[Transesophageal echocardiography (Part 3): Clinical application Part 1].

Intraoperative transesophageal echocardiography (TEE) has a wide range of applications including monitoring of left ventricular (LV) function, evaluation of natural and prosthetic valves, detection of intracardiac thrombus, mass and vegetation, assessment of surgical repair of cardiac disease, visualization of intracardiac air, positioning of an intraaortic balloon and a coronary sinus catheter, evaluation of thoracic aortic lesions, and so on. Complications are very rare with TEE, but damage of the esophagus or stomach can occur. Gentle manipulation of the TEE probe is always required. Patien

Natural history of contractile abnormalities after acute myocardial infarction in man: severity and response to nitroglycerin as a function of time.

The natural history of contraction abnormalities and their response after acute myocardial infarction in man were studied using radionuclide angiography. Sixteen patients were studied before and after sublingual nitroglycerin within 24 hours, 5-7 days and 4-6 weeks after the onset of chest pain. Within 24 hours, central chord shortening in the zone of infarction was reduced to 13.1 +/- 9.8%, but improved 27.2 +/- 18.4% (p less than 0.001) after nitroglycerin. After 5-7 days, central chord shortening improved similarly, but less markedly, from 12.9 +/- 9.2% to 24.4 +/- 13.2% (p less than 0.001)

Transmural distribution of metabolites and blood flow in the canine left ventricle following isoproterenol infusions.

The effects of intravenous infusions of isoproterenol (0.1, 1.0 or 2.5 micrograms/kg/min for 1 hr) on the transmural distribution of myocardial high-energy phosphates and glycolytic intermediates were determined in anesthetized, open chested dogs. The transmural distribution of blood flow across the left ventricular wall was determined after serial injections of 15 mu radioactively labeled microspheres. A biopsy of the posterolateral wall was frozen in liquid nitrogen, divided into epicardial, midmyocardial and endocardial thirds, and assayed for metabolites. The remainder of the heart was pro

[Phonocardiographical studies in hypertensive subjects (author's transl)].

Phonocardiographical (PCG) studies were made to evaluate the relation of the PCG to blood pressure, ECG, chest X-ray finding and changes of fundi in hypertensive subjects. One hundred and thirty hypertensive patients and sixty normal adults were examined. Results were as follows: 1) There were abnormal second heart sounds such as, narrow splitting or ringing in most cases of hypertensive subjects with high both systolic and diastolic blood pressure. These phenomena seemed to be delay and accentuation of aortic component. 2) Ejection murmur was found in majority of constantly hypertensive cases

Stroke volume analog on a chip - in vitro hydrodynamic model of cardiac pumping efficiency.

Stroke volume serves as a key metric of heart performance. Despite its central importance in diagnosing many cardiomyopathies, there exists limited in vitro models whose readouts can be directly mapped to this clinical indicator. To address this, as a simplified model of the heart's left-ventricle, here we use two-dimensional muscular thin films (2D-MTFs) as an assay for monitoring cardiac fluid-dynamics. Composed of anisotropic cardiac microtissues that recapitulate the laminar architecture of the ventricular myocardium, we studied how differing tissue alignments can be used to form small-sca

Metabolite-sensitive cross-bridge models of human atria reveal the impact of diabetes on muscle mechanoenergetics.

Type 2 diabetes is associated with a range of adverse health outcomes, including metabolic dysfunction and increased risk of heart failure. Although the interactions between diabetes and heart disease are complex and incompletely understood, they can be more effectively investigated using multiscale, biophysically based models of the underlying physiological processes. In this study, experimental data from non-diabetic and diabetic human atrial muscles were used to develop metabolite-sensitive cross-bridge models representing each group. The parameterisation of these cross-bridge models reveal

Interatrial Features of Length-Dependent Regulation of Myocardial Force in Paroxysmal Atrial Fibrillation in Rats.

Myocardial stretch is a factor that forms an arrhythmogenic substrate for the progression of atrial fibrillation (AF). The features of force generation remodeling during stretching in the atrial myocardium in AF remain poorly understood. In this study, we analyzed changes in active and passive tension of left and right rat atrial myocardial strips at their diastolic stretching and the slopes of the "length-tension" relationship in acetylcholine-CaCl2-induced paroxysmal AF. In AF, neither magnitudes of the active and passive tension, nor the slope coefficients of the "length-tension" relationsh

Acute effects of β-hydroxybutyrate on left ventricular function in young, healthy adults.

Interest in ketones as a cardiac "super fuel" has grown significantly following reports of a marked increase in cardiac output after exogenous ketone administration in heart failure. However, the extent to which this increase in cardiac output is related to changes in cardiac contractility, and dependent on the presence of heart failure, remains incompletely understood. Therefore, we performed a randomized, double-blind, placebo-controlled study of oral ketone ester in young healthy volunteers. Baseline cardiac magnetic resonance imaging was performed and repeated every 15 min for 60 min after

Triiodothyronine improves contractile recovery of human atrial trabeculae after hypoxia/reoxygenation.

BACKGROUND: In patients undergoing interventional or surgical coronary revascularization, subclinical hypothyroidism is common and associated with worse outcome, including the need for postoperative inotropic support. In isolated rat hearts with global ischemia/reperfusion, exogenous triiodothyronine (T3) reduces infarct size. Aim of this study was, to investigate whether or not exogenous T3 protects human myocardium from ischemia/reperfusion injury. METHODS: Right atrial trabeculae from patients undergoing routine coronary artery bypass grafting were isolated and transferred to Tyrode's buff

Contractility analysis of human engineered 3D heart tissues by an automatic tracking technique using a standalone application.

The use of Engineered Heart Tissues (EHT) as in vitro model for disease modeling and drug screening has increased, as they provide important insight into the genetic mechanisms, cardiac toxicity or drug responses. Consequently, this has highlighted the need for a standardized, unbiased, robust and automatic way to analyze hallmark physiological features of EHTs. In this study we described and validated a standalone application to analyze physiological features of EHTs in an automatic, robust, and unbiased way, using low computational time. The standalone application "EHT Analysis" contains two

Mastering Mitral Leaflets Coaptation After Valve Repair with Adjustable Mitral Annuloplasty Ring: Proof of Concept in Mock Loop Study.

This investigation sought to determine the feasibility of a novel mitral ring designed to reshape mitral annulus on beating heart, after surgery. The mitral ring is intended to improve mitral leaflets coaptation to correct residual and recurrent mitral regurgitations. It could also provide progressive correction of mitral regurgitation. The device was tested in ex vivo beating heart model. The novel mitral ring is selectively deformable in P1, P2, and P3 segments using a dedicated angioplasty-type balloon. The deformation should increase leaflets coaptation, reducing distance between the two l

Abnormal contractility in human heart myofibrils from patients with dilated cardiomyopathy due to mutations in TTN and contractile protein genes.

Dilated cardiomyopathy (DCM) is an important cause of heart failure. Single gene mutations in at least 50 genes have been proposed to account for 25-50% of DCM cases and up to 25% of inherited DCM has been attributed to truncating mutations in the sarcomeric structural protein titin (TTNtv). Whilst the primary molecular mechanism of some DCM-associated mutations in the contractile apparatus has been studied in vitro and in transgenic mice, the contractile defect in human heart muscle has not been studied. In this study we isolated cardiac myofibrils from 3 TTNtv mutants, and 3 with contractile

Redox signaling in cardiac physiology and pathology.

Redox signaling refers to the specific and usually reversible oxidation/reduction modification of molecules involved in cellular signaling pathways. In the heart, redox signaling regulates several physiological processes (eg, excitation-contraction coupling) and is involved in a wide variety of pathophysiological and homoeostatic or stress response pathways. Reactive oxygen species involved in cardiac redox signaling may derive from many sources, but NADPH oxidases, as dedicated sources of signaling reactive oxygen species, seem to be especially important. An increasing number of specific post

Small changes can make a big difference - microRNA regulation of cardiac hypertrophy.

Cardiac hypertrophy is a thickening of the heart muscle that results in enlargement of the ventricles, which is the primary response of the myocardium to stress or mechanical overload. Cardiac pathological and physiological hemodynamic overload causes enhanced protein synthesis, sarcomeric reorganization and density, and increased cardiomyocyte size, all culminating into structural remodeling of the heart. With clinical evidence demonstrating that sustained hypertrophy is a key risk factor in heart failure development, much effort is centered on the identification of signals and pathways leadi

Model of depressed myocardium shows orthostatic intolerance with or without reduced blood volume.

INTRODUCTION: The development of orthostatic hypotension (OH) is complex and multi-factorial. Previous simulation work indicates that myocardial contractility depression (MCD) may increase OH when there is a total blood volume decrease. This paper hypothesized that MCD increased OH in both humans with and without decrease in blood volume. METHODS: A model, which was previously used to reproduce cardiovascular response to lower body negative pressure (LBNP), hypovolemia, and MCD, was modified by incorporating the physiologic mechanism of plasma filtration into the interstitium during LBNP. The

The role of cardiovascular magnetic resonance in heart failure.

Cardiovascular Magnetic Resonance (CMR) is an accepted gold standard for non-invasive, accurate, and reproducible assessment of cardiac mass and function. The interest in its use for viability, myocardial perfusion and coronary artery imaging is also widespread and growing rapidly as the hardware and expertise becomes available in more centres, and the scans themselves become more cost effective. In patients with heart failure, accurate and reproducible serial assessment of remodelling is of prognostic importance and the lack of exposure to ionizing radiation is helpful. The concept of an inte

[Pacemaker therapy in heart failure].

Drug therapy is the standard therapy for heart failure. The current state of the art does not permit pacemaker therapy as a general recommendation for left ventricular failure. Numerous controversial reports have been published on the success of pacemaker therapies for congestive heart failure. Iskandrian reported on tachycardiac atrial stimulation in 1986. Since 1990 several papers have been published on DDD-pacemakers with short AV-time. From the data available to us, congestive heart failure is generally not considered a new pacemaker indication. In carefully selected patients, however, an
