Incidentally detected microcystic serous cystadenoma of the pancreas with splenic invasion: a case report and literature review.

Serous cystic neoplasms are relatively uncommon and rarely possess malignant potential. We report a rare case of pancreatic serous cystadenoma with splenic invasion in a female in her 60s. Dynamic contrast-enhanced CT revealed a 3 cm mass in the tail of the pancreas. The lesion showed marked enhancement in the arterial phase on dynamic CT, which extended into the spleen. No cystic components were detected in the pancreatic mass on either magnetic resonance cholangiopancreatography or T 2 weighted imaging. No metastasis or lymph node swelling was detected. Based on the hypervascularity of the t

Radiologist's Guide to the Ninth Edition TNM Staging System for Quantitative and Qualitative Assessment of Pleural Mesothelioma.

Pleural mesothelioma is a highly lethal, rare thoracic malignant neoplasm arising from mesothelial cells. Previous versions of the clinical TNM staging classification have relied on a qualitative radiologic assessment of tumor invasion. However, these qualitative assessments have not proved practical to implement, with poor interobserver agreement and lack of prognostic significance. The updated ninth edition of the International Association for the Study of Lung Cancer TNM staging system for pleural mesothelioma, jointly published by the American Joint Committee on Cancer and Union for Intern

Multiple primary malignant neoplasms and long-term survival: A case report.

At present, morbidity rates in patients with multiple primary malignant neoplasms (MPMNs) are continually improving as tumor diagnosis and treatment technology progress. The present study aimed to describe the rare case of a patient with MPMNs. Following initial detection of the first primary malignant tumor, the patient underwent six surgical interventions. The medical history showed that the patient had been sequentially diagnosed with seven types of primary tumors over a period of 38 years. These included sigmoid colon cancer, thyroid cancer, ileocecal adenocarcinoma, left lung invasive ade

Determining extent of invasion and follow-up of thymic epithelial malignancies.

Thymic malignancies may exhibit aggressive behavior such as invasion of adjacent structures and involvement of the pleura and pericardium. The role of imaging in the evaluation of primary thymic neoplasms is to properly assess tumor staging, with emphasis on the detection of local invasion and distant spread of disease, correctly identifying candidates for preoperative neoadjuvant therapy. Different imaging modalities are used in the initial investigation of thymic malignancies including chest radiography, computed tomography (CT), magnetic resonance imaging (MRI) and positron emission tomogra

IgG4-Related Hypertrophic Pachymeningitis at the Falx Cerebrii with Brain Parenchymal Invasion: A Case Report.

BACKGROUND: Hypertrophic pachymeningitis has been described as a manifestation of a number of conditions, like infection and neoplasms such as dural carcinomatosis or lymphomas. IgG4-related hypertrophic pachymeningitis is a new entity identified during the past decade and most reports described pachymeningeal involvement only. CASE DESCRIPTION: We present a 58-year-old woman with isolated IgG4 hypertrophic pachymeningitis at the falx cerebrii that exhibited cerebral parenchymal invasion. She initially presented with left lower limb weakness. Magnetic resonance imaging of brain showed a contr

Invasive onychocytic carcinoma.

Neoplasms originating from nail matrix keratinocytes are very rare. Onychomatricoma and onychocytic matricoma are benign tumors arising from nail matrix keratinocytes. Only one case of onychocytic carcinoma, the malignant counterpart of onychocytic matricoma, has been reported in the literature. Herein, we describe a case of invasive onychocytic carcinoma. Two biopsy specimens of the tumor, obtained at early and invasive stages, were examined histopathologically. The first biopsy specimen showed a retiform proliferation of eosinophilic and basophilic cells in the nail matrix. The second biopsy

Aggressive angiomyxoma of the female urethra.

Aggressive angiomyxoma is a rare locally invasive mesenchymal neoplasm of unknown pathogenesis arising predominantly in pelvic and perineal tissues of adult women. Surgical excision is the classical management but is associated with the risk of recurrence, especially with incomplete excision. There is a proposed role for adjuvant hormonal therapy. We report a very rare case of urethral aggressive angiomyxoma, managed by surgical excision, in a 40-year-old woman who presented with severe hematuria with clot retention.

Kyphoplasty for the treatment of an atypical osteoporotic vertebral compression fracture of the lumbar spine: A case report.

Vertebral augmentation is a minimally invasive but sometimes technically challenging intervention typically reserved for the treatment of older patients with painful vertebral compression fractures due to osteoporosis or neoplasms. We report the successful treatment of osteoporotic vertebral compression fractures of the first lumbar vertebral body (L1) using kyphoplasty in a paraplegic young patient with multiple comorbidities. Despite the unusual and complicated clinical scenario, kyphoplasty was nonetheless performed with immediate and lasting pain relief.

Total invasion of the pancreatic gland by a rare aggressive IPMN: Report of a case.

INTRODUCTION: The intraductal papillary mucinous neoplasms (IPMNs) are increasingly diagnosed entities with high potential of malignancy and significant recurrence rates. PRESENTATION OF CASE: In this case report a rare mixed IPMN with full invasion of the pancreatic ducts was identified. DISCUSSION: Difficulties in determining the grade of pancreatic duct involvement make the surgical management tough. CONCLUSION: In multifocal IPMNs the management of the patients is even more challenging. In this case the patient was treated successfully with total pancreatectomy.

Surgical treatment of thymic tumors.

Thymoma is a rare neoplasm usually with an indolent growth pattern; however, local invasion and/or metastases may occur. The association with several paraneoplastic syndromes, especially myasthenia gravis, makes thymoma an interesting biologic tumor model. Surgery has been the standard of care for early stage disease with high cure rates anticipated. Multimodality therapy can result in long-term disease-free survival for patients presenting with locally advanced disease.

Squamous cell carcinoma of the thymus with paraneoplastic hypercalcemia.

Primary thymic carcinoma is a rare neoplasm that in contradistinction to thymoma, is not supposed to be associated with a paraneoplastic syndrome. A 73-year-old man, with new onset of disorientation, was found to have an elevated serum calcium level as the cause. Computed tomography demonstrated a mediastinal mass, pericardial invasion, and metastases to the lung. Examination of a biopsy specimen revealed thymic squamous cell carcinoma. Thus, a paraneoplastic syndrome, in this case hypercalcemia, does not exclude primary carcinoma of the thymus.

Retinoblastoma seen after surgery for traumatic cataract.

A case is presented of traumatic cataract which was aspirated to reveal a retrolenticular mass. Enucleation of the eye showed this was a retinoblastoma whose cells were adjacent to the gaping surgical wound. I emphasize the facts that trauma and its sequelae do not preclude existence of previous serious lesions and that invasive surgical therapy in cases of intraocular neoplasm may lead to their extraocular spread. Diagnostic ultrasound examination of eyes with opaque media may be of great value in preventing such cases.

A case of transient monoclonal plasma cell aberrancy following respiratory syncytial virus (RSV) vaccination.

Plasma cell neoplasms include a spectrum of disorders ranging from monoclonal gammopathy of undetermined significance to multiple myeloma, defined by clonal plasma cell proliferation and production of monoclonal immunoglobulins. However, transient monoclonal plasma cell expansions without neoplastic features are rarely documented. Herein, we report a 77-year-old female patient with a history of stage IVa high-grade serous carcinoma of the ovary who was found to have leukocytosis and 22% circulating monoclonal plasma cells during routine preoperative testing, 4 days after receiving the RSV vacc

Solid pseudopapillary neoplasm of the pancreas: A case report.

RATIONALE: Solid pseudopapillary neoplasm (SPN) of the pancreas, first described in 1959, is a rare low-grade tumor associated with a good prognosis. PATIENT CONCERNS: We report the case of a 28-year-old man without significant preexisting disease who underwent abdominal computed tomography for suspected diverticulitis. Subsequently, an incidental finding revealed a pancreas with a homogeneous structure and a hypodense lesion measuring 22 × 19 mm located in the cranial aspect of the pancreatic body, which was visible in the arterial phase. DIAGNOSES, INTERVENTIONS, AND OUTCOME: It was suspec

Diagnosis and Prognosis of Canine Melanocytic Neoplasms.

Canine melanocytic neoplasms have a highly variable biological behavior ranging from benign cutaneous melanocytomas to malignant oral melanomas that readily metastasize to lymph nodes and internal organs. This review focuses on the diagnosis and prognosis of canine melanocytic neoplasms. While pigmented melanocytic neoplasms can be diagnosed with fine-needle aspirates, an accurate prognosis requires surgical biopsy. However, differentiating amelanotic spindloid melanomas from soft tissue sarcomas is challenging and often requires immunohistochemical labeling with a diagnostic cocktail that con

Completeness of case ascertainment in a Scottish regional cancer registry for the year 1992.

To assess completeness of case ascertainment by the Scottish Cancer Registry for the year 1992, we assembled a collection of databases containing potential registrations (excluding non-melanoma skin tumours and non-invasive neoplasms) from 14 separate sources relating to the population covered by one of the five regional registries. Apparently missed registrations were identified by linkage of these databases to cancer registration records. Their eligibility for registration was determined by reference in medical records, or when these were unavailable, by reference to the local Community Heal

Urothelial neoplasms in pediatric and young adult patients: A large single-center series.

PURPOSE: Bladder cancer is the sixth most common cancer in the United States, but is exceedingly rare in young patients, leading to a lack of accepted standards for diagnosis, treatment, and surveillance. We review our institutional experience with bladder urothelial neoplasms in pediatric and young adult patients summarizing presentation, treatment, and outcomes. METHODS: Surgical pathology records at our institution were searched for cases of urothelial neoplasms among patients ≤25 years of age treated between January 1997 and September 2016. Cases submitted exclusively for pathology review

Plexiform fibromyxoma: a distinctive benign gastric antral neoplasm not to be confused with a myxoid GIST.

A great majority of gastric mesenchymal tumors are gastrointestinal stromal tumor (GIST). A rare group of non-GISTs include myxoid mesenchymal neoplasms. In this report, we describe 12 cases of a distinctive gastric tumor, named here as plexiform fibromyxoma. These tumors occurred in 5 men and 7 women of ages 7 to 75 years (median, 41 y). All tumors were located in the gastric antrum and 6 of them also extended into extragastric soft tissues or into the duodenal bulb. The tumors measured from 3 to 15 cm (median, 5.5 cm). Histologically typical was a plexiform intramural growth with multiple mi

A Case of Robot-Assisted Pylorus-Preserving Pancreatoduodenectomy for Branch-Duct Intraductal Papillary Mucinous Neoplasms Complicated With an Annular Pancreas.

Annular pancreas is a rare congenital anatomical anomaly, in which the pancreatic parenchyma surrounds the descending duodenum. Generally, annular pancreas is diagnosed on the basis of symptoms associated with complications of peptic ulcer, pancreatitis, cholelithiasis, and rarely, malignant tumors. Herein, we report an 84-year-old man for whom, during hospitalization for a urinary tract infection, pancreatic cystic lesions and an annular pancreas were noted incidentally on computed tomography. These findings led to a diagnosis of intraductal papillary mucinous neoplasms on further examination

Non-exposed endoscopic wall-inversion surgery for an early gastric cancer arising from heterotopic submucosal gastric glands: A case report.

A 74-year-old man, who was scheduled for surgery against the main duct-type intraductal papillary mucinous neoplasm of the pancreas, was found to have a subepithelial lesion of the stomach under esophagogastroduodenoscopy. Endoscopic ultrasound-guided fine needle aspiration for the gastric lesion revealed adenocarcinoma cells. We thus considered carcinomas arising from heterotopic submucosal gastric glands and metastases from the pancreatic lesion as differential diagnoses. We first non-exposed endoscopic wall-inversion surgery to the lesion as a total biopsy. The gastric lesion was diagnosed
