ALS: pitfalls in the diagnosis.

OBJECTIVE: To evaluate a surgical technique used to straighten and support the crooked middle third of the nose. DESIGN: Retrospective review of 41 patients with a markedly crooked nose who underwent correction using extended spreader grafts made from high-density porous polyethylene. Follow-up ranged from 6 months to 2 years. RESULTS: Every patient had substantial improvement in the straightening of the middle third of the crooked nose and the airway in one procedure. There were no cases of extrusion or infection. CONCLUSIONS: The extended spreader graft technique introduces structural rei

Segmental myoclonus as the presenting symptom of an acute disseminated encephalomyelitis: a case report.

Segmental myoclonus is described as the involuntary contractions of contiguous muscles innerved by the brain stem or by spinal cord. The underlying causes of segmental myoclonus in children are demyelinating diseases and intrinsic tumors. Here, we report a case who was presented with segmental myoclonus on his left arm and later diagnosed as atypical monosymptomatic presentation of acute disseminated encephalomyelitis (ADEM). The case represents the first in the literature in which ADEM is considered as the possible cause of segmental myoclonus. Our findings demonstrate that: (i) in focal move

Neurophysiologic studies in early-onset cerebellar ataxia.

The discovery of the gene for Friedreich's ataxia (FRDA) has not only broadened the FRDA phenotype, but has also identified patients with early-onset cerebellar ataxia who resemble FRDA clinically but who do not carry a mutation in the frataxin gene. In order to identify subgroups that may represent a uniform underlying disorder, we performed neurophysiologic studies, including nerve conduction studies, electromyography, and transcranial magnetic stimulation, in 15 patients with a slowly progressive, unexplained, early-onset cerebellar ataxia (EOCA). In addition, sural nerve biopsy data were a

Towards creation of a unified view of the neurodevelopment of the infant.

During the twentieth century, study of the neurologic development of the fetus and infant has resulted in multiple neurodevelopmental assessments. They have been used both for determination of the integrity of the neonate as well as for assessment of the child's outcome from prenatal and neonatal medical interventions. These models of assessment have broadened our view and understanding of the development of functions such as movement, posture, attention, oromotor skills, and behavior. The link between these areas and the traditional areas of neurologic and psychiatric examination of older chi

[Guillain-Barré syndrome secondary to cranial surgery: direct or fortuitous relationship?].

The Guillain-Barré syndrome, an acute polyradiculonevritis (PRN), usually appears after a post-infectious immuno-allergic reaction. We report a case of acute PRN following cranial surgery. Post-surgical Guillain-barré syndrome is rarely described. We reviewed the pathophysiological mechanisms possibly involved in this disease that have to be evoked in patients with abnormal post-surgical neurological symptoms.

Do BPH patients have neurogenic detrusor dysfunction? A uro-neurological assessment.

INTRODUCTION: Most men with benign prostatic hyperplasia (BPH) are middle- aged and elderly. Neurogenic detrusor dysfunction (NDD) may also occur in these populations. We made an uro-neurological assessment in such patients in order to investigate the possibility of a concurrent NDD in patients diagnosed with BPH, and to correlate the presence of NDD with treatment outcome. MATERIALS AND METHODS: 28 men, mean age 66.4 years, referred by urologists at the outpatient clinic as BPH (initial diagnosis: BPH alone, 24, BPH and NDD, 4) with regards to digital examination and lower urinary tract symp

Cerebral autosomal dominant arteriopathy with subcortical infarcts and leucoencephalopathy (CADASIL): a case report with review of literature.

Cerebral autosomal dominant arteriopathy with subcortical infarcts and leucoencephalopathy (CADASIL) is an inherited arterial disease, commonly overlooked or misdiagnosed. We report a case of CADASIL in a 51 years old woman who presented with progressive subcortical dementia, recurrent ischemic events and seizures in the absence of known vascular risk factors of five years' duration. Her mother had a history of similar illness. Magnetic resonance imaging (MRI) of brain revealed subcortical and deep white matter hyperintense lesions within the cerebral white matter on T2-weighted images. DNA mu

Clinical manifestations and stages of Rett syndrome.

The presentation and clinical diagnosis of Rett syndrome at various ages and stages are reviewed. In addition to the classical form, variability in phenotype between different atypical Rett forms is given. Obligatory, supportive, and differential diagnostic criteria are summarized. Long-term follow-up findings in ageing Rett women are addressed.

[An autopsy case with subacute spinal cord disease showing progressive paraplegia].

We report a 72-year-old woman who died of respitory failure. History included onset of diabetes mellitus at the age of 67 years and hypertension at the age of 72 years. The patient had been in good health otherwise until 2000, when she had onset of numbness or tingling of the bilateral lower limbs. On December 3, 2000, she was admitted to a hospital in the vicinity of her home because of the above-mentioned complaints. Neurological examinations revealed progressive paraplegia. Symptoms and signs suggested Guillain-Barré syndrome. Examinations of cerebrospinal fluids revealed cell count of 338/

[How can child development disorders be recognized in early childhood?].

In the last 10 years beside the well known developmental criterias like turning from back to prone, crawling, standing up, free walking, speaking of the first words and the position-reactions, the exact observation of the spontaneous movements, of handling-planing and the speech development could be detected as more important factors for the examination of development. Certain parameters of explanation of developmental disorders are to differenciate from uncertain and only probably one. Of special interest are the frequent problems like excessive crying, feeding- and sleeping disorders.

[Biothesiometry in the diagnosis of peripheral neuropathies].

The aim of review is to give basic information on the method of peripheral neuropathy examination using biothesiometry technique. Impaired vibratory threshold can be identified in all patients with peripheral neuropathies, the diabetic, uremic, alcoholic or paraneoplastic ones. It is a simple, sensitive and comfortable method for daily screening. On the other hand it is sufficiently sensitive for detection and evaluation of peripheral neuropathy. Principle is a vibrating probe, vibration amplitude can be changed by voltage adjustment. Biothesiometry is used in diagnostics of peripheral neuropa

Neurologic conditions causing lameness in companion animals.

Animals presented with non-weight-bearing lameness are a diagnostic challenge for the veterinarian. It is extremely important to distinguish between orthopedic and neurologic causes of lameness, because the diagnostic and therapeutic plans can be quite different. Myopathies can be confused with orthopedic disease because of gait abnormalities and associated muscle pain. Common myopathies seen in companion animal medicine include polymyositis, muscular dystrophy, endocrine and infectious myopathies, and myasthenia gravis. Lameness caused by disease of the nerve root or nerve is confused with or

[30 years multiple system atrophy concept: retrospect and overview of multiple system atrophy].

Multiple system atrophy represents an enigmatic, clinico-pathologically defined neurodegenerative disease. According to findings of the last three decades, the historically derived, previously used terms olivopontocerebellar atrophy, striatonigral degeneration, and Shy-Drager-syndrome should be avoided in clinical use because of both the clinical and morphological overlap between these syndromes. Complex neurodegenerative syndromes other than multiple system atrophy according to recently developed criteria, should rather be referred to as multiple system degenerations.

Cerebellar involvement as a rare complication of pneumococcal meningitis.

A 4-year old girl with meningitis, caused by streptococcus pneumoniae, developed a subcoma with respiratory insufficiency, followed by a severe cerebellar syndrome. Cerebellar involvement after regaining consciousness consisted of a symmetrical ataxia and mutism. This mutism changed into dysarthria and finally into normal speech. Magnetic resonance imaging revealed lesions in both cerebellar hemispheres, suggesting cerebellitis. She recovered with prompt antibiotic treatment.

Aneurysms and subarachnoid hemorrhage.

Most unruptured intracranial aneurysms that produce neuro-ophthalmologic signs arise from the junction of the internal carotid and posterior communicating arteries. These aneurysms typically compress the third nerve in the subarachnoid space. Compression of cranial nerves within the cavernous sinus is less common, resulting in single or multiple and often painful ocular motor nerve pareses. Unruptured aneurysms of the proximal and distal segments of the intracranial portion of the internal carotid artery can compress the anterior visual pathways and cause visual loss. The initial recognition and diagnosis of dementia.

Dementia is characterized by a decline in cognition, behavioral disturbances, and interference with daily functioning and independence. Diagnosis is sometimes delayed as patients or family members often misattribute obvious manifestations of cognitive decline to normal aging rather than to the onset of a degenerative disease. Many physicians do not perform mental status examinations or do not use them effectively to detect early symptoms. Clinical markers are available to decrease the difficulty in distinguishing dementia from depression and confusional states such as delirium. Alzheimer's dis
