Acute hematogenous osteomyelitis and septic arthritis in children.

This retrospective study analyzed the clinical, bacteriological, and radiological features of pediatric patients with acute hematogenous osteomyelitis and septic arthritis. Eighty-four patients with septic arthritis and 39 with acute hematogenous osteomyelitis were enrolled. Their age ranged from 13 days to 17 years. In patients with septic arthritis, the hip joint was the most often infected site. The tibia was the most often involved site in acute hematogenous osteomyelitis. A bacteriological diagnosis was established in 78 (63%) patients. Overall, methicillin-susceptible Staphylococcus aure…

Role of oral antimicrobial therapy in the management of osteomyelitis.

PURPOSE OF REVIEW: Medical therapy of chronic osteomyelitis is largely based on experimental models, historical observational or non-randomized studies, and expert opinions. A minimum of 4-6 weeks of intravenous antimicrobial therapy targeting the causative organism, given in conjunction with surgery, has become the standard for chronic long-bone osteomyelitis in adults. Given the expense, inconvenience, and potential complications inherent to such a treatment program, alternative strategies including effective oral antimicrobial regimens are desirable. RECENT FINDINGS: Several oral antimicro…

Group A beta-hemolytic streptococcal osteomyelitis in children.

OBJECTIVE: Little attention has been given to acute hematogenous osteomyelitis (AHO) caused by group A beta-hemolytic Streptococcus (GABHS), although up to 10% of cases are caused by this microorganism. The objective of this study was to define the clinical and laboratory characteristics of AHO caused by GABHS. METHODS: Between January 1983 and June 1999, 29 patients were treated at Children's National Medical Center with AHO caused by GABHS. The characteristics of these patients were compared with those of 28 patients with AHO caused by Streptococcus pneumoniae and those of a matched sample …

Soft tissue reconstruction for calcaneal fractures or osteomyelitis.

A systematic approach of the surgical management of a calcaneal fracture can minimize the potential of soft tissue complications. When reduc … ng a clean wound include topical antibiotics (silver sulfadiazine), the VAC, and hyperbaric oxygen. Osteomyelitis has to be treated aggressively. Any suspicious bone has to be removed. Tuberculous osteomyelitis of the cuboid: a report of four cases.

Osteoarticular tuberculosis, although rare, has shown a resurgence in recent times, especially in immunocompromised patients. Involvement of … of the lesion in three cases, while in one case surgical intervention was necessary. Since isolated osteomyelitis is usually seen only in the early stages of the disease process, early diagnosis and appropriate therapy are imperative to get good long-term results. Concomitant extraskeletal lesions are not always seen, nor is the organism cultured in a majority …

[Pseudotumoral subacute osteomyelitis: a series of 41 children].

PURPOSE OF THE STUDY: To confirm that subacute osteomyelitis in children is a real entity, and give the main characteristics of this disease MATERIAL AND METHODS: We reviewed 41 cases as well as the litterature. RESULTS: Pain without fever was the only constant symptom. The erythrocyte sedimentation rate was increased slightly. XRays were the most important investigation, showing a geode in the metaphysis or more rarely in the epiphysis evoking a benign bone tumor in a child. DISCUSSION: The treatment was most often surgical: scraping permits the isolation of the microbe in 50 per cent of c…

[Subacute osteomyelitis in adults. Apropos of 3 cases].

We report three cases of subacute osteomyelitis in 25, 31 and 38-year old adults. The clinical presentation was an insidious condition with persistent nondisabling pain. Radiographical signs suggested a bone tumor. Diagnosis was established from the bacteriological and pathology examinations of the surgical biopsy taken when evacuating the abscess. Cure was obtained in all three patients with combination antibiotic regimens. In two cases immobilisation was required because of bone fragility.

Fluconazole in the therapy of multiple osteomyelitis in African histoplasmosis.

Amphotericin B has been the standard treatment for deep mycoses including African histoplasmosis, but its use is complicated by its toxicity. More recently, Fluconazole, a water-soluble triazole with antimycotic activity and little side effects, has become available. A case of African histoplasmosis from Nigeria, manifesting as multiple osteomyelitis and successfully treated with oral Fluconazole, is presented. Fluconazole may be preferred in the therapy of African histoplasmosis.

Total calcanectomy for the treatment of chronic calcaneal osteomyelitis.

The purpose of this study is to present the surgical and functional results of a total calcanectomy procedure as a foot salvage alternative in patients with extensive chronic osteomyelitis of the calcaneus. A retrospective review identified eight patients treated with a total calcanectomy for a chronic nonhealing plantar ulcer of the heel and osteomyelitis of the calcaneus. The primary diagnosis was insulin-dependent diabetes mellitus (six patients), pneumococcal septicemia (one patient), and an open calcaneal fracture (one patient). The average age of the patients was 52 years. Prior procedur…

Exophthalmos associated with frontal sinus osteomyelitis in a puppy.

A three-month-old, male Great Dane puppy developed progressive left exophthalmos, epiphora, and swelling of the left frontal bone. Radiographs revealed obliteration of the left frontal sinus by a bone-like density, and lateral sinus wall thickening with extension into the left orbit. On surgical exploration and trephination, the left frontal sinus was filled with soft bone which contained multiple pockets of mucopurulent material. Cytologic examination confirmed the presence of a large number of neutrophils, osteoclasts, and osteoblasts; and both extracellular and intracellular, filamentous, b

[Osteomyelitis of the ischium with ipsilateral hip dislocation in an infant].

Acute hematogenous osteomyelitis of the os ischii is a rare disease. Involvement of the acetabulum in a 4-month old girl with concomitant developmental dysplasia of the ipsilateral hip caused a dislocation of the hip joint due to synovitis. Arthroscopic joint lavage, antibiotic treatment and reduction of the hip in a pavlik-harness led to a satisfactory result.
