Ophthalmoscopic observation of the retinal nerve fiber layer.

Alterations of the retinal nerve fiber layer occur as a result of diseases affecting the anterior visual pathway. Optimal viewing conditions, familiarity with the appearance of the normal nerve fiber layer, precise focus, and a high index of suspicion are necessary for the ophthalmoscopic observation of pathologic alteration in the nerve fiber layer. The observer must learn to differentiate pseudodefects (reflexes) from true defects (areas of nerve fiber layer atrophy). Nerve fiber layer changes appear ophthalmoscopically as generalized attrition, slit defects, sector defects (all types of atr

Bilateral TMJ Ankylosis with limited mouth opening in pediatric patients: An anesthetic challenge.

Restricted mouth opening is a challenging airway in pediatric patients with temperomandibular joint (TMJ) ankylosis. The fiber-optic bronchoscopic nasotracheal intubation technique continues to be the gold standard for difficult airway, among the techniques available such as submandibular intubation, retrograde intubation, and tracheostomy. However, awake fiber-optic bronchoscopy (FOB) is difficult to achieve in pediatric patients. Prior planning of the anesthetic method and effective collaboration with the surgeon are crucial for excellent outcomes in such challenging airway cases. We present

Retinal projections in the house musk shrew, Suncus murinus, as determined by anterograde transport of WGA-HRP.

Retinal projections in the house musk shrew (Suncus murinus) were determined by the anterograde transport of wheat germ agglutinin conjugated to horseradish peroxidase (WGA-HRP). Unilateral injection of WGA-HRP into the vitreous body resulted in the terminal labeling of the optic projections in the suprachiasmatic nucleus (SCH), the ventral (CGLv) and dorsal (CGLd) lateral geniculate nuclei, the intergeniculate leaflet (IGL), the pretectum, the superficial layers of the superior colliculus (CS), and the dorsal terminal nucleus (DTN) of the accessory optic system (AOS). Labeling of the SCH was

[Study of the formation and the normal and pathological evolution of the eye neural primordium (author's transl)].

The analysis of duplicate embryos, cyclops and colobomic fissure defects show that: 1. - optic vesicle and colobomic fissure are inducted by the prechordal plate. The combined inductive influence of prechordal plate and notochord carry eye absence out; 2. - initial failure of the colobomic fissure is always associated with retina folds and produce lens, vitreous and optic nerve anomalies; 3. - colobomic fissure is one indispensable anatomic structure to the optic fiber migration from the retina to the optic pedicle. Retina cutting in 4 days old chick embryos determine the disappearing of the p

Visual system degeneration induced by blast overpressure.

The effect of blast overpressure on visual system pathology was studied in 14 male Sprague-Dawley rats weighing 360-432 g. Blast overpressure was simulated using a compressed-air driven shock tube, with the aim of studying a range of overpressures causing sublethal injury. Neither control (unexposed) rats nor rats exposed to 83 kiloPascals (kPa) overpressure showed evidence of visual system pathology. Neurological injury to brain visual pathways was observed in male rats surviving blast overpressure exposures of 104-110 kPa and 129-173 kPa. Optic nerve fiber degeneration was ipsilateral to the

Robust High-Resolution Fine OCT Needle for Side-Viewing Interstitial Tissue Imaging.

Fine optical coherence tomography (OCT) imaging needles that can be integrated with a standard biopsy needle have been developed with a new optics design to improve the optical quality and mechanical robustness, where a fiber-optic lens (that is spliced to a single-mode fiber) and a microreflector are encased within a microglass tube. The design also minimizes the cylindrical lens effect induced by the glass tube and eases the needle assembly process. Real-time cross-sectional OCT imaging of various tissue samples were performed ex vivo using the miniature-imaging needle along with a 1300-nm s

Hybrid refractive-diffractive vortex beam generator integrated on a fiber tip.

Vortex beams (VBs) carrying orbital angular momentum have attracted widespread interest for applications in optical communication, imaging, and manipulation, yet their generation still mainly relies on bulky optical components that hinder miniaturization and integration. Here, we propose a fiber-integrated hybrid refractive-diffractive device for high-efficiency and high-fidelity VB generation. Using a one-step two-photon polymerization process, a hyperbolic aspherical lens (HAL) for beam collimation and diffraction modulators for vortex phase shaping are monolithically integrated on the fiber

Selective loss of the photopic negative response in patients with optic nerve atrophy.

OBJECTIVE: To determine how the photopic negative response (PhNR) is altered in patients with optic nerve atrophy. METHODS: Ten patients with optic nerve atrophy induced by compression, trauma, or inflammation were examined. There were 6 men and 4 women with a mean age of 52.4 years. Ten age-matched control subjects were examined with the same protocol. Full-field electroretinograms were recorded, and the retinal nerve fiber layer thickness surrounding the optic nerve head was measured by means of optical coherence tomography. RESULTS: The amplitudes of the rod, maximum, cone, and 30-Hz flic

Changes of compound action potentials in retrograde axonal degeneration of rat optic nerve.

Electrophysiological recordings of compound action potentials (CAPs) were made from the optic nerve (ON) following unilateral optic tract (OT) transection in adult rats. In response to optic chiasm stimulation CAPs recorded from the ON ipsilateral to the transected OT, i.e., the ON of control side, revealed three positive waves, termed n1, n2, and n3 components, corresponding to fast-, intermediate-, and slow-conducting fibers. Two weeks after OT transection, CAPs recorded from the ON contralateral to the transected OT, i.e., the ON of cut side, were generally smaller in amplitude than those r

A unified global tractography framework for automatic visual pathway reconstruction.

The human visual pathway starts from the retina, passes through the retinogeniculate visual pathway, the optic radiation, and finally connects to the primary visual cortex. Diffusion MRI tractography is the only technology that can noninvasively reconstruct the visual pathway. However, complete and accurate visual pathway reconstruction is challenging because of the skull base environment and complex fiber geometries. Specifically, the optic nerve within the complex skull base environment can cause abnormal diffusion signals. The crossing and fanning fibers at the optic chiasm, and a sharp tur

Bright seeded and quasi-continuous two-mode intensity squeezing in commercial fiber.

Squeezed states of light enable optical measurements with sensitivity beyond the shot noise limit of detection with classical light. Optical fibers represent a major potential platform for quantum squeezed sensing due to the extremely low loss and perfect single-spatial-mode operation. In this work, we demonstrate two-mode intensity squeezing directly within a commercially available optical fiber, based on a four-wave mixing process. The setup operates in a quasi-continuous-wave mode using 15 ns long pulses and relies on standard laser diode sources, telecommunication electro-optic modulators,…

Diffusion-tensor imaging in septo-optic dysplasia.

One of the morphological correlates of septo-optic dysplasia is hypoplasia of the optic nerves. As of now, it remains unknown, in how far this disorder also affects the organization of the optic radiation. Using diffusion-tensor imaging (DTI), the non-invasive evaluation of large fiber tracts including the optic radiation has become possible. We have compared DTI-data from a patient suffering from septo-optic dysplasia with those of a group of eleven healthy control subjects. The anisotropy showed statistically significant reduction in the patient with septo-optic dysplasia within the visual f

Anatomy of the visual pathways.

The axons of the retinal ganglion cells form the optic nerve. The two optic nerves meet at the optic chiasm, where the nerve fibers originating in the nasal retina of each eye decussate to join the temporal fibers of the fellow eye. From the chiasm, the same axons continue on as the optic tract. These axons travel to and synapse in the lateral geniculate nucleus, the cells of which send their axons through the optic radiations to the visual cortex. The main blood supply to visual cortex is provided by the posterior cerebral arteries and their branches (the calcarine, posterior temporal, and pa

Direct laser and laser-thermal irradiation of normal canine coronary arteries: implications for laser delivery methods.

Clinical use of laser recanalization has been mostly limited by arterial perforation. Two modifications of the optical fiber were used to decrease the perforation rate: (1) a guidewire to align the fiber in a coaxial position with the vascular lumen and (2) encapsulation of the optical fiber tip with a metal cap. Fourteen dogs were studied. Argon laser radiation was delivered through optical fibers, 9 with a 1.7-mm metal encapsulated fiber (Laserprobe-PLR,¿ Trimedyne, Santa Ana, CA) and 5 with a 2-mm metal encapsulated fiber with a window at the tip (Spectraprobe-PLR). Three dogs served as con…

Disconnection's renaissance takes shape: Formal incorporation in group-level lesion studies.

Group-level voxelwise statistical analyses of lesion-deficit relationships have been used to implicate brain structures critical for specific aspects of human cognition and behavior. Current approaches fail to account for the role of fiber tract disruptions in causing deficit, and confound cortical damage with damage to fibers of passage. Here, we develop a framework, Generalized Lesion-Symptom Mapping (GLSM), to integrate fiber tract information from DTI-based tractographic atlases in tractwise and voxelwise lesion-deficit analyses. First, we used the geniculo-calcarine system as a validation

Photographic documentation of the loss of medullated nerve fibers of the retina in uncontrolled primary open angle glaucoma.

PURPOSE: We report the first case of retinal myelin loss associated with uncontrolled primary open angle glaucoma (POAG). METHODS: The clinical features, visual fields, and serial disc photos documenting the changes are described. The literature regarding this phenomenon in other forms of optic neuropathy and the specific implications in glaucoma patients are discussed. RESULTS: A 75-year-old black man with POAG had medullated nerve fibers of the retina bilaterally abutting the optic discs inferonasally. Optic disc appearance and visual fields were stable over 6 years. The patient was then l

Femtomolar Detection by Nanocoated Fiber Label-Free Biosensors.

The advent of optical fiber-based biosensors combined with that of nanotechnologies has provided an opportunity for developing in situ, portable, lightweight, versatile, and high-performance optical sensing platforms. We report on the generation of lossy mode resonances by the deposition of nanometer-thick metal oxide films on optical fibers, which makes it possible to measure precisely and accurately the changes in optical properties of the fiber-surrounding medium with very high sensitivity compared to other technology platforms, such as long period gratings or surface plasmon resonances, th…

Isaac Newton's description of the optic chiasm.

Sir Isaac Newton is widely regarded as one of the greatest scientific minds in history, with seminal contributions across mathematics, physics-particularly optics-and the formulation of the law of universal gravitation. Less well known, however, is his extraordinary and rarely recognized contribution to the field of neurology: the early conceptualization of fiber decussation within the optic chiasm. Through his studies on light and vision, Newton proposed that optic nerve fibers cross at the chiasm-a hypothesis that would later be anatomically confirmed and provided the basis for understanding
