New primary malignant melanoma, epidermotropism and Indian-file arrangement of metastatic tumor cells in a case with intransit metastases of acral type of malignant melanoma.

Multiple malignant melanocytic lesions developed on a localized area of the thigh in an 80-year-old woman 3 years after excision of a primary malignant melanoma on the left second toe and dissection of left inguinal lymph nodes. Biopsy specimens obtained from 2 small pigmented macules revealed the histologic features of new primary lesions, simulating that of the primary lesion on the toe; no tumor cells were found within vessel lumina. A brown-black papule neighboring one of the macules histologically resembled compound type of melanocytic nevus, but the tumor cells compressed thinned epiderm

Clinical and Dermoscopic Characteristics of Facial Hyperpigmentation Disorders in Skin of Color.

Facial hyperpigmentation disorders (FHD) are common in dermatology, particularly in people with skin of color (SOC). They include melasma, post-inflammatory hyperpigmentation, and less frequent conditions such as exogenous ochronosis, lichen planus pigmentosus, Riehl's melanosis, acanthosis nigricans, seborrheic melanosis, pigmentary demarcation lines, drug-induced pigmentation, and nevus of Ota, among others. These conditions may significantly affect quality of life. They can present similar clinical and dermoscopic features, making diagnosis challenging. A thorough medical history, physical

Linear and whorled nevoid hypermelanosis.

A case of hyperpigmentation distributed in streaks and whorls was recently delineated and separated from a confusing number of pigmentary disorders. A 13-year-old boy had hyperpigmented, reticulate spots on his trunk and neck in a linear and whorled distribution following the lines of Blaschko. Onset was at age 1 year. Histologically, the spots showed increased melanin in the basal layer of the epidermis, but no increase in melanocytes, incontinence of pigment, or melanophages were seen in the dermis. The karyotype was 46,XY and there was no evidence of chromosomal mosaicism or chimerism. This

[Dermo-neuro-hypsarrhythmia. Association of a congenital dermatosis and hypsarthymia. 9 cases (author's transl)].

These 9 cases confirm that tuberose sclerosis, von Recklinghausen's neurofibromatosis, incontinentia pigmenti, linear warty sebaceous naevus and alopecic naevus resulting in a woolly naevus of the scalp may be complicated by flexion spasms with hypsarhythmia. Early diagnosis of hypsarhythmia makes possible specific treatment with ACTH or hydrocortisone.

Versatile Xenopus tropicalis model with targeted integration of human BRAFV600E.

Targeting exogenous gene integrations in animals often exhibits low efficiency, limiting the development of gene knock-in models. Theoretically, by screening founder generation individuals based on the cell phenotypes resulting from gene knock-ins and leveraging the high fecundity of animals, heritable descendants with targeted knock-ins can be efficiently generated. Therefore, we utilized the high fecundity of Xenopus tropicalis and easily observable pigment phenotypes to construct a BRAFV600E-targeted mitf locus knock-in model. Results indicated that this approach enabled efficient generatio

Melanoma Metastasizing to the Breast: A Case Report.

BACKGROUND: Melanoma metastasizing to the breast is a rare event, accounting for only 1.3%-2.7% of reported cases. Due to its overlapping clinical and imaging features, it is frequently misdiagnosed as primary breast cancer. CASE PRESENTATION: This study presents a rare case of metastatic malignant melanoma involving the breast. A 49-year-old female was admitted with a palpable mass in the right breast. The breast lesion was classified as BI-RADS 4b. Core needle biopsy initially suggested primary breast malignancy. However, postoperative histopathological examination of the resected specimen

Penile Skin as a Rare Location of Melanoma in the Balkan Region: A Case Report.

Skin melanoma is very rarely found in the genital region. Additionally, out of all penile skin malignancies, melanoma is one of the rarest, with no cases reported before in the Balkan region. A 68-year-old male presented with atypical and irregular pigmented skin lesions on his left flank and penile shaft. After dermoscopy and radiological work-up, both lesions were completely excised, with pathological examination revealing a high-grade dysplastic nevus with nodular melanoma in the penile location. Wide re-excision was performed one month later, followed by skin reconstruction with a rotation

Neural Crest-Derived Co-localization: A Case of Granular Cell Tumor Arising Beneath a Nevus Spilus.

Granular cell tumors (GCTs) are rare neoplasms believed to originate from Schwann cells, exhibiting immunohistochemical and ultrastructural features consistent with neural crest derivation. Similarly, a nevus spilus (NS) is a pigmented skin lesion composed of lentiginous background macules with superimposed darker macules or papules, also thought to arise from neural crest-derived melanocytes. We report a unique case of a 27-year-old female who presented with a painful subcutaneous nodule on the left medial midback, overlaid by an NS. Surgical excision revealed a lesion consistent with a GCT b

Polarization-sensitive hyperspectral imaging in vivo: a multimode dermoscope for skin analysis.

Attempts to understand the changes in the structure and physiology of human skin abnormalities by non-invasive optical imaging are aided by spectroscopic methods that quantify, at the molecular level, variations in tissue oxygenation and melanin distribution. However, current commercial and research systems to map hemoglobin and melanin do not correlate well with pathology for pigmented lesions or darker skin. We developed a multimode dermoscope that combines polarization and hyperspectral imaging with an efficient analytical model to map the distribution of specific skin bio-molecules. This c

Soft-tissue sarcomas other than rhabdomyosarcoma in children.

Forty-seven children with nonrhabdomyosarcomatous soft-tissue sarcomas (NRSTS) were treated by the Hematology-Oncology Service at Texas Children's Hospital, Houston, Texas, between 1958 and 1990. The male:female ratio was 1:1, and the median age was 11 years (3 weeks-16 years). A preexisting condition was found in 9/47 (19%) patients including neurofibromatosis (3), Down's syndrome (1), spina bifida (1), congenital facial asymmetry (1), giant pigmented nevus (1), juvenile onset diabetes mellitus (1), and acquired immune deficiency syndrome (1). The site of primary tumor was head and neck (3),

[Cranial hemihypertrophy in Klippel-Trenaunay syndrome (author's transl)].

Klippel-Trenaunay Syndrome with hemicranial hypertrophy and macrodactyly is reported. The patient was a 3-year-old Japanese woman with an extensive nevus flammeus which extended over the right side of the neck, axilla, upper arm, and leg. Her right extremities were longer and thicker than left, and showed apparent macrodactyly in the right hand. Her skin of right side body was irregular, rough surface and was pigmented prominently. Hyperthermia was present in the hypertrophic limbs, but hyperhidrosis was none. She had moderate mental retardation, delayed speech. Her hemicranial hypertrophy (sa

Nested Melanoma, a New Morphological Variant of Superficial Spreading Melanoma with Characteristic Dermoscopic Features.

A new morphological variant of superficial spreading melanoma (SSM) was first described by Kutzner et al. (1) and named "melanoma composed exclusively or predominantly of large nests"; it was later named "nested melanoma" (NM) (2,3). Clinically, lesions are larger in diameter (>6 mm), mostly showing typical clinical features of melanoma (the ABCD rule), and significantly different from all other pigmented lesions (the "ugly duckling sign") (1). The majority of NM is found on the trunk and limbs of patients older than 60 years (1-9). Dermoscopy shows typical features of melanoma (asymmetry, irr

Mole or Early Melanoma? Rethinking Early Detection, Overdiagnosis, and the Benign-Malignant Paradigm.

Melanoma early detection has been widely promoted on the assumption that diagnosing early tumors improves outcomes, but epidemiologic data increasingly challenge this assumption. In fair-skinned populations subjected to intensified surveillance, diagnoses of thin invasive and in situ melanomas have risen sharply without corresponding declines in advanced disease or melanoma-related mortality, a pattern consistent with cancer overdiagnosis. Heightened scrutiny preferentially detects biologically indolent melanocytic proliferations that often share clinical and histologic features with genuinely

Living with a rare disease: Why lived experience must shape research.

Rare diseases are often defined by numbers and clinical perspectives, a narrative that needs re-writing. Drawing on decades of patient advocacy, two women living with the rare condition Congenital Melanocytic Naevus argue that lived experience must shape research from the outset, and discuss how genuine patient-researcher partnership transforms science, care and identity.

Congenital melanocytic nevi and risk of melanoma.

The presence of congenital melanocytic nevi (CMN) is determined in utero. The location, size, and number of CMN may be of cosmetic concern with significant psychosocial implications. They may also be associated with symptoms such as pruritus, eczema and/or xerosis, and skin fragility; however, the most medically concerning issue is the association of CMN with the risk of developing cutaneous melanoma, extracutaneous melanoma, and neurocutaneous melanocytosis. Patients with CMN are currently risk-stratified based on the projected adult maximum diameter of the largest CMN and the number of CMN (

Noonan syndrome-like disorder: Case report and review of the literature.

Of patients with a Noonan syndrome phenotype, only about 1% are found to be related to pathological variants in CBL, also known as Noonan syndrome-like disorder (NSLD). We present a case of a 4-year-old boy diagnosed with NSLD, presenting with multiple melanocytic nevi and superficial neurofibromas. A literature review identified common cutaneous findings of NSLD, for example, café-au-lait macules (22%), juvenile xanthogranuloma (16%), and thin hair (10%). As there are no documented cases of neurofibromas associated with NSLD, and only a single report of multiple melanocytic nevi, inclusion of

Giant Congenital Melanocytic Nevus with Congenital Neurofibroma: A Case Report.

BACKGROUND: Giant congenital melanocytic nevus (GCMN) is characterized by its large size and potential for transformation into melanoma. It can be associated with other neural cristopathies, including neurofibroma, however, it has not previously been described with a congenital neurofibroma. CASE REPORT: A newborn girl presented with a large congenital neurofibroma arising in a bathing trunk type of giant congenital melanocytic nevus. CONCLUSION: Congenital neurofibromas can be associated with (or a component of) a GCMN.

Can the MOLES acronym and scoring system improve the management of patients with melanocytic choroidal tumours?

It can be difficult for practitioners to determine the likelihood of malignancy in melanocytic choroidal tumours. This author has therefore devised the MOLES acronym to highlight the most informative clinical features, which comprise mushroom shape, orange pigment, large size, enlargement, and subretinal fluid. Each of these is scored 0 if absent, 1 if subtle or uncertain, and 2 if present. Tumours are categorised as 'common naevus', 'low-risk naevus', 'high-risk naevus' and 'probable melanoma' according to whether the sum of these five scores is 0, 1, 2 or 3 or more, respectively. Tentative r

Neurological signs, symptoms and MRI abnormalities in patients with congenital melanocytic naevi and evaluation of routine MRI-screening: systematic review and meta-analysis.

BACKGROUND: A congenital melanocytic naevus (CMN) is a rare skin condition that can be associated with abnormalities of the central nervous system (CNS). These anomalies can sometimes cause severe complications, and rarely death. Adequate information about aetiology and management is therefore crucial. To identify how to monitor patients with CMN, we aimed to estimate the prevalence of neurological involvement in patients with CMN and to summarize what specific neurological signs and symptoms and MRI abnormalities are reported in the medical literature. In addition, we summarized and evaluated
