, and elevated muscle and cardiac enzyme levels, who developed seizures and a decreased level of consciousness. Among the complementary tests, electromyography revealed a postsynaptic neuromuscular j…

Intrathecal morphine pump associated Trichosporon infection complicated by new-onset myasthenia gravis.

Fungal infections in immunocompetent patients-even those with intrathecal drug delivery pumps (ITDDPs)-are exceedingly rare. This is the first report of Trichosporon arachnoiditis in a patient with an ITDDP. The patient presented with falls and progressive lower extremity weakness. Imaging showed a compressive lesion, for which the patient was taken for exploration and decompression. Cultures grew Trichosporon and pathology showed fungus consistent with Aspergillus The patient's recovery was complicated by respiratory failure requiring re-intubation secondary to an exacerbation of myasthenia g…

Case Report: Dual response to efgartigimod in myasthenia gravis and comorbid autoimmune disorders: a case series.

The coexistence of myasthenia gravis (MG) with other IgG-mediated autoimmune diseases represents a clinically heterogeneous condition, driven by pathogenic autoantibodies. Efgartigimod, a neonatal Fc receptor antagonist, has the ability to clear all IgG subclasses. Here, we report four cases of MG concomitant with IgG-mediated autoimmune diseases, specifically idiopathic inflammatory myopathies (IIM), Isaacs syndrome, autoimmune encephalitis (AE), and IgG4-related disease (IgG4-RD). All of them demonstrated a favorable therapeutic response to efgartigimod, with marked improvement in QMG and MG…

An Exceptional Preaortic Ectopic Type B2 Thymoma as the Cause of Refractory Myasthenia Gravis: A Case Report.

Myasthenia gravis (MG) is a chronic autoimmune neuromuscular disorder mediated by autoantibodies directed against acetylcholine receptors (anti-AChRs), in which thymic pathology plays a central pathogenic role. While thymomas predominantly arise in the anterior mediastinum, ectopic locations represent a rare and clinically distinct entity. Among these, preaortic ectopic thymomas are exceedingly rare, and their association with refractory MG has not previously been described. We report a 48-year-old woman with generalized MG with bulbar predominance, presenting with an eight-month history, conf…

Successful treatment of anti-LGI1-associated autoimmune encephalitis with efgartigimod: A case report and literature review.

RATIONALE: Anti leucine rich glioma inactivated 1 (LGI1)- associated autoimmune encephalitis (AE) is the common form of AE, The treatments for AE may cause many adverse effects and some treatments rely on limited blood resources. Monoclonal antibodies directed against neonatal crystallizable fragment receptor (FcRn), efgartigimod, accelerate autoantibody degradation by blocking IgG recirculation. The efficacy of efgartigimod has been demonstrated in myasthenia gravis. However, there are few case reports on the treatment of anti-LGI1-associated AE with efgartigimod. In this study, we present th…

Case Report: Positioning head tilt observed in six dogs with meningoencephalitis of unknown origin.

Positioning head tilt (PHT) is a dynamic clinical neurological sign that is characterized by a head tilt to the opposite side of a voluntary … aratus. As examples of the former, it has been observed in feline cases of hypokalemic myopathy and myasthenia gravis. As an example of the latter, it has been observed in a dog and four cats with bilateral peripheral vestibular dysfunction. In this study, we describe and discuss our observations of PHT in six dogs with meningoencephalitis of unknown …

Refractory Pembrolizumab-Induced Myocarditis-Myositis-Myasthenia Gravis Overlap Syndrome With Concurrent Hepatitis and Thyroiditis in Endometrial Carcinoma: A Case Report.

Immune checkpoint inhibitors (ICIs) have transformed oncologic care but can trigger a spectrum of immune-related adverse events. The triad of myocarditis, myositis, and myasthenia gravis, termed MMM overlap syndrome, is a rare but life-threatening complication. Reports have predominantly involved melanoma and lung cancer; cases of endometrial carcinoma have rarely been described. A 77-year-old woman with stage IV serous endometrial carcinoma presented 24 days after a single dose of carboplatin, paclitaxel, and pembrolizumab, with progressive weakness, ptosis, diplopia, and respiratory compromi…

Associated Autoimmunity in Myasthenia Gravis in Denmark: A Nationwide Case-Control Study.

BACKGROUND: Myasthenia gravis (MG) is an autoantibody-mediated neuromuscular disease often co-occurring with other autoimmune diseases (ADs). We aimed to determine the temporal relationship between other ADs and MG. METHODS: We conducted a nationwide, population-based case-control study of MG patients and 10 age-, sex-, and index date-matched controls from 1985 to 2020. We used conditional logistic regression to estimate odds ratios (ORs) for AD diagnoses preceding MG, and Cox regression to calculate hazard ratios (HRs) for ADs succeeding MG. Analyses were stratified by sex, age group (≤ 50 a…

Efficacy of efgartigimod in the treatment of autoimmune encephalitis: a systematic review.

BACKGROUND: Autoimmune encephalitis (AE) encompasses a group of inflammatory disorders of the brain parenchyma mediated by autoimmune mechanisms. Currently, first-line immunotherapy for AE typically involves corticosteroids, intravenous immunoglobulin, and plasma exchange or immune adsorption. However, a subset of patients shows an inadequate response to these treatments, which are also associated with an increased risk of infection and other potential adverse effects. Efgartigimod, a neonatal Fc receptor (FcRn) antagonist, is primarily approved for treating generalized myasthenia gravis. Prel…
