The role of pudendal nerve damage in the aetiology of genuine stress incontinence in women.

Conduction was studied in the terminal branches of the pudendal nerve in 42 women with normal urinary control and 87 women with stress incontinence of urine, genitourinary prolapse, or both. Women with stress incontinence of urine had delayed conduction to both the striated urethral muscle and the pelvic floor muscle, indicative of denervation injury. Women with normal urinary control and genitourinary prolapse had similar conduction times to the urethral sphincter striated muscle as normal women but clear evidence of denervation damage to the pelvic floor.

The muscle fiber conduction velocity and power spectra in familial hypokalemic periodic paralysis.

Surface EMG has been used to determine the average muscle fiber conduction velocity (MFCV) and power spectra of the m. biceps of 10 patients and 15 asymptomatic offspring of a large kinship with familial hypokalemic periodic paralysis (HOPP). The MFCV of the patients was 3.37 +/- 0.35 m/sec (mean +/- SD, n = 9), the median frequency (Fmed) of the power spectra was 55.0 +/- 5.8 Hz (mean +/- SD, n = 9), both values are significantly (P0.001, Student's t-test) lower than the control values: MFCV = 4.55 +/- 0.33 m/sec; Fmed = 88.6 +/- 15.5 Hz (mean +/- SD). In 6 of the 15 asymptomatic relatives, t

Diaphragm strength in patients with recent hemidiaphragm paralysis.

Eleven patients with unilateral diaphragm paralysis of recent onset were studied to investigate the effect of the paralysis on inspiratory muscle function. Nine of the patients had noticed a decrease in exercise tolerance, which was not explained by any other pathological condition. Hemidiaphragm dysfunction was confirmed by the demonstration of a greatly reduced or absent transdiaphragmatic pressure on stimulation of the phrenic nerve in the neck, by means of surface bipolar electrodes (unilateral twitch Pdi), compared with normal values on the contralateral side. Transdiaphragmatic pressure

Lumbrical sparing in carpal tunnel syndrome: anatomic, physiologic, and diagnostic implications.

Motor axons supplying lumbrical muscles are less severely affected than axons supplying thenar muscles in the carpal tunnel syndrome; sometimes lumbrical motor fibers are less affected than digit 2 sensory fibers. This pattern is consistent with compression of both the anterior and posterior aspects of the median nerve in the carpal tunnel because nerve fibers responsible for thenar, lumbrical, and digit 2 functions lie in an anterior-posterior gradient within the distal median nerve. Recognition of lumbrical sparing supports the electrodiagnosis of carpal tunnel syndrome when the distal laten

Cryoglobulinaemic neuropathy--a clinical spectrum.

In this report we present two cases who illustrate both ends of the spectrum of neuropathy with cryoglobulinaemia. Both patients responded well to treatment. A cryoglobulin assay should be done routinely in patients with mononeuritis multiplex of unknown cause, cutaneous vasculitis with cold sensitivity and chronic demyelinating neuropathies of unknown cause.

Sensory conduction velocity of plantar digital nerves in Morton's metatarsalgia.

The orthodromic sensory conduction velocity of the plantar interdigital nerves were measured with needle electrodes in 28 healthy subjects. The stimulating needle electrodes were placed close to the shaft of the proximal phalanx of the toe, and the recording needle electrode was placed behind the medial malleolus. We examined six subjects with clinical symptoms of entrapment of the common plantar digital nerve (Morton's metatarsalgia). Five of the subjects later had surgery. The conduction velocities of the affected interdigital nerves were abnormally slow. The method is technically simple, bu

Multifocal neuropathy and vocal cord paralysis in relapsing fever.

A 19-year-old soldier developed multifocal neuropathy following an infection with tick-borne Borrelia. He suffered from involvement of the right accessory nerve, the right brachial plexus and the left recurrent laryngeal nerve. As was expected, all these complications were reversible. In contrast to louse-borne relapsing fever, neurological manifestations in the tick-borne variants are not common. To the best of our knowledge, vocal cord paralysis has never been described before.

[Electroneurographic findings in cyclist's palsy (author's transl)].

The electromyographic and electroneurographic findings of a patient with cyclist's palsy of both hands are reported. The electrophysiological results show a compression syndrome of the deep palmar motor branch of the ulnar nerve without involvement of the hypothenar branches (Fig. 2a, b). Two more types of cyclist's palsy should be differentiated reviewing the literature: 1. a compression of the ulnar nerve at the wrist a lesion of the superficial branch, and 2. a compression of the deep motor branch including the hypothenar branches. As in the literature described probably exists a further ty

Injection injury of the sciatic nerve: unusual anatomic distribution of nerve damage.

Sciatic nerve damage by intragluteal injections tends to injure the peroneal nerve trunk more severely than the other components of the sciatic nerve. 2 patients with predominant or sole damage to the posterior tibial nerve trunk were observed. This distribution was documented electrophysiologically. Our cases are unusual and point to a wider spectrum of such injuries. It is also suggested that internal fibrosis of the nerve is of greatest importance as a pathogenetic mechanism in these injuries.

Properties of intraoral mechanoreceptors represented in the mesencephalic nucleus of the fifth nerve in the cat.

1. The activity of neurones in the mesencephalic nucleus of the fifth nerve that respond to forces applied to the teeth were recorded using extracellular microelectrodes; the properties of these neurones have been studied. 2. Electrophysiological evidence consistent with the view that primary afferent intraoral mechanoreceptor fibres have their cell bodies in the trigeminal mesencephalic nucleus is presented. 3. Two groups of intraoral mechanoreceptor neurones were found. The first group, the periodontal mechanoreceptor neurones, which have been described by previous workers, responded to elec

Membrane properties and conduction velocity in sensory neurones following central or peripheral axotomy.

1. The properties of dorsal root ganglion cells in the lumbosacral segments were examined with intracellular electrodes about 3 weeks after section of the central (dorsal roots) or peripheral processes in the cat.2. Chronic section of the peripheral nerve in the hind limb resulted in a reduction in conduction velocity of both the central and peripheral processes of sensory neurones.3. Sensory neurones arising from the triceps surae and plantaris muscles were subject to ;disuse' conditions for about 3 weeks by section of the ventral roots combined with severance of the Achilles tendon. [Results of repeated operations after primary nerve suture].

The results of repeat suturing of 124 nerves following a failure of the initial intervention are analysed. In most cases nerves were injured at the level of the forearm (67%). A delay of the reoperation ranged from 1 month 19 days to 2 years 11 months, more frequently 3-6 months. The results were studied basing on a clinical investigation with grade estimation of muscular strength and sensitivity, measuring skin temperature, perspiration rate and the use of electrophysiological methods.

Clinical Reasoning: A Woman With Subacute Progression of Distal Upper Extremity Weakness.

A 65-year-old woman presented with 3 months of progressive hand weakness, initially with distal-predominant symptoms. EMG was notable for diminished amplitudes in bilateral radial nerves without evidence of conduction block and with normal sensory nerve action potentials. Anti-acetylcholine receptor and antistriated muscle antibodies were positive, but subsequent EMG did not reveal abnormalities on repetitive nerve stimulation. Muscle biopsy was performed, revealing extensive inflammatory infiltrates with significant associated fibrosis. This case discusses the approach to localization within
