Reducing Laboratory Testing and Blood Draws in the Newborn Nursery to Increase High-value Care.

INTRODUCTION: Newborn infants undergo multiple blood tests before hospital discharge, some of which are unnecessary due to outdated hospital policies and provider variability. Reducing nonevidence-based laboratory testing can improve healthcare value, and clustering of blood sampling can reduce pain for newborns. METHODS: This quality improvement project aimed to reduce the weekly number of laboratory tests and blood draws per patient-day for newborns by 10% within 8 months. A multidisciplinary team developed and addressed key drivers with Plan-Do-Study-Act cycles. Interventions included high

Congenital Syphilis in the United States from 2019 to 2024: A Public Health Crisis in Vulnerable Populations.

OBJECTIVE: Congenital syphilis (CS) has reemerged as a critical public health concern in the United States, with rates increasing 10-fold over the past decade. Rising CS cases pose clinical challenges for hospital-based pediatricians, including newborn nursery hospitalists and neonatologists, who are often central to diagnosis, treatment, and follow-up. This study examines patient- and hospital-level characteristics, risk factors, and neonatal outcomes among infants born with CS. METHODS: A retrospective cohort study of 6.75 million patients (2019-2024) was conducted using the Vizient Clinica

Impact of public health measures on the post-COVID-19 respiratory syncytial virus epidemics in France.

Since the beginning of the COVID-19 pandemic, other respiratory illnesses decreased worldwide. This study described the consequences of public health measures on respiratory syncytial virus (RSV) severe infections in France, where an interseasonal resurgence of RSV occurred recently. All patients admitted to Necker Hospital (Paris) between August 2018 and April 2021 with a diagnosis of RSV-associated acute lung respiratory infection (ALRI) were enrolled. Characteristics of subjects with RSV-associated ALRI in 2020/2021 were compared to those infected during the two previous outbreaks. Overall,

Does antenatal corticosteroid therapy improve neonatal outcomes in late preterm birth?

BACKGROUND: Infants who are born at 340/7 to 366/7 weeks of gestation (late preterm) are at greater risk for respiratory and other neonatal morbidities. The objective of this study was to examine the effects of administration of antenatal corticosteroids (ACSs) to women at risk for late preterm delivery on the incidence of neonatal outcomes. METHODS: This was a prospective cohort study of singleton gestations at risk of imminent delivery between 340/7 and 366/7 weeks. Neonatal outcomes were compared between mothers who received ACS and those who did not. Primary outcome was the rate of compos

Associations of dietetic management with maternal and neonatal health outcomes in women diagnosed with gestational diabetes: a retrospective cohort study.

BACKGROUND: In 2010, the recommended diagnostic thresholds for gestational diabetes mellitus (GDM) were amended, which has increased GDM diagnoses, as well as pressure on the services involved in GDM management, specifically impacting dietetic workloads. The present study examined the associations between dietetic intervention in women with GDM and maternal and neonatal health outcomes. METHODS: The present study involved 1233 adult women with GDM who delivered at The Royal Women's Hospital (RWH), Melbourne, Australia, between July 2015 and May 2017. Retrospective data assessing GDM-care (the

Acceptability of a family-centered newborn care model among providers and receivers of care in a Public Health Setting: a qualitative study from India.

BACKGROUND: Family-centered care (FCC), based on collaborative participation of the family along with a team of health care providers, is found to increase the well-being of sick infants in neonatal critical care units. Over the last 4 years, the neonatal unit of Dr. Ram Manohar Lohia Hospital in Delhi has innovated and developed an implementation framework for FCC. This qualitative study assessed the acceptability of family-centered care among providers and family members of neonates to identify gaps and challenges in implementation. METHODS: In-depth interviews were conducted among a purpos

Immersion in water in the first stage of labor: a randomized controlled trial.

BACKGROUND: Current forms of analgesia often have significant side effects for women in labor. Bathing in warm water during labor has been reported to increase a woman's comfort level and cause a reduction in painful contractions. The objective of this trial was to compare immersion in warm water during labor with traditional pain management for a range of clinical and psychological outcomes. METHODS: A prospective randomized controlled trial of 274 pregnant women, who were free from medical and obstetric complications and expecting a singleton pregnancy at term, was conducted at the Women's

Transmission Routes of Extended-Spectrum Beta-Lactamase-Producing Enterobacteriaceae in a Neonatology Ward in Madagascar.

The diffusion of extended-spectrum beta-lactamase (E-ESBL)-producing Enterobacteriaceae is a major concern worldwide, especially in low-income countries, where they may lead to therapeutic failures. In hospitals, where colonization is the highest, E-ESBL transmission is poorly understood, limiting the possibility of establishing effective control measures. We assessed E-ESBL-acquisition routes in a neonatalogy ward in Madagascar. Individuals from a neonatology ward were longitudinally followed-up (August 2014-March 2015). Newborns' family members' and health-care workers (HCWs) were stool-samp

A laboratory-confirmed outbreak of rifampicin-methicillin resistant Staphylococcus aureus (RMRSA) in a newborn nursery.

The routine laboratory monitoring of methicillin-resistant strains of Staphylococcus aureus (MRSA) at a large teaching hospital led to the detection of a new, multiply-resistant strain of MRSA, which was resistant not only to penicillin, oxacillin, methicillin, cephamandole, erythromycin, tetracycline, kanamycin and gentamicin but also to rifampicin and sulphamethoxazole-trimethoprim. The rifampicin-methicillin resistant strain of S. aureus (RMRSA) was first detected in blood cultures of babies from the newborn nursery. A bacteriological investigation of the nursery revealed the source to be a

TORCH: a literature review and implications for practice.

Major issues related to TORCH (toxoplasmosis, rubella, cytomegalo virus, and herpes virus) perinatal infection are of concern to maternity and neonatal nurses. General and congenital modes of transmission, associated congenital defects, incidence of infection, and relative fetal/neonatal effects of primary as compared to recurrent maternal infection are described. Associated nursing implications emphasizing prenatal prevention and early detection, intrapartal management, neonatal symptom detection, and nursery infection control as well as concerns about breastfeeding with maternal TORCH are di

Hospital readmission with feeding-related problems after early postpartum discharge of normal newborns.

CONTEXT: Increasingly short postpartum hospital stays in the United States precipitated a policy debate that culminated in passage of the Newborns' and Mothers' Health Protection Act of 1996. The debate occurred without population-based evidence for adverse health effects in newborns who are discharged early. OBJECTIVE: To determine whether early postpartum hospital discharge of normal newborns increases their risk for hospital readmission with feeding-related problems. DESIGN AND SETTING: Nested case-control analysis of 1991 to 1994 Wisconsin birth certificate and hospital discharge data.

Follow-up studies of very low birth weight infants (1,250 grams or less) born and treated within a perinatal center.

The growth and development of inborn very low birth weight infants was evaluated in 50 of 60 survivors from 132 babies weighing less than or equal to 1,250 gm born July 1974 to December 1977. Mean +/- SE birth weight and gestation was 1,066 +/- 19.3 gm and 29.5 +/- 0.3 weeks, respectively, with 13 infants small-for-gestational age. Of the survivors, 26% weighed less than or equal to 1,000 gm. Male to female ratio was 1:1.4. Apgar scores less than or equal to 5 at five minutes occurred in 16% of the infants. Respiratory distress syndrome occurred in 56%, but only 10% (5/50) required mechanical
