Panniculitis associated with histoplasmosis and alpha 1-antitrypsin deficiency.

A patient with mediastinal histoplasmosis, alpha 1-antitrypsin deficiency, and panniculitis is presented. The patient showed dramatic response to treatment with ketoconazole. The associations of panniculitis with histoplasmosis and alpha 1-antitrypsin deficiency are discussed.

Disseminated Mycobacterium avium-intracellulare infection appearing as a panniculitis.

A 34-year-old woman had wide-spread panniculitis due to a disseminated infection with Mycobacteria avium-intracellulare. The patient had previously received treatment with high dosages of corticosteroids. Suppurative lesions teeming with acid-fast bacilli and without formation of granulomas were found in many organs, including the skin, mediastinum, spleen, liver, and gastrointestinal tract. Both the appearance of disseminated M avium-intracellular infection resembling panniculitis and the involvement of mediastinum have not previously been reported, to our knowledge.

Pancreatitis, panniculitis, and polyarthritis syndrome presentation, diagnosis, and management.

Patient presented with asymmetric polyarthritis and tender skin erythematous lesions over his lower limbs associated with intermittent fever. He has a history of excessive alcohol usage without previous episode of pancreatitis. He subsequently developed abdominal pain and was noted to have elevated lipase levels and computed tomography (CT) of his abdomen showed evidence of acute pancreatitis. Biopsy of a lower limb lesion proved panniculitis. CT scan of his ankle showed arthritis and an insufficiency fracture. The diagnosis of pancreatitis, panniculitis, and polyarthritis (PPP) syndrome was m…

Unusual Presentation of Subcutaneous Panniculitis-Like T-Cell Lymphoma.

Subcutaneous panniculitis-like T-cell lymphoma (SPTCL) is a rare variant of primary cutaneous T-cell lymphoma primarily affecting subcutaneous adipose tissue and mimicking lobular panniculitis. It often presents with erythematous subcutaneous nodules or plaques, and diagnosis can be challenging due to its diverse clinical manifestations. We report an unusual presentation of SPTCL in a 40-year-old female with recurrent, painless ulcerated plaques on the thighs and lower legs, accompanied by systemic symptoms including weight loss, fatigue, and laboratory abnormalities. Initially misdiagnosed as…

Mycobacterium porcinum panniculitis in a cat from northern California.

CASE SUMMARY: A 9-year-old male castrated domestic shorthair cat from northern California was evaluated for a 12-month history of dermal and subcutaneous dermatitis in the inguinal region. Histopathologic examination of a biopsy revealed severe, chronic, multifocal to coalescing pyogranulomatous dermatitis and panniculitis, accompanied by ulceration and central necrosis. Aerobic bacterial culture of lesions yielded mycobacterial growth. Empiric antimicrobial therapy was initiated with doxycycline and marbofloxacin pending culture and susceptibility. Culture of a biopsy followed by rpoB gene se…

An unusual case of acquired generalized lipodystrophy (panniculitis variety).

Acquired generalized lipodystrophy (AGL) is a rare disease characterized by variable loss of adipose tissue and concurrent metabolic derangements, typically with childhood or adolescent onset. AGL has three subclassifications: panniculitis (type 1), autoimmune disease (type 2), and idiopathic (type 3). This report highlights a rare case of AGL type 1 in a previously healthy 3-year-old female who presented with diffuse erythematous subcutaneous nodules, progressive lipoatrophy, and histopathological findings of a lobular panniculitis.

Subcutaneous Panniculitis-Like T-Cell Lymphoma With Hemophagocytic Lymphohistiocytosis.

Subcutaneous panniculitis-like T-cell lymphoma (SPTLP), a unique variant of primary cutaneous T-cell lymphomas, clinically mimics subcutaneous panniculitis. It is typified by the development of multiple plaques or subcutaneous erythematous nodules, predominantly on the extremities and trunk. Epidemiological findings reveal a greater incidence in females than males, affecting a wide demographic, including pediatric and adult cohorts, with a median onset age of around 30 years. Diagnosis of SPTLP is complex, hinging on skin biopsy analyses and the identification of T-cell lineage-specific immuno…
