Diagnosis of inflammatory optic neuropathy is very likely in a young adult presenting acute, unilateral and painful visual loss. Relative afferent pupillary defect is a very sensitive and specific sign of optic neuropathy. Ophthalmological exam is mandatory to estimate severity of visual loss, appearance of optic disks and check for another eye disease. Magnetic resonance imaging is indicated to rule out a compressive optic neuropathy and to quantify the inflammatory lesion index, which is the best prognostic indicator of multiple sclerosis development (MRI is abnormal at entry in about 50% of…

BACKGROUND: To evaluate the diagnostic ability of optical coherence tomography (OCT) images to detect posterior pole deformation and anterior displacement of the optic disc (ADOD) in papilledema before and after intracranial pressure reduction with medical treatment. Additionally, we compared the analysis of these images with peripapillary retinal nerve fibre layer (pRNFL) thickness measurements in detecting papilledema.
METHODS: In this retrospective, observational, descriptive, and comparative study, participants underwent swept-source OCT with high-resolution imaging. Papilledema cases wer

Nonarteritic anterior ischemic optic neuropathy (NAION) is the most common clinical presentation of acute ischemic damage to the optic nerve. Most treatments proposed for NAION are empirical and include a wide range of agents presumed to act on thrombosis, on the blood vessels, or on the disk edema itself. Others are presumed to have a neuroprotective effect. Although there have been multiple therapies attempted, most have not been adequately studied, and animal models of NAION have only recently emerged. The Ischemic Optic Neuropathy Decompression Trial, the only class I large multicenter pro

In a child born of a full-term pregnancy, unilateral diplophthalmos without proboscis was observed along with other craniocervical abnormalities (ipsilateral temporoparietal porencephaly, supernumerary teeth, and cervical cyst). A globe almost normal in shape and size was observed within the left orbit. The upper quadrant of this globe was attached to a smaller, pear-shaped, supernumerary eye containing a small lens, normal vitreous humor, ciliary body structures, avascular retina, choroid, and a rudimentary optic disk. When a two-peaked and enlarging intraocular mass was observed clinically, …

We report a case of cat-scratch disease neuroretinitis for which systemic and ocular investigations proved the responsibility of Bartonella henselae. An 11-year-old boy was referred to the hospital in November 2002 for severe visual loss in the left eye over the preceding 2 weeks. At the same time, he also developed a flu-like illness. The best corrected acuity in the left eye was counting fingers at 30 cm. Posterior segment examination on the left eye showed an optic disk edema with papillary and peripapillary hemorrhages and serous retinal detachment, mild vitreous inflammation, and two litt…

BACKGROUND: Optic disc drusen (ODD) are calcified deposits in the prelaminar portion of the optic nerve head. Although often asymptomatic, these deposits can cause progressive visual field defects and vision loss. The purpose of this study was to evaluate rates of functional loss in eyes with ODD and to investigate risk factors associated with rates of visual field progression.
METHODS: This was a retrospective cohort study including 65 eyes of 43 patients with ODD from the Duke Ophthalmic Registry. All eyes had at least 12 months of follow-up and at least 3 reliable standard automated perime

The differential diagnosis for optic atrophy can be challenging and requires expensive, time-consuming ancillary testing to determine the cause. While Leber's hereditary optic neuropathy (LHON) and optic neuritis (ON) are both clinically significant causes for optic atrophy, both relatively rare in the general population, contributing to limitations in obtaining large imaging datasets. This study therefore aims to develop a deep learning (DL) model based on small datasets that could distinguish the cause of optic disc atrophy using only fundus photography. We retrospectively reviewed fundus ph

The vascular supply of the optic nerve head is complex and remains incompletely delineated. Over the past 50 years, various investigators have attempted to clarify the relative contributions of the choroid, the short posterior ciliary arteries and the central retinal artery to the vascular beds of the inner retinal, prelaminar, laminar and retrolaminar segments of the nerve head. Conflicting theories have evolved, in no small part due to differing techniques of study, involving both flow parameters and anatomical constructs. These have included studies, both in normal subjects and in those wit

BACKGROUND: β-Zone parapapillary atrophy (β-PPA) is a common sign in patients with open-angle glaucoma (OAG). Some studies have suggested that β-PPA can aid in the diagnosis of OAG. We performed a systematic review and meta-analysis of the prevalence and diagnostic ability of β-PPA in OAG.
METHODS: We performed a literature search in PubMed, Web of Science, Embase and Google Scholar from inception to 1st November, 2021. Both hospital-based and population-based studies that reported details of β-PPA in OAG were included.
RESULTS: We screened 1404 articles from these databases and ultimately i

Objective: To report an unusual optical coherence tomography (OCT) finding, bilayered retinoschisis formation in the macular region, during non-arteritic anterior ischemic optic neuropathy (NAION). Methods: This is a case report of a patient with NAION and an unusual OCT finding, bilayered retinoschisis formation. Results: A 62-year-old female presented with decreased visual acuity in her right eye for one year. The best-corrected visual acuity was determined as 20/ 160 and no abnormal anterior segment finding was found in the slit-lamb biomicroscopy. A pale optic disc was detected and a wides

BACKGROUND: Although nonarteritic anterior ischemic optic neuropathy (NAION) is considered a disorder that primarily affects the optic nerve head, optical coherence tomography (OCT) shows peripapillary and foveal subretinal fluid associated with optic disc swelling from NAION. We sought to further evaluate retinal and vitreous changes in patients with NAION.
METHODS: Patients diagnosed with NAION at the New England Eye Center between 2013 and 2017 were evaluated using OCT. The presence and distribution of subretinal fluid was analyzed. Evidence of other vitreoretinal changes, including vitreo

Optic nerve (ON) aplasia is a rare developmental anomaly comprising of absence of the ON, ganglion cells and the central retinal vessels. It is usually accompanied by a variety of central nervous system (CNS) malformations. We report an extremely rare case of bilateral true ON aplasia occurring as an isolated CNS anomaly. A 10-month-old female child was detected with bilateral absence of fixation, associated with multiple ocular findings of microcornea, anterior embryotoxon, pupillary corectopia, partial aniridia and lens coloboma. The optic disc locations were visible as avascular whitish are

One of the most susceptible sites to vascular diseases is the optic nerve head. By innovative approaches using morphometry and in situ hybridization, vascular and extracellular characteristics of the human optic nerve head were examined. Nonarteritic ischemic optic neuropathy occurs due to vascular insufficiency within the optic nerve head. Various local and systemic risk factors have been proposed among which smoking and acute hypotension are now included. Anatomically abnormal discs such as the small optic disc, tilted disc, and optic nerve drusen are recognized as "disks at risk" and hypero

The optic disc appearance in the normal fellow eye of 126 patients with nonarteritic anterior ischemic optic neuropathy (n-AION) was compared with the discs in 23 patients with arteritic AION (a-AION) and 122 normal subjects. The number of discs with no cup was significantly greater (P less than 0.001) and the number of discs with a large cup was significantly fewer (P less than 0.001) in the n-AION group compared to the other two groups. No significant differences were found in cup size between the a-AION and normal groups. The pathogenesis of n-AION appears to be multifactorial. There is ove

Terminology, pathogenesis and differential diagnosis of papilloedema are discussed. A series of 100 consecutive patients with optic disc swelling is reviewed. Increased intracerebral pressure accounted for 32% of the cases. Vascular disease, such as central vein occlusion, central arterial occlusion and ischaemic neuropathy were the underlying diagnoses in the second largest group (22%) and in 18% of cases inflammatory disease caused the papillitis.
